plpks_is_available() can be called on any platform via kexec but calls
_plpks_get_config() which makes a hcall, which will only work on pseries.
Fix this by returning early in plpks_is_available() if hcalls aren't
possible.

The plpks driver uses the H_PKS_GET_CONFIG hcall to retrieve configuration
and status information about the PKS from the hypervisor.
Update _plpks_get_config() to handle some additional fields. Add getter
functions to allow the PKS configuration information to be accessed from
other files. Validate that the values we're getting comply with the spec.
While we're here, move the config struct in _plpks_get_config() off the
stack - it's getting large and we also need to make sure it doesn't cross
a page boundary.

A number of structures and buffers passed to PKS hcalls have alignment
requirements, which could on occasion cause problems:
- Authorisation structures must be 16-byte aligned and must not cross a
page boundary
- Label structures must not cross page boundaries
- Password output buffers must not cross page boundaries
To ensure correct alignment, we adjust the allocation size of each of
these structures/buffers to be the closest power of 2 that is at least the
size of the structure/buffer (since kmalloc() guarantees that an
allocation of a power of 2 size will be aligned to at least that size).

kexec on pseries disables AIL (reloc_on_exc), required for scv
instruction support, before other CPUs have been shut down. This means
they can execute scv instructions after AIL is disabled, which causes an
interrupt at an unexpected entry location that crashes the kernel.
Change the kexec sequence to disable AIL after other CPUs have been
brought down.
As a refresher, the real-mode scv interrupt vector is 0x17000, and the
fixed-location head code probably couldn't easily deal with implementing
such high addresses so it was just decided not to support that interrupt
at all.

It's not an error or exceptional situation when the hosting
environment does not expose a name for the LP/guest via RTAS or the
device tree. This happens with qemu when run without the '-name'
option. The message also lacks a newline. Remove it.

At the time of LPAR boot up, partition firmware provides Open Firmware
property ibm,dma-window for the PE. This property is provided on the PCI
bus the PE is attached to.
There are execptions where the partition firmware might not provide this
property for the PE at the time of LPAR boot up. One of the scenario is
where the firmware has frozen the PE due to some error condition. This
PE is frozen for 24 hours or unless the whole system is reinitialized.
Within this time frame, if the LPAR is booted, the frozen PE will be
presented to the LPAR but ibm,dma-window property could be missing.
Today, under these circumstances, the LPAR oopses with NULL pointer
dereference, when configuring the PCI bus the PE is attached to.

Currently, plpks_confirm_object_flushed() function polls for 5msec in
total instead of 5sec.
Keep max polling time consistent for all the H_CALLs, which take longer
than expected, to be 5sec. Also, make use of fsleep() everywhere to
insert delay.

At the time of LPAR dump, before kexec hands over control to kdump
kernel, DDWs (Dynamic DMA Windows) are scanned and added to the FDT.
For the SR-IOV case, default DMA window "ibm,dma-window" is removed from
the FDT and DDW added, for the device.
Now, kexec hands over control to the kdump kernel.
When the kdump kernel initializes, PCI busses are scanned and IOMMU
group/tables created, in pci_dma_bus_setup_pSeriesLP(). For the SR-IOV
case, there is no "ibm,dma-window". The original commit: b1fc44eaa9,
fixes the path where memory is pre-mapped (direct mapped) to the DDW.
When TCEs are direct mapped, there is no need to initialize IOMMU
tables.
iommu_table_setparms_lpar() only considers "ibm,dma-window" property
when initiallizing IOMMU table. In the scenario where TCEs are
dynamically allocated for SR-IOV, newly created IOMMU table is not
initialized. Later, when the device driver tries to enter TCEs for the
SR-IOV device, NULL pointer execption is thrown from iommu_area_alloc().
The fix is to initialize the IOMMU table with DDW property stored in the
FDT. There are 2 points to remember:
1. For the dedicated adapter, kdump kernel would encounter both
default and DDW in FDT. In this case, DDW property is used to
initialize the IOMMU table.
2. A DDW could be direct or dynamic mapped. kdump kernel would
initialize IOMMU table and mark the existing DDW as
"dynamic". This works fine since, at the time of table
initialization, iommu_table_clear() makes some space in the
DDW, for some predefined number of TCEs which are needed for
kdump to succeed.

powerVM hypervisor updates the VPA fields with stolen time data.
It currently reports enqueue_dispatch_tb and ready_enqueue_tb for
this purpose. In linux these two fields are used to report the stolen time.
The VPA fields are updated at the TB frequency. On powerPC its mostly
set at 512Mhz. Hence this needs a conversion to ns when reporting it
back as rest of the kernel timings are in ns. This conversion is already
handled in tb_to_ns function. So use that function to report accurate
stolen time.
Observed this issue and used an Capped Shared Processor LPAR(SPLPAR) to
simplify the experiments. In all these cases, 100% VP Load is run using
stress-ng workload. Values of stolen time is in percentages as reported
by mpstat. With the patch values are close to expected.

dlpar_memory_remove_by_index() may access beyond the bounds of the
drmem lmb array when the LMB lookup fails to match an entry with the
given DRC index. When the search fails, the cursor is left pointing to
&drmem_info->lmbs[drmem_info->n_lmbs], which is one element past the
last valid entry in the array. The debug message at the end of the
function then dereferences this pointer:

This happens because the copy is targeting item->msg, which is only 4
bytes in size, even though the enclosing item was allocated with extra
space following the msg.
To fix the warning define struct opal_prd_msg with a union of the header
and a flex array, and have the memcpy target the flex array.

The VAS open window call prints error message and returns -EBUSY
after the migration suspend event initiated and until the resume
event completed on the destination system. It can cause the log
buffer filled with these error messages if the user space issues
continuous open window calls. Similar case even for DLPAR CPU
remove event when no credits are available until the credits are
freed or with the other DLPAR CPU add event.
So changes in the patch to use pr_err_ratelimited() instead of
pr_err() to display open window failure and not-available credits
error messages.
Use pr_fmt() and make the corresponding changes to have the
consistencein prefix all pr_*() messages (vas-api.c).

If device_register() returns error in ibmebus_bus_init(), name of kobject
which is allocated in dev_set_name() called in device_add() is leaked.
As comment of device_add() says, it should call put_device() to drop
the reference count that was set in device_initialize() when it fails,
so the name can be freed in kobject_cleanup().

lppaca_shared_proc() takes a pointer to the lppaca which is typically
accessed through get_lppaca(). With DEBUG_PREEMPT enabled, this leads
to checking if preemption is enabled, for example:

This isn't actually a problem however, as it does not matter which
lppaca is accessed, the shared proc state will be the same.
vcpudispatch_stats_procfs_init() already works around this by disabling
preemption, but the lparcfg code does not, erroring any time
/proc/powerpc/lparcfg is accessed with DEBUG_PREEMPT enabled.
Instead of disabling preemption on the caller side, rework
lppaca_shared_proc() to not take a pointer and instead directly access
the lppaca, bypassing any potential preemption checks.

On a powermac platform, via the call path:
start_kernel()
time_init()
ppc_md.calibrate_decr() (pmac_calibrate_decr)
via_calibrate_decr()
ioremap() and iounmap() are called. The unmap can enable interrupts
unexpectedly (cond_resched() in vunmap_pmd_range()), which causes a
warning later in the boot sequence in start_kernel().
Use the early_* variants of these IO functions to prevent this.
The issue is pre-existing, but is surfaced by commit 721255b982
("genirq: Use a maple tree for interrupt descriptor management").

Commit 8ef7b9e176 ("powerpc/pseries/vas: Close windows with DLPAR
core removal") unmaps the window paste address and issues HCALL to
close window in the hypervisor for migration or DLPAR core removal
events. So holds mmap_mutex and then mmap lock before unmap the
paste address. But if the user space issue mmap paste address at
the same time with the migration event, coproc_mmap() is called
after holding the mmap lock which can trigger deadlock when trying
to acquire mmap_mutex in coproc_mmap().

Currently in tce_freemulti_pSeriesLP() there is no limit on how many
TCEs are passed to the H_STUFF_TCE hcall. This has not caused an issue
until now, but newer firmware releases have started enforcing a limit of
512 TCEs per call.
The limit is correct per the specification (PAPR v2.12 § 14.5.4.2.3).
The code has been in it's current form since it was initially merged.

For an SR-IOV device, while enabling DDW, a new table is created and
added at index 1 in the group. In the below 2 scenarios, the table is
incorrectly referenced at index 0 (which is where the table is for
default DMA window).
1. When adding DDW
This issue is exposed with "slub_debug". Error thrown out from
dma_iommu_dma_supported()

Platform device helper routines won't update the NUMA distance table
while creating a platform device, even if the device is present on a
NUMA node that doesn't have memory or CPU. This is especially true for
pmem devices. If the target node of the pmem device is not online, we
find the nearest online node to the device and associate the pmem device
with that online node. To find the nearest online node, we should have
the numa distance table updated correctly. Update the distance
information during the device probe.
For a papr scm device on NUMA node 3 distance_lookup_table value for
distance_ref_points_depth = 2 before and after fix is below:

The hypervisor supports user-mode NX from Power10.
pseries_vas_dlpar_cpu() is called from lparcfg_write() to update VAS
windows for DLPAR event in shared processor mode and the kernel gets
-ENOTSUPP for HCALLs if the user-mode NX is not supported. The current
VAS implementation also supports only with Radix page tables. Whereas in
dedicated processor mode, pseries_vas_notifier() is registered only if
the copy/paste feature is enabled. So instead of displaying HCALL error
messages, update VAS capabilities if the copy/paste feature is
available.
This patch ignores updating VAS capabilities in pseries_vas_dlpar_cpu()
and returns success if the copy/paste feature is not enabled. Then
lparcfg_write() completes the processor DLPAR operations without any
failures.

Since commit 0069f3d14e ("powerpc/64e: Tie PPC_BOOK3E_64 to
PPC_E500MC"), the only possible BOOK3E/64 are E500, so no need of a
default CPU over the E5500.
When the user selects book3e, they must have an e500 compatible
compiler, and it won't work anymore with the default -mcpu=power64, see
commit d6b551b8f9 ("powerpc/64e: Fix build failure with GCC
12 (unrecognized opcode: `wrteei')").
For book3s/64, replace GENERIC_CPU by POWERPC64_CPU to match the PPC32
POWERPC_CPU, and set a default mpcu value in Kconfig directly.
When a user selects a particular CPU, they must ensure the compiler has
the requested capability. Therefore, remove hidden fallback, instead
offer user the possibility to say they want to use the toolchain
default.

rtas-error-log-max is not the name of an RTAS function, so rtas_token()
is not the appropriate API for retrieving its value. We already have
rtas_get_error_log_max() which returns a sensible value if the property
is absent for any reason, so use that instead.

The hypervisor assigns VAS (Virtual Accelerator Switchboard)
windows depends on cores configured in LPAR. The kernel uses
OF reconfig notifier to reconfig VAS windows for DLPAR CPU event.
In the case of shared CPU mode partition, the hypervisor assigns
VAS windows depends on CPU entitled capacity, not based on vcpus.
When the user changes CPU entitled capacity for the partition,
drmgr uses /proc/ppc64/lparcfg interface to notify the kernel.
This patch adds the following changes to update VAS resources
for shared mode:
- Call vas reconfig windows from lparcfg_write()
- Ignore reconfig changes in the VAS notifier

irq_default_primary_handler() can be used only with IRQF_ONESHOT
flag, but the flag disables IRQ before executing the thread handler
and enables it after the interrupt is handled. But this IRQ disable
sets the VAS IRQ OFF state in the hypervisor. In case if NX faults
during this window, the hypervisor will not deliver the fault
interrupt to the partition and the user space may wait continuously
for the CSB update. So use VAS specific IRQ handler instead of
calling the default primary handler.
Increment pending_faults counter in IRQ handler and the bottom
thread handler will process all faults based on this counter.
In case if the another interrupt is received while the thread is
running, it will be processed using this counter. The synchronization
of top and bottom handlers will be done with IRQTF_RUNTHREAD flag
and will re-enter to bottom half if this flag is set.
