phy: ti: dm816x: Fix the error handling path in 'dm816x_usb_phy_probe()

Add an error handling path in the probe to release some resources, as
already done in the remove function.

Adding vendor specific directories in phy to group
phy drivers under their respective vendor umbrella.
Also updated the MAINTAINERS file to reflect the correct
directory structure for phy drivers.
