e04480920d Bluetooth: defer cleanup of resources in hci_unregister_dev()
syzbot is hitting might_sleep() warning at hci_sock_dev_event() due to
calling lock_sock() with rw spinlock held [1].

It seems that history of this locking problem is a trial and error.

Commit b40df5743e ("[PATCH] bluetooth: fix socket locking in
hci_sock_dev_event()") in 2.6.21-rc4 changed bh_lock_sock() to
lock_sock() as an attempt to fix lockdep warning.

Then, commit 4ce61d1c7a ("[BLUETOOTH]: Fix locking in
hci_sock_dev_event().") in 2.6.22-rc2 changed lock_sock() to
local_bh_disable() + bh_lock_sock_nested() as an attempt to fix the
sleep in atomic context warning.

Then, commit 4b5dd696f8 ("Bluetooth: Remove local_bh_disable() from
hci_sock.c") in 3.3-rc1 removed local_bh_disable().

Then, commit e305509e67 ("Bluetooth: use correct lock to prevent UAF
of hdev object") in 5.13-rc5 again changed bh_lock_sock_nested() to
lock_sock() as an attempt to fix CVE-2021-3573.

This difficulty comes from current implementation that
hci_sock_dev_event(HCI_DEV_UNREG) is responsible for dropping all
references from sockets because hci_unregister_dev() immediately
reclaims resources as soon as returning from
hci_sock_dev_event(HCI_DEV_UNREG).

But the history suggests that hci_sock_dev_event(HCI_DEV_UNREG) was not
doing what it should do.

Therefore, instead of trying to detach sockets from device, let's accept
not detaching sockets from device at hci_sock_dev_event(HCI_DEV_UNREG),
by moving actual cleanup of resources from hci_unregister_dev() to
hci_cleanup_dev() which is called by bt_host_release() when all
references to this unregistered device (which is a kobject) are gone.

Since hci_sock_dev_event(HCI_DEV_UNREG) no longer resets
hci_pi(sk)->hdev, we need to check whether this device was unregistered
and return an error based on HCI_UNREGISTER flag.  There might be subtle
behavioral difference in "monitor the hdev" functionality; please report
if you found something went wrong due to this patch.

9f90a4ddef tty: drop put_tty_driver
put_tty_driver() is an alias for tty_driver_kref_put(). There is no need
for two exported identical functions, therefore switch all users of
old put_tty_driver() to new tty_driver_kref_put() and remove the former
for good.

39b7b42be4 tty: stop using alloc_tty_driver
alloc_tty_driver was deprecated by tty_alloc_driver in commit
7f0bc6a68e (TTY: pass flags to alloc_tty_driver) in 2012.

I never got into eliminating alloc_tty_driver until now. So we still
have two functions for allocating drivers which might be confusing. So
get rid of alloc_tty_driver uses to eliminate it for good in the next
patch.

Note we need to switch return value checking as tty_alloc_driver uses
ERR_PTR. And flags are now a parameter of tty_alloc_driver.

 A bit more than normal, but nothing major, lots of cleanups.  Highlights
 are:
 	- lots of tty api cleanups and mxser driver cleanups from Jiri
 	- build warning fixes
 	- various serial driver updates
 	- coding style cleanups
 	- various tty driver minor fixes and updates
 	- removal of broken and disable r3964 line discipline (finally!)
 
 All of these have been in linux-next for a while with no reported

23837a6d7a Bluetooth: Fix handling of HCI_LE_Advertising_Set_Terminated event
Error status of this event means that it has ended due reasons other
than a connection:

 'If advertising has terminated as a result of the advertising duration
 elapsing, the Status parameter shall be set to the error code
 Advertising Timeout (0x3C).'

 'If advertising has terminated because the
 Max_Extended_Advertising_Events was reached, the Status parameter
 shall be set to the error code Limit Reached (0x43).'

c9ed0a7077 Bluetooth: Fix Set Extended (Scan Response) Data
These command do have variable length and the length can go up to 251,
so this changes the struct to not use a fixed size and then when
creating the PDU only the actual length of the data send to the
controller.

c32d624640 Bluetooth: disable filter dup when scan for adv monitor
Disable duplicates filter when scanning for advertisement monitor for
the following reasons. The scanning includes active scan and passive
scan.

For HW pattern filtering (ex. MSFT), Realtek and Qualcomm controllers
ignore RSSI_Sampling_Period when the duplicates filter is enabled.

For SW pattern filtering, when we're not doing interleaved scanning, it
is necessary to disable duplicates filter, otherwise hosts can only
receive one advertisement and it's impossible to know if a peer is still
in range.

de75cd0d9b Bluetooth: Add ncmd=0 recovery handling
During command status or command complete event, the controller may set
ncmd=0 indicating that it is not accepting any more commands. In such a
case, host holds off sending any more commands to the controller. If the
controller doesn't recover from such condition, host will wait forever,
until the user decides that the Bluetooth is broken and may power cycles
the Bluetooth.

This patch triggers the hardware error to reset the controller and
driver when it gets into such state as there is no other wat out.

3cfdf8fcaa Bluetooth: cmtp: fix file refcount when cmtp_attach_device fails
When cmtp_attach_device fails, cmtp_add_connection returns the error value
which leads to the caller to doing fput through sockfd_put. But
cmtp_session kthread, which is stopped in this path will also call fput,
leading to a potential refcount underflow or a use-after-free.

Add a refcount before we signal the kthread to stop. The kthread will try
to grab the cmtp_session_sem mutex before doing the fput, which is held
when get_file is called, so there should be no races there.

e305509e67 Bluetooth: use correct lock to prevent UAF of hdev object
The hci_sock_dev_event() function will cleanup the hdev object for
sockets even if this object may still be in used within the
hci_sock_bound_ioctl() function, result in UAF vulnerability.

This patch replace the BH context lock to serialize these affairs
and prevent the race condition.

6a137caec2 Bluetooth: fix the erroneous flush_work() order
In the cleanup routine for failed initialization of HCI device,
the flush_work(&hdev->rx_work) need to be finished before the
flush_work(&hdev->cmd_work). Otherwise, the hci_rx_work() can
possibly invoke new cmd_work and cause a bug, like double free,
in late processings.

This was assigned CVE-2021-3564.

This patch reorder the flush_work() to fix this bug.
