This patch adds the full CEX6S card support to the zcrypt device
driver. A CEX6A/C/P is detected and displayed as such, the card
and queue device driver code is updated to recognize it and the
relative weight values for CEX4, CEX5 and CEX6 have been updated.

A few lines down, qeth_prepare_control_data() makes further changes to
the control cmd buffer, and then also writes a trace entry for it.
So the first entry just pollutes the trace file with intermediate data,
drop it.

Switch to napi_complete_done(), and thus enable delayed GRO flushing.
The timeout is configured via /sys/class/net/<if>/gro_flush_timeout.
Default timeout is 0, so no change in behaviour.

Current code bails out when two subsequent buffer elements hold
insufficient data to contain a qeth_hdr packet descriptor.
This seems reasonable, but it would be legal for quirky hardware to
leave a few elements empty and then present packets in a subsequent
element. These packets would currently be dropped.
So make sure to check all buffer elements, until we hit the LAST_ENTRY
indication.

Move the allocation of SG skbs into the main path. This allows for
a little code sharing, and handling ENOMEM from within one place.
As side effect, L2 SG skbs now get the proper amount of additional
headroom (read: zero) instead of the hard-coded ETH_HLEN.

There's no reason why l2_set_mac_address() should ever be called for
a netdevice that's not owned by qeth. It's certainly not required for
VLAN devices, which have their own netdev_ops.
Also:
1) we don't do such validation for any of the other netdev_ops routines.
2) the code in question clearly has never been actually exercised;
it's broken. After determining that the device is not owned
by qeth, it would still use dev->ml_priv to write a qeth trace entry.
Remove the check, and its helper that walked the global card list.

With commit "s390/ccwgroup: tie a ccwgroup driver to its ccw driver",
the ccwgroup core now ensures that a qeth group device only consists of
ccw devices which are supported by qeth. Therefore remove qeth's
internal device matching, and use .driver_info to determine the card
type.

When recovering a device, qeth needs to re-run the IPA commands that
enable all previously active HW features.
Instead of duplicating qeth_set_features(), let netdev_update_features()
recover the missing HW features from dev->wanted_features.

debug_event currently truncates the data if used with a size larger than
the buf_size of the debug feature. For lots of callers of this function,
wrappers have been implemented that loop until all data is handled.
Move that functionality into debug_event_common and get rid of the wrappers.

v4.10 commit 6f2ce1c6af ("scsi: zfcp: fix rport unblock race with LUN
recovery") extended accessing parent pointer fields of struct
zfcp_erp_action for tracing. If an erp_action has never been enqueued
before, these parent pointer fields are uninitialized and NULL. Examples
are zfcp objects freshly added to the parent object's children list,
before enqueueing their first recovery subsequently. In
zfcp_erp_try_rport_unblock(), we iterate such list. Accessing erp_action
fields can cause a NULL pointer dereference. Since the kernel can read
from lowcore on s390, it does not immediately cause a kernel page
fault. Instead it can cause hangs on trying to acquire the wrong
erp_action->adapter->dbf->rec_lock in zfcp_dbf_rec_action_lvl()

while holding already other locks with IRQs disabled.

zfcp_erp_action is always fully embedded into its container object. Such
container object is never moved in its object tree (only add or delete).
Hence, erp_action parent pointers can never change.
To fix the issue, initialize the erp_action parent pointers before
adding the erp_action container to any list and thus before it becomes
accessible from outside of its initializing function.
In order to also close the time window between zfcp_erp_setup_act()
memsetting the entire erp_action to zero and setting the parent pointers
again, drop the memset and instead explicitly initialize individually
all erp_action fields except for parent pointers. To be extra careful
not to introduce any other unintended side effect, even keep zeroing the
erp_action fields for list and timer. Also double-check with
WARN_ON_ONCE that erp_action parent pointers never change, so we get to
know when we would deviate from previous behavior.

We currently return the same error code (-EFAULT) to indicate two
different error cases:
1. a bug in vfio-ccw implementation has been found.
2. a buggy channel program has been detected.
This brings difficulty for userland program (specifically Qemu) to
handle.
Let's use -EFAULT to only indicate the first case. For the second
case, we simply hand over the ccws to lower level for further
handling.
Notice:
Once a bad idaw address is detected, the current behavior is to
suppress the ssch. With this fix, the channel program will be
accepted, and part of the channel program (the part ahead of
the bad idaw) could possibly be executed by the device before
I/O conclusion.

Remove uses of init_timer_on_stack() with open-coded function and data
assignments that could be expressed using timer_setup_on_stack(). Several
were removed from the stack entirely since there was a one-to-one mapping
of parent structure to timer, those are switched to using timer_setup()
instead. All related callbacks were adjusted to use from_timer().

There is no recent user space application available anymore which still
supports this old virtio transport. Additionally, commit 3b2fbb3f06
("virtio/s390: deprecate old transport") introduced a deprecation message
in the driver, and apparently nobody complained so far that it is still
required. So let's simply remove it.

When grouping devices, the ccwgroup core only checks whether all of the
devices are bound to the same ccw_driver. It has no means of checking
if the requesting ccwgroup driver actually supports this device type.
qeth implements its own device matching in qeth_core_probe_device(),
while ctcm and lcs currently have no sanity-checking at all.
Enable ccwgroup drivers to optionally defer the device type checking to
the ccwgroup core, by specifying their supported ccw_driver.
This allows us drop the device type matching from qeth, and improves
the robustness of ctcm and lcs.

The function to prepare MEX type 50 ap messages did
not explicitly check for the data length in case of
data > 512 bytes. Instead the function assumes the
boundary check done in the ioctl function will always
reject requests with invalid data length values.
However, screening just the function code may give the
illusion, that there may be a gap which could be
exploited by userspace for buffer overwrite attacks.

All (but one) cmf related sysfs attributes have been converted to read
directly from the measurement block using cmf_read. This is not
possible for the avg_utilization attribute since this is an aggregation
of several values for which cmf_read only returns average values.
Move the computation of the utilization value to the cmf_read interface
such that it can use the raw data.

To ensure data consistency when reading from the channel measurement
block we wait for the subchannel to become idle and copy the whole
block. This is unnecessary when all we do is export the individual
values via sysfs. Read the values for sysfs export directly from
the measurement block.

cmf_copy_block tries to ensure data consistency by copying the
channel measurement block twice and comparing the data. This was
needed on very old machines only. Nowadays we guarantee
consistency by copying the data when the channel is idle.

When enabling channel measurement fails with a busy condition we wait
for the next interrupt to arrive before we retry the operation. For
devices which usually don't create interrupts we wait forever.
Although the waiting is done interruptible that behavior is not
expected and confused some users. Abort the operation after a 10s
timeout.

In some situations we don't receive notification from firmware that
a previously unusable channelpath is usable again.
Schedule recovery for devices that return from path verification
without using all potentially usable paths. The recovery thread will
periodically trigger a path verification on the affected devices.

Locking the output queue prior to TX is needed on OSA devices,
to synchronize against a packing flush from the TX completion code
(via qeth_check_outbound_queue()).
But send_packet_fast() is only used for IQDs, which don't do packing.
So remove the locking, and apply some easy cleanups.

HiperSockets allow configuring so called VNIC Characteristics (VNICC)
that influence how the underlying hardware handles packets. For VNICCs,
additional commands for getting and setting timeouts are available.
Currently, the learning VNICC uses these commands.
* Learning VNICC: If learning is enabled on a qeth device, the device
learns the source MAC addresses of outgoing packets and incoming
packets to those learned MAC addresses are received.
For learning, the timeout specifies the idle period in seconds, after
which the underlying hardware removes a learned MAC address again.
This patch adds support for the IPA commands that are required to get

HiperSocket devices allow enabling and disabling so called VNIC
Characteristics (VNICC) that influence how the underlying hardware
handles packets. These VNICCs are:
* Flooding VNICC: Flooding allows specifying if packets to unknown
destination MAC addresses are received by the qeth device.
* Multicast flooding VNICC: Multicast flooding allows specifying if
packets to multicast MAC addresses are received by the qeth device.
* Learning VNICC: If learning is enabled on a qeth device, the device
learns the source MAC addresses of outgoing packets and incoming
packets to those learned MAC addresses are received.
* Takeover setvmac VNICC: If takeover setvmac is configured on a qeth
device, the MAC address of this device can be configured on a
different qeth device with the setvmac IPA command.
* Takeover by learning VNICC: If takeover learning is enabled on a qeth
device, the MAC address of this device can be learned (learning VNICC)
on a different qeth device.
* BridgePort invisible VNICC: If BridgePort invisible is enabled on a
qeth device, (1) packets from this device are not sent to a BridgePort
enabled qeth device and (2) packets coming from a BridgePort enabled
qeth device are not received by this device.
* Receive broadcast VNICC: Receive broadcast allows configuring if a
qeth device receives packets with the broadcast destination MAC
address.
This patch adds support for the IPA commands that are required to enable
and disable these VNIC characteristics on qeth devices. As a
prerequisite, it also adds the query commands IPA command.
The query commands IPA command allows requesting the supported commands
for each characteristic from the underlying hardware.
Additionally, this patch provides users with a sysfs user interface to
enable/disable the VNICCs mentioned above.

VNIC Characteristics (VNICC) are features of HiperSockets that define
how packets are handled by the underlying network hardware. For example,
if the VNICC flooding is configured on a qeth device, ethernet frames to
unknown destination MAC addresses are received.
Currently, there is support for seven VNICCs: flooding, multicast
flooding, receive broadcast, learning, takeover learning, takeover
setvmac, bridge invisible. Also, six IPA commands exist for configuring
VNICCs on a qeth device: query characteristics, query commands, enable
characteristic, disable characteristic, set timeout, get timeout.
This patch adds the basic code infrastructure for VNICC support to qeth.
It allows querying VNICC support from the underlying hardware. To this
end, it adds:
* basic message formats for IPA commands
* basic data structures
* basic error handling
* query characteristics IPA command support
The query characteristics IPA command allows requesting the currently
supported and currently enabled VNIC characteristics from the underlying
hardware.
Support for the other IPA commands and for the configuration of VNICCs
is added in follow-up patches together with the respective user
interface functions.

KVM has a need to fetch the crypto configuration information
as it is returned by the PQAP(QCI) instruction. This patch

Under certain specified conditions, the Test AP Queue (TAPQ)
subfunction of the Process Adjunct Processor Queue (PQAP) instruction
will be intercepted by a guest VM. The guest VM must have a means for
executing the intercepted instruction.
The vfio_ap driver will provide an interface to execute the
PQAP(TAPQ) instruction subfunction on behalf of a guest VM.
The code for executing the AP instructions currently resides in the
AP bus. This patch refactors the AP bus code to externalize access
to the PQAP(TAPQ) instruction subfunction to make it available to
the vfio_ap driver.

The z/VM hypervisor provides virtual disks (VDISK) which are backed by
main memory of the hypervisor. Those devices are seen as DASD FBA disks
within the Linux guest.
Whenever data is written to such a device, memory is allocated
on-the-fly by z/VM accordingly. This memory, however, is not being freed
if data on the device is deleted by the guest OS.
In order to make memory usable after deletion again, add discard support
to the FBA discipline.
While at it, update comments regarding the DASD_FEATURE_* flags.

This way we don't need a block_device structure to submit I/O. The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open. Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).
For the actual I/O path all that we need is the gendisk, which exists
once per block device. But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.
Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

Unsigned long long and unsigned long were different in size for 31-bit.
For 64-bit the size for both datatypes is 8 Bytes and since the support
for 31-bit is long gone we can clean up a little and change everything
to unsigned long.
Change get_phys_clock() along the way to accept unsigned long as well so
that the DASD code can be consistent.

Taking a full copy via skb_realloc_headroom() on every xmit is overkill
and wastes CPU time; all we actually need is to push on the qeth_hdr.
So rework the L2 OSA TX path to avoid the copy.
Minor complications arise because struct qeth_hdr must not cross a page
boundary. So add a new helper qeth_push_hdr() that catches this, and
falls back to the hdr cache that we already use for IQDs.
This change uncovered that qeth's TX completion takes rather long.
Now that we no longer free the original skb straight away and thus call
skb->destructor later than before, throughput regresses significantly.
For now, restore old behaviour by adding an explicit skb_orphan(),
and a big TODO to improve the TX completion time.

For TSO we need to skip the skb's qeth/IP/TCP headers when mapping
it into buffer elements. Instead of (mis)using skb_pull(), pass a
corresponding offset to fill_buffer() like we already do for IQDs.
No actual change in the resulting TSO buffers.

future changes will require more flexibility for OSA as well. The
device-specific path knows best what header length it requires, so just
pass it from there.

IP-address setting and removal are delayed when the device is not yet in
state SOFTSETUP or UP. ref_counter has been implemented only for
ip-address with type normal. In this patch ref_counter logic is also used
for ip-address with type rxip to allow appropriate handling of multiple
postponed rxip add and del calls.

There exist different commands to add unicast and multicast addresses on
the OSA card. rxip addresses are always set as unicast addresses and
thus just unicast addresses should be allowed.
Adding a multicast address now fails and a grace message is generated.

For some xmit paths we pass down a data offset to qeth_fill_buffer(),
to indicate that the first k bytes of the skb should be skipped when
mapping it into buffer elements.
Commit acd9776b5c ("s390/qeth: no ETH header for outbound AF_IUCV")
recently switched the offset for the IUCV-over-HiperSockets path
from 0 to ETH_HLEN, and now we have
device offset
OSA = 0
IQD > 0
for all xmit paths.
OSA would previously pass down -1 from do_send_packet(), to distinguish
between 1) OSA and 2) IQD with offset 0. That's no longer needed now,
so have it pass 0, make the offset unsigned and clean up how we apply
the offset in __qeth_fill_buffer().
No change of behaviour for any of our current xmit paths.

After transmitting a skb via send_packet[_fast](), the statistics
code accesses the skb once more to account for transmitted page frags.
This has a (theoretical?) race against the TX completion - if the TX
completion is processed and frees the skb before hard_start_xmit()
gets to the statistics part, we access random memory.
Fix this by caching the # of page frags, before the skb is transmitted.

This patch adds early checks to avoid burning CPU cycles on
the assembly of trace entries which would be skipped anyway.
Introduce a static const variable to keep the trace level to check with
debug_level_enabled() in sync with the actual trace emit with
debug_event(). In order not to refactor the SAN tracing too much,
simply use a define instead.
This change is only for the non / semi hot paths,
while the actual (I/O) hot path was already improved earlier:
zfcp_dbf_scsi() is already guarded by its only caller _zfcp_dbf_scsi()
since commit dcd20e2316 ("[SCSI] zfcp: Only collect SCSI debug data for
matching trace levels").
zfcp_dbf_hba_fsf_res() is already guarded by its only caller
zfcp_dbf_hba_fsf_response() since commit 2e261af84c ("[SCSI] zfcp: Only
collect FSF/HBA debug data for matching trace levels").

s390 is big endian, this has been working correctly without conversions
and all the new conversions are NOPs so no performance impact.
Nonetheless, use the conversion on the constant expression where possible.

This eases crash dump analysis by automatically dissecting these
protocol headers at least somewhat rather than getting a string
interpretation of large unstructured character array buffer fields.
Also, we can get rid of some unnecessary and error-prone type casts.

v2.6.33 commit dbf5dfe9db ("[SCSI] zfcp: Use common code definitions for
FC CT structs") replaced own definitions with common code definitions.
While FC_BA_RJT_UNABLE happens to be defined with the same value 9 as
FC_FS_RJT_UNABL and thus also works, here we should use the latter from
fc_gs.h.

Complements debugging aspects of the otherwise functionally complete
v3.17 commit 9cb78c16f5 ("scsi: use 64-bit LUNs").
While I don't have access to a target exporting 3 or 4 level LUNs,
I did test it by explicitly attaching a non-existent fake 4 level LUN
by means of zfcp sysfs attribute "unit_add".
In order to see corresponding trace records of otherwise successful
events, we had to increase the trace level of area SCSI and HBA to 6.

At the default trace level, we only trace unsuccessful events including
FSF responses.
zfcp_dbf_hba_fsf_response() only used protocol status and FSF status to
decide on an unsuccessful response. However, this is only one of multiple
possible sources determining a failed struct zfcp_fsf_req.
An FSF request can also "fail" if its response runs into an ERP timeout
or if it gets dismissed because a higher level recovery was triggered
[trace tags "erscf_1" or "erscf_2" in zfcp_erp_strategy_check_fsfreq()].
FSF requests with ERP timeout are:
FSF_QTCB_EXCHANGE_CONFIG_DATA, FSF_QTCB_EXCHANGE_PORT_DATA,
FSF_QTCB_OPEN_PORT_WITH_DID or FSF_QTCB_CLOSE_PORT or
FSF_QTCB_CLOSE_PHYSICAL_PORT for target ports,
FSF_QTCB_OPEN_LUN, FSF_QTCB_CLOSE_LUN.
One example is slow queue processing which can cause follow-on errors,
e.g. FSF_PORT_ALREADY_OPEN after FSF_QTCB_OPEN_PORT_WITH_DID timed out.
In order to see the root cause, we need to see late responses even if the
channel presented them successfully with FSF_PROT_GOOD and FSF_GOOD.

In case of problems detecting that new responses are waiting on the input
queue, we sooner or later trigger adapter recovery due to an FSF request
timeout (trace tag "fsrth_1").
FSF requests with FSF request timeout are:
typically FSF_QTCB_ABORT_FCP_CMND; but theoretically also
FSF_QTCB_EXCHANGE_CONFIG_DATA or FSF_QTCB_EXCHANGE_PORT_DATA via sysfs,
FSF_QTCB_OPEN_PORT_WITH_DID or FSF_QTCB_CLOSE_PORT for WKA ports,
FSF_QTCB_FCP_CMND for task management function (LUN / target reset).
One or more pending requests can meanwhile have FSF_PROT_GOOD and FSF_GOOD
because the channel filled in the response via DMA into the request's QTCB.
In a theroretical case, inject code can create an erroneous FSF request
on purpose. If data router is enabled, it uses deferred error reporting.
A READ SCSI command can succeed with FSF_PROT_GOOD, FSF_GOOD, and
SAM_STAT_GOOD. But on writing the read data to host memory via DMA,
it can still fail, e.g. if an intentionally wrong scatter list does not
provide enough space. Rather than getting an unsuccessful response,
we get a QDIO activate check which in turn triggers adapter recovery.
One or more pending requests can meanwhile have FSF_PROT_GOOD and FSF_GOOD
because the channel filled in the response via DMA into the request's QTCB.

Only with luck in both above cases, we could see a follow-on trace record
of an unsuccesful event following a successful but late FSF response with
FSF_PROT_GOOD and FSF_GOOD. Typically this was the case for I/O requests
resulting in a SCSI trace record "rsl_err" with DID_TRANSPORT_DISRUPTED
[On ZFCP_STATUS_FSFREQ_DISMISSED, zfcp_fsf_protstatus_eval() sets
ZFCP_STATUS_FSFREQ_ERROR seen by the request handler functions as failure].
However, the reason for this follow-on trace was invisible because the
corresponding HBA trace record was missing at the default trace level
(by default hidden records with tags "fs_norm", "fs_qtcb", or "fs_open").
On adapter recovery, after we had shut down the QDIO queues, we perform
unsuccessful pseudo completions with flag ZFCP_STATUS_FSFREQ_DISMISSED
for each pending FSF request in zfcp_fsf_req_dismiss_all().
In order to find the root cause, we need to see all pseudo responses even
if the channel presented them successfully with FSF_PROT_GOOD and FSF_GOOD.
Therefore, check zfcp_fsf_req.status for ZFCP_STATUS_FSFREQ_DISMISSED
or ZFCP_STATUS_FSFREQ_ERROR and trace with a new tag "fs_rerr".
It does not matter that there are numerous places which set
ZFCP_STATUS_FSFREQ_ERROR after the location where we trace an FSF response
early. These cases are based on protocol status != FSF_PROT_GOOD or
== FSF_PROT_FSF_STATUS_PRESENTED and are thus already traced by default
as trace tag "fs_perr" or "fs_ferr" respectively.
NB: The trace record with tag "fssrh_1" for status read buffers on dismiss
all remains. zfcp_fsf_req_complete() handles this and returns early.
All other FSF request types are handled separately and as described above.

If the FCP_RSP UI has optional parts (FCP_SNS_INFO or FCP_RSP_INFO) and
thus does not fit into the fsp_rsp field built into a SCSI trace record,
trace the full FCP_RSP UI with all optional parts as payload record
instead of just FCP_SNS_INFO as payload and
a 1 byte RSP_INFO_CODE part of FCP_RSP_INFO built into the SCSI record.
That way we would also get the full FCP_SNS_INFO in case a
target would ever send more than
min(SCSI_SENSE_BUFFERSIZE==96, ZFCP_DBF_PAY_MAX_REC==256)==96.
The mandatory part of FCP_RSP IU is only 24 bytes.
PAYload costs at least one full PAY record of 256 bytes anyway.
We cap to the hardware response size which is only FSF_FCP_RSP_SIZE==128.
So we can just put the whole FCP_RSP IU with any optional parts into
PAYload similarly as we do for SAN PAY since v4.9 commit aceeffbb59
("zfcp: trace full payload of all SAN records (req,resp,iels)").
This does not cause any additional trace records wasting memory.
Decoded trace records were confusing because they showed a hard-coded
sense data length of 96 even if the FCP_RSP_IU field FCP_SNS_LEN showed
actually less.
Since the same commit, we set pl_len for SAN traces to the full length of a
request/response even if we cap the corresponding trace.
In contrast, here for SCSI traces we set pl_len to the pre-computed
length of FCP_RSP IU considering SNS_LEN or RSP_LEN if valid.
Nonetheless we trace a hardcoded payload of length FSF_FCP_RSP_SIZE==128
if there were optional parts.
This makes it easier for the zfcpdbf tool to format only the relevant
part of the long FCP_RSP UI buffer. And any trailing information is still
available in the payload trace record just in case.
Rename the payload record tag from "fcp_sns" to "fcp_riu" to make the new
content explicit to zfcpdbf which can then pick a suitable field name such
as "FCP rsp IU all:" instead of "Sense info :"
Also, the same zfcpdbf can still be backwards compatible with "fcp_sns".

For problem determination we need to see that we were in scsi_eh
as well as whether and why we were successful or not.
The following commits introduced new early returns without adding
a trace record:

on fc_block_scsi_eh() returning != 0 which is FAST_IO_FAIL,

attempts and thus could not issue a TMF and has to return FAILED.

Without this fix we get SCSI trace records on task management functions
which cannot be correlated to HBA trace records because all fields
related to the FSF request are empty (zero).
Also, the FCP_RSP_IU is missing as well as any sense data if available.
This was caused by v2.6.14 commit 8a36e4532e ("[SCSI] zfcp: enhancement
of zfcp debug features") introducing trace records for TMFs but
hard coding NULL for a possibly existing TMF FSF request.
The scsi_cmnd scribble is also zero or unrelated for the TMF request
so it also could not lookup a suitable FSF request from there.

There are some true cases where we really do not have an FSF request:
"rsl_fai" from zfcp_dbf_scsi_fail_send() called for early
returns / completions in zfcp_scsi_queuecommand(),
"abrt_or", "abrt_bl", "abrt_ru", "abrt_ar" from
zfcp_scsi_eh_abort_handler() where we did not get as far,
"lr_nres", "tr_nres" from zfcp_task_mgmt_function() where we're
successful and do not need to do anything because adapter stopped.
For these cases it's correct to pass NULL for fsf_req to _zfcp_dbf_scsi().

("[SCSI] zfcp: Redesign of the debug tracing for SAN records.")
necessary for problem determination, e.g. to see the
currently active zone set during automatic port scan.
While it already saves space by not dumping any empty residual entries
of the large successful GPN_FT response (4 pages), there are seldom cases
where the GPN_FT response is unsuccessful and likely does not have
FC_NS_FID_LAST set in fp_flags so we did not cap the trace record.
We typically see such case for an initiator WWPN, which is not in any zone.
Cap unsuccessful responses to at least the actual basic CT_IU response
plus whatever fits the SAN trace record built-in "payload" buffer
just in case there's trailing information
of which we would at least see the existence and its beginning.
In order not to erroneously cap successful responses, we need to swap
calling the trace function and setting the CT / ELS status to success (0).

Up until now zfcp would just ignore the FCP_RESID_OVER flag in the FCP
response IU. When this flag is set, it is possible, in regards to the
FCP standard, that the storage-server processes the command normally, up
to the point where data is missing and simply ignores those.
In this case no CHECK CONDITION would be set, and because we ignored the
FCP_RESID_OVER flag we resulted in at least a data loss or even
-corruption as a follow-up error, depending on how the
applications/layers on top behave. To prevent this, we now set the
host-byte of the corresponding scsi_cmnd to DID_ERROR.
Other storage-behaviors, where the same condition results in a CHECK
CONDITION set in the answer, don't need to be changed as they are
handled in the mid-layer already.

Since commit db007fc5e2 ("[SCSI] Command protection operation"),
scsi_eh_prep_cmnd() saves scmd->prot_op and temporarily resets it to
SCSI_PROT_NORMAL.
Other FCP LLDDs such as qla2xxx and lpfc shield their queuecommand()
to only access any of scsi_prot_sg...() if
(scsi_get_prot_op(cmd) != SCSI_PROT_NORMAL).
Do the same thing for zfcp, which introduced DIX support with
commit ef3eb71d8b ("[SCSI] zfcp: Introduce experimental support for
DIF/DIX").
Otherwise, TUR SCSI commands as part of scsi_eh likely fail in zfcp,
because the regular SCSI command with DIX protection data, that scsi_eh
re-uses in scsi_send_eh_cmnd(), of course still has
(scsi_prot_sg_count() != 0) and so zfcp sends down bogus requests to the
FCP channel hardware.
This causes scsi_eh_test_devices() to have (finish_cmds == 0)
[not SCSI device is online or not scsi_eh_tur() failed]
so regular SCSI commands, that caused / were affected by scsi_eh,
are moved to work_q and scsi_eh_test_devices() itself returns false.
In turn, it unnecessarily escalates in our case in scsi_eh_ready_devs()
beyond host reset to finally scsi_eh_offline_sdevs()
which sets affected SCSI devices offline with the following kernel message:
"kernel: sd H:0:T:L: Device offlined - not ready after error recovery"

inclusion of linux/miscdevice.h is unnecessary.

Since commit caf7df1227 ("block: remove the errors field from struct request")
rq->errors can't be (mis)used by block device drivers to store the error
condition for usage during async completion. Because of that I simply used
async completion only for the non-error paths.
This patch places the error within the private data of struct request and
uses async completion for all paths again.

Return -ENOTTY for unknown ioctl commands instead of
-ENOIOCTLCMD. This isn't that much of difference, since common code
will translate -ENOIOCTLCMD to -ENOTTY anyway, but this way it seems

If memory is fragmented it is unlikely that large order memory
allocations succeed. This has been an issue with the vmcp device
driver since a long time, since it requires large physical contiguous
memory ares for large responses.
To hopefully resolve this issue make use of the contiguous memory
allocator (cma). This patch adds a vmcp specific vmcp cma area with a
default size of 4MB. The size can be changed either via the
VMCP_CMA_SIZE config option at compile time or with the "vmcp_cma"
kernel parameter (e.g. "vmcp_cma=16m").
For any vmcp response buffers larger than 16k memory from the cma area
will be allocated. If such an allocation fails, there is a fallback to
the buddy allocator.

According to the CP Programming Services manual Diagnose Code 8
"Virtual Console Function" can be used in all addressing modes. Also
the input and output buffers do not have a limitation which specifies
they need to be below the 2GB line.
This is true at least since z/VM 5.4.
Therefore remove the sam31/64 instructions and allow for simple
GFP_KERNEL allocations. This makes it easier to allocate a 1MB page
if the user requested such a large return buffer.

The vmcp device driver should return -EFAULT if get_user() fails, due
to an invalid user space address. In addition the buffer size value
from user space is passed unchecked to get_order(). The return value
of get_order(0) undefined.
Therefore explicitly test for zero before calling get_order() and also
return -EFAULT if get_user() fails.

On L3, the qeth_hdr struct needs to be filled with the next-hop
IP address.
The current code accesses rtable->rt_gateway without checking that
rtable is a valid address. The accidental access to a lowcore area
results in a random next-hop address in the qeth_hdr.
rtable (or more precisely, skb_dst(skb)) can be NULL in rare cases
(for instance together with AF_PACKET sockets).
This patch adds the missing NULL-ptr checks.

When we are translating channel data addresses from guest to host
address space for TIC instructions we are getting incorrect
addresses because of a pointer arithmetic error.
We currently calculate the offset of the TIC's cda from the start
of the channel program chain (ccw->cda - ccw_head). We then add
that to the address of the ccw chain in host memory (iter->ch_ccw).
The problem is that iter->ch_ccw is a pointer to struct ccw1 so
when we increment it we are actually incrementing by the size of
struct ccw1 which is 8 bytes. The intent was to increment by
n-bytes, not n*8.
The fix: cast iter->ch_ccw to char* so it will be incremented by
n*1.

When channel path is identified as the report source code (RSC)
of a CRW, and initialized (CRW_ERC_INIT) is recognized as the
error recovery code (ERC) by the channel subsystem, it indicates
a "path has come" event.

__GFP_REPEAT was designed to allow retry-but-eventually-fail semantic to
the page allocator. This has been true but only for allocations
requests larger than PAGE_ALLOC_COSTLY_ORDER. It has been always
ignored for smaller sizes. This is a bit unfortunate because there is
no way to express the same semantic for those requests and they are
considered too important to fail so they might end up looping in the
page allocator for ever, similarly to GFP_NOFAIL requests.
Now that the whole tree has been cleaned up and accidental or misled
usage of __GFP_REPEAT flag has been removed for !costly requests we can
give the original flag a better name and more importantly a more useful
semantic. Let's rename it to __GFP_RETRY_MAYFAIL which tells the user
that the allocator would try really hard but there is no promise of a
success. This will work independent of the order and overrides the
default allocator behavior. Page allocator users have several levels of
guarantee vs. cost options (take GFP_KERNEL as an example)
- GFP_KERNEL & ~__GFP_RECLAIM - optimistic allocation without _any_
attempt to free memory at all. The most light weight mode which even
doesn't kick the background reclaim. Should be used carefully because
it might deplete the memory and the next user might hit the more
aggressive reclaim
- GFP_KERNEL & ~__GFP_DIRECT_RECLAIM (or GFP_NOWAIT)- optimistic
allocation without any attempt to free memory from the current
context but can wake kswapd to reclaim memory if the zone is below
the low watermark. Can be used from either atomic contexts or when
the request is a performance optimization and there is another
fallback for a slow path.
- (GFP_KERNEL|__GFP_HIGH) & ~__GFP_DIRECT_RECLAIM (aka GFP_ATOMIC) -
non sleeping allocation with an expensive fallback so it can access
some portion of memory reserves. Usually used from interrupt/bh
context with an expensive slow path fallback.
- GFP_KERNEL - both background and direct reclaim are allowed and the
_default_ page allocator behavior is used. That means that !costly
allocation requests are basically nofail but there is no guarantee of
that behavior so failures have to be checked properly by callers
(e.g. OOM killer victim is allowed to fail currently).
- GFP_KERNEL | __GFP_NORETRY - overrides the default allocator behavior
and all allocation requests fail early rather than cause disruptive
reclaim (one round of reclaim in this implementation). The OOM killer
is not invoked.
- GFP_KERNEL | __GFP_RETRY_MAYFAIL - overrides the default allocator
behavior and all allocation requests try really hard. The request
will fail if the reclaim cannot make any progress. The OOM killer
won't be triggered.
- GFP_KERNEL | __GFP_NOFAIL - overrides the default allocator behavior
and all allocation requests will loop endlessly until they succeed.
This might be really dangerous especially for larger orders.
Existing users of __GFP_REPEAT are changed to __GFP_RETRY_MAYFAIL
because they already had their semantic. No new users are added.
__alloc_pages_slowpath is changed to bail out for __GFP_RETRY_MAYFAIL if
there is no progress and we have already passed the OOM point.
This means that all the reclaim opportunities have been exhausted except
the most disruptive one (the OOM killer) and a user defined fallback
behavior is more sensible than keep retrying in the page allocator.

all persistent memory specific dax functionality to be pushed into
libnvdimm and the pmem driver directly. It also allows dax-specific
sysfs attributes to be linked to a host device, for example:

* Add support for the new NVDIMM platform/firmware mechanisms introduced
in ACPI 6.2 and UEFI 2.7. This support includes the v1.2 namespace

The Prefix CCW is not mandatory and raw I/O can also be issued without
it. Check whether the Prefix CCW is supported and if not use the
combination of Define Extent and Locate Record Extended instead.
While at it, sort the variable declarations, replace the gotos with
early exits, and remove an error check at the end which is irrelevant.
Also, remove the XRC check as it is not relevant for raw I/O.

We already have define_extent() that prepares necessary data for the
Define Extent CCW. The exact same thing is done in prefix_LRE().
Remove the duplicate code and move commands that were only used in
combination with the Prefix command to define_extent(). One of these
commands needs the blocksize to be specified. Add the blksize parameter
to define_extent() to account for that.
In addition, the check_XRC() function can be made more generic. Do this
and remove the Prefix-specific check_XRC_on_prefix() function.
Furthermore, prefix_LRE() uses fill_LRE_data() to prepare Locate Record
Extended data. Rename the function to fit the scheme better and make it
usable outside of the Prefix context by adding the corresponding CCW
command.

Here is the big driver core update for 4.13-rc1.
The large majority of this is a lot of cleanup of old fields in the
driver core structures and their remaining usages in random drivers.
All of those fixes have been reviewed by the various subsystem
maintainers. There's also some small firmware updates in here, a new
kobject uevent api interface that makes userspace interaction easier,
and a few other minor things.
All of these have been in linux-next for a long while with no reported
issues.

round in terms of features, but there's a lot of churn related to some
core cleanups.

refcount_t type and corresponding API should be
used instead of atomic_t when the variable is used as
a reference counter. This allows to avoid accidental
refcounter overflows that might lead to use-after-free
situations.

If a device is offline it can still be set to read-only via the bus id
through sysfs. Only the read-only feature flag for the ccw_device is
then set. If the device is online the corresponding block device needs
to be set to read-only as well (via set_disk_ro()).
The check whether there is a device to do so, however, happens after the
feature flag was set. This leads to an unnecessary "no such device"
error in the offline case.
This bug was introduced by commit 7571cb1c8e3cc ("s390/dasd: Make use of
dasd_set_feature() more often"). Fix this by simply returning count if
no device is available.

Pull in the fix for shared tags, as it conflicts with the pending

When a s390 guest runs on a z/VM host that's part of a SSI cluster,
it can be migrated to a different host. In this case, the MAC address
it originally obtained on the old host may be re-assigned to a new
guest. This would result in address conflicts between the two guests.
When running as z/VM guest, use the diag26c MAC Service to obtain
a hypervisor-managed MAC address. The MAC Service is SSI-aware, and
won't re-assign the address after the guest is migrated to a new host.
This patch adds support for the z/VM MAC Service on L2 devices.

There's two spots in qeth_send_packet() where we don't accurately
account for transmitted packing buffers in qeth's performance
statistics:
1) when flushing the current buffer due to insufficient size,
and the next buffer is not EMPTY, we need to account for that
flushed buffer.
2) when synchronizing with the TX completion code, we reset
flush_count and thus forget to account for any previously
flushed buffers.

blk_queue_split() is always called with the last arg being q->bio_split,
where 'q' is the first arg.
Also blk_queue_split() sometimes uses the passed-in 'bs' and sometimes uses
q->bio_split.
This is inconsistent and unnecessary. Remove the last arg and always use
q->bio_split inside blk_queue_split()

and in many places that means casts are required, more often than not.

The safe offline processing may hang forever because it waits for I/O
which can not be started because of the offline flag that prevents new
I/O from being started.
Allow I/O to be started during safe offline processing because in this
special case we take care that the queues are empty before throwing away
the device.

The safe offline processing needs, as well as the normal offline
processing, to be locked against multiple parallel executions. But it
should be able to be overtaken by a normal offline processing to make sure
that the device does not wait forever for outstanding I/O if the user
wants to.
Unfortunately the parallel processing of safe offline and normal offline
might lead to a race situation where both threads report successful
execution to the CIO layer which in turn tries to deregister the kobject
of the device twice. This leads to a
refcount_t: underflow; use-after-free.
error and the device is not able to be set online again afterwards without
a reboot.
Correct the locking of the safe offline processing by doing the following:
- Use the cdev lock to secure all set and test operations to the
device flags.
- Two safe offline processes are locked against each other using
the DASD_FLAG_SAFE_OFFLINE and DASD_FLAG_SAFE_OFFLINE_RUNNING
device flags.
The differentiation between offline triggered and offline running
is needed since the normal offline attribute is owned by CIO and
we have to pass over control in between.
- The dasd_generic_set_offline process handles the offline
processing. It is locked against parallel execution using the
DASD_FLAG_OFFLINE.
- Only a running safe offline should be able to be overtaken by a
single normal offline. This is ensured by clearing the
DASD_FLAG_SAFE_OFFLINE_RUNNING flag when a normal offline
overtakes. So this can only happen ones.
- The safe offline just aborts in this case doing nothing and
the normal offline processing finishes as usual.

We have two flags, DASD_FLAG_DEVICE_RO and DASD_FEATURE_READONLY, that
tell us whether a device is read-only. DASD_FLAG_DEVICE_RO is set when a
device is attached as read-only to z/VM and DASD_FEATURE_READONLY is set
when either the corresponding kernel parameter is configured, or the
read-only state is changed via sysfs.
This is valuable information in any case. However, only the feature flag
is being checked at the moment when we display the current state.
Fix this by checking both flags.

Added some dbf debug messages on failure of the most important
ioctl calls. These messages are only enabled with dbf level
6 (debug) and so do not affect the normal operating mode which
uses level 3 (errors and higher).

When a out of range domain parameter was given, the init function
returned with -EINVAL and the driver was not operational. As the
driver is statically build into the kernel and is able to work
with multiple domains anyway the init function should continue.
Now the user has a chance to write a new default domain value
via sysfs attribute file. Also added two new dbf debug messages
related to the domain value handling.

The zcrypt code contains a couple of functions which receive a
"big_endian" argument. All callers naturally pass "1" for big endian,
since s390 is big endian. Therefore get rid of this argument and also
get rid of the cpu_to_le()/cpu_to_be() calls.
This way we get rid of a couple of sparse warnings:

Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings. This patch
instead introduces a new blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning. Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.
For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.
blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

Similar to how qeth currently does early L2 setup of OSM and OSN
devices, add support for early setup of L3-only devices.
This adds a qeth_l3_devtype that contains all core and l3-specific
sysfs attributes, so that they can be created in one go while probing.
This just adds the infrastructure, exploitation of the support happens
in a subsequent patch.

qeth doesn't advertise NETIF_F_SG for L3 IQDs. So trust the stack to
not hand us any nonlinear skbs, and remove an always-true condition.
With the fact that data_offset < 0 is no longer possible on IQDs,
apply a small cleanup to subsequent code.

When the association between a queue device and the
driver is released via unbind and later re-associated
the queue device was not operational any more. Reason
was a wrong administration of the card/queue lists
within the ap device driver.
This patch introduces revised card/queue list handling
within the ap device driver: when an ap device is
detected it is initial not added to the card/queue list
any more. With driver probe the card device is added to
the card list/the queue device is added to the queue list
within a card. With driver remove the device is removed
from the card/queue list. Additionally there are some
situations within the ap device live where the lists
need update upon card/queue device release (for example
device hot unplug or suspend/resume).

The feature member of virtio_feature_desc contains little endian
values, given that it contents will be converted with
le32_to_cpu(). The "wrong" __u32 type leads to the sparse warnings
below.

broke the support for OSM and OSN devices as follows:
As OSM and OSN are L2 only, qeth_core_probe_device() does an early
setup by loading the l2 discipline and calling qeth_l2_probe_device().
In this context, adding the l2-specific bridgeport sysfs attributes
via qeth_l2_create_device_attributes() hits a BUG_ON in fs/sysfs/group.c,
since the basic sysfs infrastructure for the device hasn't been
established yet.
Note that OSN actually has its own unique sysfs attributes
(qeth_osn_devtype), so the additional attributes shouldn't be created
at all.
For OSM, add a new qeth_l2_devtype that contains all the common
and l2-specific sysfs attributes.
When qeth_core_probe_device() does early setup for OSM or OSN, assign
the corresponding devtype so that the ccwgroup probe code creates the
full set of sysfs attributes.
This allows us to skip qeth_l2_create_device_attributes() in case
of an early setup.
Any device that can't do early setup will initially have only the
generic sysfs attributes, and when it's probed later
qeth_l2_probe_device() adds the l2-specific attributes.
If an early-setup device is removed (by calling ccwgroup_ungroup()),
device_unregister() will - using the devtype - delete the
l2-specific attributes before qeth_l2_remove_device() is called.
So make sure to not remove them twice.
What complicates the issue is that qeth_l2_probe_device() and
qeth_l2_remove_device() is also called on a device when its
layer2 attribute changes (ie. its layer mode is switched).
For early-setup devices this wouldn't work properly - we wouldn't
remove the l2-specific attributes when switching to L3.
But switching the layer mode doesn't actually make any sense;
we already decided that the device can only operate in L2!
So just refuse to switch the layer mode on such devices. Note that
OSN doesn't have a layer2 attribute, so we only need to special-case
OSM.

When setting up the device from within the layer discipline's
probe routine, creating the layer-specific sysfs attributes can fail.
Report this error back to the caller, and handle it by
releasing the layer discipline.

When vfio_ccw_mdev_reset fails during the remove process of the mdev,
the current implementation simply returns.
The failure indicates that the subchannel device is in a NOT_OPER state,
thus the right thing to do should be removing the mdev.
While we are at here, reverse the condition check to make the code more
concise and readable.

Pull vfio-ccw branch to add the basic channel I/O passthrough
intrastructure based on vfio.
The focus is on supporting dasd-eckd(cu_type/dev_type = 0x3990/0x3390)
as the target device.

Although Linux does not use format-0 channel command words (CCW0)
these are a non-optional part of the platform spec, and for the sake
of platform compliance, and possibly some non-Linux guests, we have
to support CCW0.
Making the kernel execute a format 0 channel program is too much hassle
because we would need to allocate and use memory which can be addressed
by 24 bit physical addresses (because of CCW0.cda). So we implement CCW0
support by translating the channel program into an equivalent CCW1
program instead.

The current implementation doesn't check if the subchannel is in a
proper device state when handling an event. Let's introduce

Introduce a singlethreaded workqueue to handle the I/O interrupts.
With the work added to this queue, we store the I/O results to the
io_region of the subchannel, then signal the userspace program to
handle the results.

Realize VFIO_DEVICE_GET_IRQ_INFO ioctl to retrieve
VFIO_CCW_IO_IRQ information.
Realize VFIO_DEVICE_SET_IRQS ioctl to set an eventfd fd for
VFIO_CCW_IO_IRQ. Once a write operation to the ccw_io_region
was performed, trigger a signal on this fd.

Introduce VFIO_DEVICE_RESET ioctl for vfio-ccw to make it possible
to hot-reset the device.
We try to achieve a reset by first disabling the subchannel and
then enabling it again: this should clear all state at the subchannel.

We implement the basic ccw command handling infrastructure
here:
1. Translate the ccw commands.
2. Issue the translated ccw commands to the device.
3. Once we get the execution result, update the guest SCSW
with it.

To provide user-space a set of interfaces to:
1. pass in a ccw program to perform an I/O operation.
2. read back I/O results of the completed I/O operations.
We introduce an MMIO region for the vfio-ccw device here.
This region is defined to content:
1. areas to store arguments that an ssch required.
2. areas to store the I/O results.
Using pwrite/pread to the device on this region, a user-space program
could write/read data to/from the vfio-ccw device.

To make vfio support subchannel devices, we need to leverage the
mediated device framework to create a mediated device for the
subchannel device.
This registers the subchannel device to the mediated device
framework during probe to enable mediated device creation.

Introduce ccwchain structure and helper functions that can be used to
handle a channel program issued from a virtual machine.
The following limitations apply:
1. Supports only prefetch enabled mode.
2. Supports idal(c64) ccw chaining.
3. Supports 4k idaw.
4. Supports ccw1.
5. Supports direct ccw chaining by translating them to idal ccws.
CCW translation requires to leverage the vfio_(un)pin_pages interfaces
to pin/unpin sets of mem pages frequently. Currently we have a lack of
support to do this in an efficient way. So we introduce pfn_array data
structure and helper functions to handle pin/unpin operations here.

To make vfio support subchannel devices, we need a css driver for
the vfio subchannels. This patch adds a basic vfio-ccw subchannel
driver for this purpose.
To enable VFIO for vfio-ccw, enable S390_CCW_IOMMU config option
and configure VFIO as required.

On some z/VM systems the query host access command is not supported for
temp disks, though the corresponding feature code is set.
This does not have any impact beside that the information is not available.
Suppress the full blown command reject error messages to not confuse the
user. The error is still logged in the s390dbf.

With AF_IUCV traffic, the skb passed to hard_start_xmit() has a 14 byte
slot at skb->data, intended for an ETH header. qeth_l3_fill_af_iucv_hdr()
fills this ETH header... and then immediately moves it to the
skb's headroom, where it disappears and is never seen again.
But it's still possible for us to return NETDEV_TX_BUSY after the skb has
been modified. Since we didn't get a private copy of the skb, the next
time the skb is delivered to hard_start_xmit() it no longer has the
expected layout (we moved the ETH header to the headroom, so skb->data
now starts at the IUCV_TRANS header). So when qeth_l3_fill_af_iucv_hdr()
does another round of rebuilding, the resulting qeth header ends up
all wrong. On transmission, the buffer is then rejected by
the HiperSockets device with SBALF15 = x'04'.
When this error is passed back to af_iucv as TX_NOTIFY_UNREACHABLE, it
tears down the offending socket.
As the ETH header for AF_IUCV serves no purpose, just align the code to
what we do for IP traffic on L3 HiperSockets: keep the ETH header at
skb->data, and pass down data_offset = ETH_HLEN to qeth_fill_buffer().
When mapping the payload into the SBAL elements, the ETH header is then
stripped off. This avoids the skb manipulations in
qeth_l3_fill_af_iucv_hdr(), and any buffer re-entering hard_start_xmit()
after NETDEV_TX_BUSY is now processed properly.

Depending on the device type, hard_start_xmit() builds different output
buffer formats. For instance with HiperSockets, on both L2 and L3 we
strip the ETH header from the skb - L3 doesn't need it, and L2 carries
it in the buffer's header element.
For this, we pass data_offset = ETH_HLEN all the way down to
__qeth_fill_buffer(), where skb->data is then adjusted accordingly.
But the initial size calculation still considers the *full* skb length
(including the ETH header). So qeth_get_elements_no() can erroneously
reject a skb as too big, even though it would actually fit into an
output buffer once the ETH header has been trimmed off later.
Fix this by passing an additional offset to qeth_get_elements_no(),
that indicates where in the skb the on-wire data actually begins.
Since the current code uses data_offset=-1 for some special handling
on OSA, we need to clamp data_offset to 0...
On HiperSockets this helps when sending ~MTU-size skbs with weird page
alignment. No change for OSA or AF_IUCV.

User space needs some information about the secure key(s)
before actually invoking the pkey and/or paes funcionality.
This patch introduces a new ioctl API and in kernel API to
verify the the secure key blob and give back some
information about the key (type, bitsize, old MKVP).
Both APIs are described in detail in the header files
arch/s390/include/asm/pkey.h and arch/s390/include/uapi/asm/pkey.h.

When a secure key with an old Master Key Verification
Pattern was given to the pkey_findcard function, there was
no responsible card found because only the current MKVP of
each card was compared. With this fix also the old MKVP
values are considered and so a matching card able to handle
the key is reported back to the caller.
