Suren Baghdasaryan 68f48381d7 mm: introduce __vm_flags_mod and use it in untrack_pfn
There are scenarios when vm_flags can be modified without exclusive
mmap_lock, such as:
- after VMA was isolated and mmap_lock was downgraded or dropped
- in exit_mmap when there are no other mm users and locking is unnecessary
Introduce __vm_flags_mod to avoid assertions when the caller takes
responsibility for the required locking.
Pass a hint to untrack_pfn to conditionally use __vm_flags_mod for
flags modification to avoid assertion.

Pankaj Gupta 867400af90 mm/memremap.c: map FS_DAX device memory as decrypted
virtio_pmem use devm_memremap_pages() to map the device memory.  By
default this memory is mapped as encrypted with SEV.  Guest reboot changes
the current encryption key and guest no longer properly decrypts the FSDAX
device meta data.

Mark the corresponding device memory region for FSDAX devices (mapped with
memremap_pages) as decrypted to retain the persistent memory property.

Alistair Popple 0dc45ca1ce mm/memremap.c: take a pgmap reference on page allocation
ZONE_DEVICE pages have a struct dev_pagemap which is allocated by a
driver.  When the struct page is first allocated by the kernel in
memremap_pages() a reference is taken on the associated pagemap to ensure
it is not freed prior to the pages being freed.

Prior to 27674ef6c7 ("mm: remove the extra ZONE_DEVICE struct page
refcount") pages were considered free and returned to the driver when the
reference count dropped to one.  However the pagemap reference was not
dropped until the page reference count hit zero.  This would occur as part
of the final put_page() in memunmap_pages() which would wait for all pages
to be freed prior to returning.

When the extra refcount was removed the pagemap reference was no longer
being dropped in put_page().  Instead memunmap_pages() was changed to
explicitly drop the pagemap references.  This means that memunmap_pages()
can complete even though pages are still mapped by the kernel which can
lead to kernel crashes, particularly if a driver frees the pagemap.

To fix this drivers should take a pagemap reference when allocating the
page.  This reference can then be returned when the page is freed.

Alistair Popple ef23345089 mm: free device private pages have zero refcount
Since 27674ef6c7 ("mm: remove the extra ZONE_DEVICE struct page
refcount") device private pages have no longer had an extra reference
count when the page is in use.  However before handing them back to the
owning device driver we add an extra reference count such that free pages
have a reference count of one.

This makes it difficult to tell if a page is free or not because both free
and in use pages will have a non-zero refcount.  Instead we should return
pages to the drivers page allocator with a zero reference count.  Kernel
code can then safely use kernel functions such as get_page_unless_zero().

Muchun Song f4f451a16d mm: fix missing wake-up event for FSDAX pages
FSDAX page refcounts are 1-based, rather than 0-based: if refcount is
1, then the page is freed.  The FSDAX pages can be pinned through GUP,
then they will be unpinned via unpin_user_page() using a folio variant
to put the page, however, folio variants did not consider this special
case, the result will be to miss a wakeup event (like the user of
__fuse_dax_break_layouts()).  This results in a task being permanently
stuck in TASK_INTERRUPTIBLE state.

Since FSDAX pages are only possibly obtained by GUP users, so fix GUP
instead of folio_put() to lower overhead.

Alex Sierra f25cbb7a95 mm: add zone device coherent type memory support
Device memory that is cache coherent from device and CPU point of view. 
This is used on platforms that have an advanced system bus (like CAPI or
CXL).  Any page of a process can be migrated to such memory.  However, no
one should be allowed to pin such memory so that it can always be evicted.

David Hildenbrand 78fbe906cc mm/page-flags: reuse PG_mappedtodisk as PG_anon_exclusive for PageAnon() pages
The basic question we would like to have a reliable and efficient answer
to is: is this anonymous page exclusive to a single process or might it be
shared?  We need that information for ordinary/single pages, hugetlb
pages, and possibly each subpage of a THP.

Introduce a way to mark an anonymous page as exclusive, with the ultimate
goal of teaching our COW logic to not do "wrong COWs", whereby GUP pins
lose consistency with the pages mapped into the page table, resulting in
reported memory corruptions.

Most pageflags already have semantics for anonymous pages, however,
PG_mappedtodisk should never apply to pages in the swapcache, so let's
reuse that flag.

As PG_has_hwpoisoned also uses that flag on the second tail page of a
compound page, convert it to PG_error instead, which is marked as
PF_NO_TAIL, so never used for tail pages.

Use custom page flag modification functions such that we can do additional
sanity checks.  The semantics we'll put into some kernel doc in the future
are:

"
  PG_anon_exclusive is *usually* only expressive in combination with a
  page table entry. Depending on the page table entry type it might
  store the following information:

       Is what's mapped via this page table entry exclusive to the
       single process and can be mapped writable without further
       checks? If not, it might be shared and we might have to COW.

  For now, we only expect PTE-mapped THPs to make use of
  PG_anon_exclusive in subpages. For other anonymous compound
  folios (i.e., hugetlb), only the head page is logically mapped and
  holds this information.

  For example, an exclusive, PMD-mapped THP only has PG_anon_exclusive
  set on the head page. When replacing the PMD by a page table full
  of PTEs, PG_anon_exclusive, if set on the head page, will be set on
  all tail pages accordingly. Note that converting from a PTE-mapping
  to a PMD mapping using the same compound page is currently not
  possible and consequently doesn't require care.

  If GUP wants to take a reliable pin (FOLL_PIN) on an anonymous page,
  it should only pin if the relevant PG_anon_exclusive is set. In that
  case, the pin will be fully reliable and stay consistent with the pages
  mapped into the page table, as the bit cannot get cleared (e.g., by
  fork(), KSM) while the page is pinned. For anonymous pages that
  are mapped R/W, PG_anon_exclusive can be assumed to always be set
  because such pages cannot possibly be shared.

  The page table lock protecting the page table entry is the primary
  synchronization mechanism for PG_anon_exclusive; GUP-fast that does
  not take the PT lock needs special care when trying to clear the
  flag.

  Page table entry types and PG_anon_exclusive:
  * Present: PG_anon_exclusive applies.
  * Swap: the information is lost. PG_anon_exclusive was cleared.
  * Migration: the entry holds this information instead.
               PG_anon_exclusive was cleared.
  * Device private: PG_anon_exclusive applies.
  * Device exclusive: PG_anon_exclusive applies.
  * HW Poison: PG_anon_exclusive is stale and not changed.

  If the page may be pinned (FOLL_PIN), clearing PG_anon_exclusive is
  not allowed and the flag will stick around until the page is freed
  and folio->mapping is cleared.
"

We won't be clearing PG_anon_exclusive on destructive unmapping (i.e.,
zapping) of page table entries, page freeing code will handle that when
also invalidate page->mapping to not indicate PageAnon() anymore.  Letting
information about exclusivity stick around will be an important property
when adding sanity checks to unpinning code.

Note that we properly clear the flag in free_pages_prepare() via
PAGE_FLAGS_CHECK_AT_PREP for each individual subpage of a compound page,
so there is no need to manually clear the flag.

that pages are mapped at a given huge page alignment and utilize uses
compound pages as opposed to order-0 pages.

Take advantage of the fact that most tail pages look the same (except the
first two) to minimize struct page overhead.  Allocate a separate page for
the vmemmap area which contains the head page and separate for the next 64
pages.  The rest of the subsections then reuse this tail vmemmap page to
initialize the rest of the tail pages.

Sections are arch-dependent (e.g.  on x86 it's 64M, 128M or 512M) and when
initializing compound devmap with big enough @vmemmap_shift (e.g.  1G PUD)
it may cross multiple sections.  The vmemmap code needs to consult @pgmap
so that multiple sections that all map the same tail data can refer back
to the first copy of that data for a given gigantic page.

On compound devmaps with 2M align, this mechanism lets 6 pages be saved
out of the 8 necessary PFNs necessary to set the subsection's 512 struct
pages being mapped.  On a 1G compound devmap it saves 4094 pages.

Altmap isn't supported yet, given various restrictions in altmap pfn
allocator, thus fallback to the already in use vmemmap_populate().  It is
worth noting that altmap for devmap mappings was there to relieve the
pressure of inordinate amounts of memmap space to map terabytes of pmem. 
With compound pages the motivation for altmaps for pmem gets reduced.

Hugh Dickins bb43b14b57 mm: delete __ClearPageWaiters()
The PG_waiters bit is not included in PAGE_FLAGS_CHECK_AT_FREE, and
vmscan.c's free_unref_page_list() callers rely on that not to generate
bad_page() alerts.  So __page_cache_release(), put_pages_list() and
release_pages() (and presumably copy-and-pasted free_zone_device_page())
are redundant and misleading to make a special point of clearing it (as
the "__" implies, it could only safely be used on the freeing path).

Delete __ClearPageWaiters().  Remark on this in one of the "possible"
comments in folio_wake_bit(), and delete the superfluous comments.

Christoph Hellwig 27674ef6c7 mm: remove the extra ZONE_DEVICE struct page refcount
ZONE_DEVICE struct pages have an extra reference count that complicates
the code for put_page() and several places in the kernel that need to
check the reference count to see that a page is not being used (gup,
compaction, migration, etc.). Clean up the code so the reference count
doesn't need to be treated specially for ZONE_DEVICE pages.

Note that this excludes the special idle page wakeup for fsdax pages,
which still happens at refcount 1.  This is a separate issue and will
be sorted out later.  Given that only fsdax pages require the
notifiacation when the refcount hits 1 now, the PAGEMAP_OPS Kconfig
symbol can go away and be replaced with a FS_DAX check for this hook
in the put_page fastpath.

Christoph Hellwig be8a80b3e2 mm: remove a pointless CONFIG_ZONE_DEVICE check in memremap_pages
Patch series "start sorting out the ZONE_DEVICE refcount mess", v2.

This series removes the offset by one refcount for ZONE_DEVICE pages that
are freed back to the driver owning them, which is just device private
ones for now, but also the planned device coherent pages and the ehanced
p2p ones pending.

It does not address the fsdax pages yet, which will be attacked in a
follow on series.

Joao Martins c4386bd8ee mm/memremap: add ZONE_DEVICE support for compound pages
Add a new @vmemmap_shift property for struct dev_pagemap which specifies
that a devmap is composed of a set of compound pages of order
@vmemmap_shift, instead of base pages.  When a compound page devmap is
requested, all but the first page are initialised as tail pages instead
of order-0 pages.

For certain ZONE_DEVICE users like device-dax which have a fixed page
size, this creates an opportunity to optimize GUP and GUP-fast walkers,
treating it the same way as THP or hugetlb pages.

Additionally, commit 7118fc2906 ("hugetlb: address ref count racing in
prep_compound_gigantic_page") removed set_page_count() because the
setting of page ref count to zero was redundant.  devmap pages don't
come from page allocator though and only head page refcount is used for
compound pages, hence initialize tail page count to zero.

Anshuman Khandual bca3feaa07 mm/memory_hotplug: prevalidate the address range being added with platform
Patch series "mm/memory_hotplug: Pre-validate the address range with platform", v5.

This series adds a mechanism allowing platforms to weigh in and
prevalidate incoming address range before proceeding further with the
memory hotplug.  This helps prevent potential platform errors for the
given address range, down the hotplug call chain, which inevitably fails
the hotplug itself.

This mechanism was suggested by David Hildenbrand during another
discussion with respect to a memory hotplug fix on arm64 platform.

This mechanism focuses on the addressibility aspect and not [sub] section
alignment aspect.  Hence check_hotplug_memory_range() and check_pfn_span()
have been left unchanged.

This patch (of 4):

This introduces mhp_range_allowed() which can be called in various memory
hotplug paths to prevalidate the address range which is being added, with
the platform.  Then mhp_range_allowed() calls mhp_get_pluggable_range()
which provides applicable address range depending on whether linear
mapping is required or not.  For ranges that require linear mapping, it
calls a new arch callback arch_get_mappable_range() which the platform can
override.  So the new callback, in turn provides the platform an
opportunity to configure acceptable memory hotplug address ranges in case
there are constraints.

This mechanism will help prevent platform specific errors deep down during
hotplug calls.  This drops now redundant
check_hotplug_memory_addressable() check in __add_pages() but instead adds
a VM_BUG_ON() check which would ensure that the range has been validated
with mhp_range_allowed() earlier in the call chain.  Besides
mhp_get_pluggable_range() also can be used by potential memory hotplug
callers to avail the allowed physical range which would go through on a
given platform.

This does not really add any new range check in generic memory hotplug but
instead compensates for lost checks in arch_add_memory() where applicable
and check_hotplug_memory_addressable(), with unified mhp_range_allowed().

Dan Williams 34dc45be45 mm: fix memory_failure() handling of dax-namespace metadata
Given 'struct dev_pagemap' spans both data pages and metadata pages be
careful to consult the altmap if present to delineate metadata.  In fact
the pfn_first() helper already identifies the first valid data pfn, so
export that helper for other code paths via pgmap_pfn_valid().

Other usage of get_dev_pagemap() are not a concern because those are
operating on known data pfns having been looked up by get_user_pages().
I.e.  metadata pfns are never user mapped.

Ralph Campbell 46b1ee38b2 mm/mremap_pages: fix static key devmap_managed_key updates
commit 6f42193fd8 ("memremap: don't use a separate devm action for
devmap_managed_enable_get") changed the static key updates such that we
now call devmap_managed_enable_put() without doing the equivalent
devmap_managed_enable_get().

devmap_managed_enable_get() is only called for MEMORY_DEVICE_PRIVATE and
MEMORY_DEVICE_FS_DAX, But memunmap_pages() get called for other pgmap

Ira Weiny 433e7d3177 mm/memremap.c: convert devmap static branch to {inc,dec}
While reviewing Protection Key Supervisor support it was pointed out that
using a counter to track static branch enable was an anti-pattern which
was better solved using the provided static_branch_{inc,dec} functions.[1]

Fix up devmap_managed_key to work the same way.  Also this should be safer
because there is a very small (very unlikely) race when multiple callers
try to enable at the same time.

Yu Zhao 6f4dd8de48 mm: remove superfluous __ClearPageActive()
To activate a page, mark_page_accessed() always holds a reference on it.
It either gets a new reference when adding a page to
lru_pvecs.activate_page or reuses an existing one it previously got when
it added a page to lru_pvecs.lru_add.  So it doesn't call SetPageActive()
on a page that doesn't have any reference left.  Therefore, the race is
impossible these days (I didn't brother to dig into its history).

For other paths, namely reclaim and migration, a reference count is always
held while calling SetPageActive() on a page.

SetPageSlabPfmemalloc() also uses SetPageActive(), but it's irrelevant to
LRU pages.

Dan Williams b7b3c01b19 mm/memremap_pages: support multiple ranges per invocation
In support of device-dax growing the ability to front physically
dis-contiguous ranges of memory, update devm_memremap_pages() to track
multiple ranges with a single reference counter and devm instance.

Dan Williams a4574f63ed mm/memremap_pages: convert to 'struct range'
The 'struct resource' in 'struct dev_pagemap' is only used for holding
resource span information.  The other fields, 'name', 'flags', 'desc',
'parent', 'sibling', and 'child' are all unused wasted space.

This is in preparation for introducing a multi-range extension of
devm_memremap_pages().

The bulk of this change is unwinding all the places internal to libnvdimm
that used 'struct resource' unnecessarily, and replacing instances of
'struct dev_pagemap'.res with 'struct dev_pagemap'.range.

P2PDMA had a minor usage of the resource flags field, but only to report
failures with "%pR".  That is replaced with an open coded print of the
range.

Logan Gunthorpe a50d8d98a8 mm/memremap: set caching mode for PCI P2PDMA memory to WC
PCI BAR IO memory should never be mapped as WB, however prior to this
the PAT bits were set WB and it was typically overridden by MTRR
registers set by the firmware.

Set PCI P2PDMA memory to be UC as this is what it currently, typically,
ends up being mapped as on x86 after the MTRR registers override the
cache setting.

Future use-cases may need to generalize this by adding flags to select
the caching type, as some P2PDMA cases may not want UC.  However, those
use-cases are not upstream yet and this can be changed when they arrive.

Logan Gunthorpe bfeb022f8f mm/memory_hotplug: add pgprot_t to mhp_params
devm_memremap_pages() is currently used by the PCI P2PDMA code to create
struct page mappings for IO memory.  At present, these mappings are
created with PAGE_KERNEL which implies setting the PAT bits to be WB.
However, on x86, an mtrr register will typically override this and force
the cache type to be UC-.  In the case firmware doesn't set this
register it is effectively WB and will typically result in a machine
check exception when it's accessed.

Other arches are not currently likely to function correctly seeing they
don't have any MTRR registers to fall back on.

To solve this, provide a way to specify the pgprot value explicitly to
arch_add_memory().

Of the arches that support MEMORY_HOTPLUG: x86_64, and arm64 need a
simple change to pass the pgprot_t down to their respective functions
which set up the page tables.  For x86_32, set the page tables
explicitly using _set_memory_prot() (seeing they are already mapped).

For ia64, s390 and sh, reject anything but PAGE_KERNEL settings -- this
should be fine, for now, seeing these architectures don't support
ZONE_DEVICE.

A check in __add_pages() is also added to ensure the pgprot parameter
was set for all arches.

Dan Williams 9ffc1d19fc mm/memremap_pages: Introduce memremap_compat_align()
The "sub-section memory hotplug" facility allows memremap_pages() users
like libnvdimm to compensate for hardware platforms like x86 that have a
section size larger than their hardware memory mapping granularity.  The
compensation that sub-section support affords is being tolerant of
physical memory resources shifting by units smaller (64MiB on x86) than
the memory-hotplug section size (128 MiB). Where the platform
physical-memory mapping granularity is limited by the number and
capability of address-decode-registers in the memory controller.

While the sub-section support allows memremap_pages() to operate on
sub-section (2MiB) granularity, the Power architecture may still
require 16MiB alignment on "!radix_enabled()" platforms.

In order for libnvdimm to be able to detect and manage this per-arch
limitation, introduce memremap_compat_align() as a common minimum
alignment across all driver-facing memory-mapping interfaces, and let
Power override it to 16MiB in the "!radix_enabled()" case.

The assumption / requirement for 16MiB to be a viable
memremap_compat_align() value is that Power does not have platforms
where its equivalent of address-decode-registers never hardware remaps a
persistent memory resource on smaller than 16MiB boundaries. Note that I
tried my best to not add a new Kconfig symbol, but header include
entanglements defeated the #ifndef memremap_compat_align design pattern
and the need to export it defeats the __weak design pattern for arch
overrides.

John Hubbard 07d8026995 mm: devmap: refactor 1-based refcounting for ZONE_DEVICE pages
An upcoming patch changes and complicates the refcounting and especially
the "put page" aspects of it.  In order to keep everything clean,
refactor the devmap page release routines:

* Rename put_devmap_managed_page() to page_is_devmap_managed(), and
  limit the functionality to "read only": return a bool, with no side
  effects.

* Add a new routine, put_devmap_managed_page(), to handle decrementing
  the refcount for ZONE_DEVICE pages.

* Change callers (just release_pages() and put_page()) to check
  page_is_devmap_managed() before calling the new
  put_devmap_managed_page() routine.  This is a performance point:
  put_page() is a hot path, so we need to avoid non- inline function calls
  where possible.

* Rename __put_devmap_managed_page() to free_devmap_managed_page(), and
  limit the functionality to unconditionally freeing a devmap page.

This is originally based on a separate patch by Ira Weiny, which applied
to an early version of the put_user_page() experiments.  Since then,
Jérôme Glisse suggested the refactoring described above.

Dan Williams 429589d647 mm: Cleanup __put_devmap_managed_page() vs ->page_free()
After the removal of the device-public infrastructure there are only 2
->page_free() call backs in the kernel.  One of those is a
device-private callback in the nouveau driver, the other is a generic
wakeup needed in the DAX case.  In the hopes that all ->page_free()
callbacks can be migrated to common core kernel functionality, move the
device-private specific actions in __put_devmap_managed_page() under the
is_device_private_page() conditional, including the ->page_free()
callback.  For the other page types just open-code the generic wakeup.

Yes, the wakeup is only needed in the MEMORY_DEVICE_FSDAX case, but it
does no harm in the MEMORY_DEVICE_DEVDAX and MEMORY_DEVICE_PCI_P2PDMA
case.

Aneesh Kumar K.V 77e080e768 mm/memunmap: don't access uninitialized memmap in memunmap_pages()
Patch series "mm/memory_hotplug: Shrink zones before removing memory",
v6.

This series fixes the access of uninitialized memmaps when shrinking
zones/nodes and when removing memory.  Also, it contains all fixes for
crashes that can be triggered when removing certain namespace using
memunmap_pages() - ZONE_DEVICE, reported by Aneesh.

We stop trying to shrink ZONE_DEVICE, as it's buggy, fixing it would be
more involved (we don't have SECTION_IS_ONLINE as an indicator), and
shrinking is only of limited use (set_zone_contiguous() cannot detect
the ZONE_DEVICE as contiguous).

We continue shrinking !ZONE_DEVICE zones, however, I reduced the amount
of code to a minimum.  Shrinking is especially necessary to keep
zone->contiguous set where possible, especially, on memory unplug of
DIMMs at zone boundaries.

Zones are now properly shrunk when offlining memory blocks or when
onlining failed.  This allows to properly shrink zones on memory unplug
even if the separate memory blocks of a DIMM were onlined to different
zones or re-onlined to a different zone after offlining.

With an altmap, the memmap falling into the reserved altmap space are not
initialized and, therefore, contain a garbage NID and a garbage zone.
Make sure to read the NID/zone from a memmap that was initialized.

The "page_zone(pfn_to_page(pfn)" was introduced by 69324b8f48 ("mm,
devm_memremap_pages: add MEMORY_DEVICE_PRIVATE support"), however, I
think we will never have driver reserved memory with
MEMORY_DEVICE_PRIVATE (no altmap AFAIKS).

Anshuman Khandual 6d0e984941 mm/memremap: drop unused SECTION_SIZE and SECTION_MASK
SECTION_SIZE and SECTION_MASK macros are not getting used anymore.  But
they do conflict with existing definitions on arm64 platform causing
