Make POSIX threads improvements
- Ensure SIGTHR isn't blocked in newly created threads
- Use TIB rather than thread_local for thread atexits
- Make POSIX thread keys atomic within thread
- Don't bother logging prctl() to --strace
- Log thread destructor names to --strace

flip et / noet in modelines
`et` means `expandtab`.

Fix vi modelines (#989)
At least in neovim, `│vi:` is not recognized as a modeline because it
has no preceding whitespace. After fixing this, opening a file yields
an error because `net` is not an option. (`noet`, however, is.)

Hunt down more bugs
After going through the MODE=dbg and MODE=zero build modes, a bunch of
little issues were identified, which have been addressed. Fixing those
issues created even more troubles for the project, because it improved
our ability to detect latent problems which are getting fixed so fast.
