xen: update ring.h

Update include/xen/interface/io/ring.h to its newest version.

XEN_RING_NR_UNCONSUMED_RESPONSES() in order to avoid the nasty
XEN_RING_HAS_UNCONSUMED_IS_BOOL #define.
