User applications can register memory regions for TID buffers that are not
aligned on page boundaries. Hfi1 is expected to pin those pages in memory
and cache the pages with mmu_rb. The rb tree will fail to insert pages
that are not aligned correctly.
Validate whether a given virtual address is page aligned before pinning.

The command 'ibv_devinfo -v' reports 0 for max_mr.

The qpn allocation logic has a WARN_ON() that intends to detect the use of
an index that will introduce bits in the lower order bits of the QOS bits
in the QPN.
Unfortunately, it has the following bugs:
- it misfires when wrapping QPN allocation for non-QOS
- it doesn't correctly detect low order QOS bits (despite the comment)
The WARN_ON() should not be applied to non-QOS (qos_shift == 1).
Additionally, it SHOULD test the qpn bits per the table below:

Currently if alloc_skb fails to allocate the skb a null skb is passed to
t4_set_arp_err_handler and this ends up dereferencing the null skb. Avoid
the NULL pointer dereference by checking for a NULL skb and returning
early.

Since QP destruction frees memory, hfi1_wq should have the WQ_MEM_RECLAIM.
The hfi1_wq does not allocate memory with GFP_KERNEL or otherwise become

When an old ack_queue entry is used to store an incoming request, it may
need to clean up the old entry if it is still referencing the
MR. Originally only RDMA READ request needed to reference MR on the
responder side and therefore the opcode was tested when cleaning up the
old entry. The introduction of tid rdma specific operations in the
ack_queue makes the specific opcode tests wrong. Multiple opcodes (RDMA
READ, TID RDMA READ, and TID RDMA WRITE) may need MR ref cleanup.
Remove the opcode specific tests associated with the ack_queue.

Current implementation was not properly handling frwr memory
registrations. This was uncovered by commit 27f26cec761das ("xprtrdma:
Plant XID in on-the-wire RDMA offset (FRWR)") in which xprtrdma, which is
used for NFS over RDMA, started failing as it was the first ULP to modify
the ib_mr iova resulting in the NFS server getting REMOTE ACCESS ERROR
when attempting to perform RDMA Writes to the client.
The fix is to properly capture the true iova, offset, and length in the
call to ib_map_mr_sg, and then update the iova when processing the
IB_WR_REG_MEM on the send queue.

Code review revealed a race condition which could allow the catas error
flow to interrupt the alias guid query post mechanism at random points.
Thiis is fixed by doing cancel_delayed_work_sync() instead of
cancel_delayed_work() during the alias guid mechanism destroy flow.

When an application aborts the connection by moving QP from RTS to ERROR,
then iw_cxgb4's modify_rc_qp() RTS->ERROR logic sets the
*srqidxp to 0 via t4_set_wq_in_error(&qhp->wq, 0), and aborts the
connection by calling c4iw_ep_disconnect().
c4iw_ep_disconnect() does the following:
1. sends up a close_complete_upcall(ep, -ECONNRESET) to libcxgb4.
2. sends abort request CPL to hw.
But, since the close_complete_upcall() is sent before sending the
ABORT_REQ to hw, libcxgb4 would fail to release the srqidx if the
connection holds one. Because, the srqidx is passed up to libcxgb4 only
after corresponding ABORT_RPL is processed by kernel in abort_rpl().
This patch handle the corner-case by moving the call to
close_complete_upcall() from c4iw_ep_disconnect() to abort_rpl(). So that
libcxgb4 is notified about the -ECONNRESET only after abort_rpl(), and
libcxgb4 can relinquish the srqidx properly.

Using CX-3 virtual functions, either from a bare-metal machine or
pass-through from a VM, MAD packets are proxied through the PF driver.
Since the VF drivers have separate name spaces for MAD Transaction Ids
(TIDs), the PF driver has to re-map the TIDs and keep the book keeping
in a cache.
Following the RDMA Connection Manager (CM) protocol, it is clear when
an entry has to evicted form the cache. But life is not perfect,
remote peers may die or be rebooted. Hence, it's a timeout to wipe out
a cache entry, when the PF driver assumes the remote peer has gone.
During workloads where a high number of QPs are destroyed concurrently,
excessive amount of CM DREQ retries has been observed
The problem can be demonstrated in a bare-metal environment, where two
nodes have instantiated 8 VFs each. This using dual ported HCAs, so we
have 16 vPorts per physical server.
64 processes are associated with each vPort and creates and destroys
one QP for each of the remote 64 processes. That is, 1024 QPs per
vPort, all in all 16K QPs. The QPs are created/destroyed using the
CM.
When tearing down these 16K QPs, excessive CM DREQ retries (and
duplicates) are observed. With some cat/paste/awk wizardry on the
infiniband_cm sysfs, we observe as sum of the 16 vPorts on one of the
nodes:

Enabling pr_debug in cm.c gives tons of:

By increasing the CM_CLEANUP_CACHE_TIMEOUT from 5 to 30 seconds, the
tear-down phase of the application is reduced from approximately 90 to
50 seconds. Retries/duplicates are also significantly reduced:

Increasing the timeout further didn't help, as these duplicates and
retries stems from a too short CMA timeout, which was 20 (~4 seconds)
on the systems. By increasing the CMA timeout to 22 (~17 seconds), the
numbers fell down to about 10 for both of them.
Adjustment of the CMA timeout is not part of this commit.

The work completion length for a receiving a UD send with immediate is
short by 4 bytes causing application using this opcode to fail.
The UD receive logic incorrectly subtracts 4 bytes for immediate
value. These bytes are already included in header length and are used to
calculate header/payload split, so the result is these 4 bytes are
subtracted twice, once when the header length subtracted from the overall
length and once again in the UD opcode specific path.
Remove the extra subtraction when handling the opcode.

Since .scsi_done() must only be called after scsi_queue_rq() has
finished, make sure that the SRP initiator driver does not call
.scsi_done() while scsi_queue_rq() is in progress. Although
invoking sg_reset -d while I/O is in progress works fine with kernel
v4.20 and before, that is not the case with kernel v5.0-rc1. This
patch avoids that the following crash is triggered with kernel
v5.0-rc1:

Fix potential memory corruption and panic in loopback for IB_WR_SEND
variants.
The code blindly assumes the posted length will fit in the fetched rwqe,
which is not a valid assumption.
Fix by adding a limit test, and triggering the appropriate send completion
and putting the QP in an error state. This mimics the handling for
non-loopback QPs.

Currently, When a reserved operation is completed, its entry in the send
queue will not be unreserved, which leads to the miscalculation of
qp->s_avail and thus the triggering of a WARN_ON call trace. This patch
fixes the problem by unreserving the reserved operation when it is
completed.

Applications that use the stack for execution purposes cause userspace PSM
jobs to fail during mmap().
Both Fortran (non-standard format parsing) and C (callback functions
located in the stack) applications can be written such that stack
execution is required. The linker notes this via the gnu_stack ELF flag.
This causes READ_IMPLIES_EXEC to be set which forces all PROT_READ mmaps
to have PROT_EXEC for the process.
Checking for VM_EXEC bit and failing the request with EPERM is overly
conservative and will break any PSM application using executable stacks.

Acquiring the rtnl lock while holding usdev_lock could result in a
deadlock.
For example:

This commit moves the usdev_lock acquisition after the rtnl lock has been
released.
This is safe to do because usdev_lock is not protecting anything being
accessed in ib_get_eth_speed(). Hence, the correct order of holding locks
(rtnl -> usdev_lock) is not violated.

FRWR memory registration is done with a series of calls and WRs.
1. ULP invokes ib_dma_map_sg()
2. ULP invokes ib_map_mr_sg()
3. ULP posts an IB_WR_REG_MR on the Send queue
Step 2 generates an iova. It is permissible for ULPs to change this
iova (with certain restrictions) between steps 2 and 3.
rxe_map_mr_sg captures the MR's iova but later when rxe processes the
REG_MR WR, it ignores the MR's iova field. If a ULP alters the MR's iova
after step 2 but before step 3, rxe never captures that change.
When the remote sends an RDMA Read targeting that MR, rxe looks up the
R_key, but the altered iova does not match the iova stored in the MR,
causing the RDMA Read request to fail.

Packet queue state is over used to determine SDMA descriptor
availablitity and packet queue request state.

At this point pq->n_reqs == 0 and pq->state is incorrectly
SDMA_PKT_Q_ACTIVE. The close path will hang waiting for the state
to return to _INACTIVE.
This can also change the state from _DEFERRED to _ACTIVE. However,
this is a mostly benign race.
Remove the racy code path.
Use n_reqs to determine if a packet queue is active or not.

Memory windows are implemented with an indirect MKey, when a page fault
event comes for a MW Mkey we need to find the MR at the end of the list of
the indirect MKeys by iterating on all items from the first to the last.
The offset calculated during this process has to be zeroed after the first
iteration or the next iteration will start from a wrong address, resulting
incorrect ODP faulting behavior.

rdmavt uses a crazy system that looses the type checking when assinging
functions to struct ib_device function pointers. Because of this the
signature to this function was not changed when the below commit revised
things.
Fix the signature so we are not calling a function pointer with a
mismatched signature.

Currently, for IB_WR_LOCAL_INV WR, when the next fence is None, the
current fence will be SMALL instead of Normal Fence.
Without this patch krping doesn't work on CX-5 devices and throws

If the firmware reports a connection width that is not 1x, 4x, 8x or 12x
it causes the driver to fail during initialization.
To prevent this failure every time a new width is introduced to the RDMA
stack, we will set a default 4x width for these widths which ar unknown to
the driver.
This is needed to allow to run old kernels with new firmware.

If i40iw_allocate_dma_mem fails when creating a QP, the
memory allocated for the QP structure using kzalloc is not
freed because iwqp->allocated_buffer is used to free the
memory and it is not setup until later. Fix this by setting
iwqp->allocated_buffer before allocating the dma memory.

pq_update() can only be called in two places: from the completion
function when the complete (npkts) sequence of packets has been
submitted and processed, or from setup function if a subset of the
packets were submitted (i.e. the error path).
Currently both paths can call pq_update() if an error occurrs. This
race will cause the n_req value to go negative, hanging file_close(),
or cause a crash by freeing the txlist more than once.
Several variables are used to determine SDMA send state. Most of
these are unnecessary, and have code inspectible races between the
setup function and the completion function, in both the send path and
the error path.
The request 'status' value can be set by the setup or by the
completion function. This is code inspectibly racy. Since the status
is not needed in the completion code or by the caller it has been
removed.
The request 'done' value races between usage by the setup and the
completion function. The completion function does not need this.
When the number of processed packets matches npkts, it is done.
The 'has_error' value races between usage of the setup and the
completion function. This can cause incorrect error handling and leave
the n_req in an incorrect value (i.e. negative).
Simplify the code by removing all of the unneeded state checks and
variables.
Clean up iovs node when it is freed.
Eliminate race conditions in the error path:
If all packets are submitted, the completion handler will set the
completion status correctly (ok or aborted).
If all packets are not submitted, the caller must wait until the
submitted packets have completed, and then set the completion status.
These two change eliminate the race condition in the error path.

Currently qp->port stores the port number whenever IB_QP_PORT
QP attribute mask is set (during QP state transition to INIT state).
This port number should be stored for the real QP when XRC target QP
is used.
Follow the ib_modify_qp() implementation and hide the access to ->real_qp.

Don't reset the resp opcode for a replayed read response.
The resp opcode could be in the middle of a write or send
sequence, when the duplicate read request was received.
An example sequence is as follows:
- Receive read request for 12KB PSN 20. Transmit read response
first, middle and last with PSNs 20,21,22.
- Receive write first PSN 23.
At this point the resp psn is 24 and resp opcode is write first.
- The sender notices that PSN 20 is dropped and retransmits.
Receive read request for 12KB PSN 20. Transmit read response
first, middle and last with PSNs 20,21,22. The resp opcode is
set to -1, the resp psn remains 24.
- Receive write first PSN 23. This is processed by duplicate_request().
The resp opcode remains -1 and resp psn remains 24.
- Receive write middle PSN 24. check_op_seq() reports a missing
first error since the resp opcode is -1.
When sending an ack for a duplicate send or write request,
use the psn of the previous ack sent. Do not use the psn
of a read response for the ack.
An example sequence is as follows:
- Receive write PSN 30. Transmit ACK for PSN 30.
- Receive read request 4KB PSN 31. Transmit read response with
PSN 31. The resp psn is now 32.
- The sender notices that PSN 30 is dropped and retransmits.
Receive write PSN 30. duplicate_request() sends an ACK with
PSN 31. That is incorrect since PSN 31 was a read request.

If the provider driver (such as rdma_rxe) doesn't support pma counters,
avoid exposing its directory similar to optional hw_counters directory.
If core fails to read the PMA counter, return an error so that user can
retry later if needed.

Fix this by sanitizing hdr.cmd before using it to index

to kill the speculation on the first load and not worry if it can be
completed with a dependent load/store [1].

During QP creation, the mlx5 driver translates the QP type to an
internal value which is passed on to FW. There was no check to make
sure that the translated value is valid, and -EINVAL was coerced into
the mailbox command.
Current firmware refuses this as an invalid QP type, but future/past
firmware may do something else.

Adding the vector offset when calling to mlx5_vector2eqn() is wrong.
This is because mlx5_vector2eqn() checks if EQ index is equal to vector number
and the fact that the internal completion vectors that mlx5 allocates
don't get an EQ index.
The second problem here is that using effective_affinity_mask gives the same
CPU for different vectors.
This leads to unmapped queues when calling it from blk_mq_rdma_map_queues().
This doesn't happen when using affinity_hint mask.

If the allocation of elem fails, it is not sufficient to simply check
for NULL and return. We need to also put our reference on the pool or
else we will leave the pool with a permanent ref count and we will never
be able to free it.

quiesce_qp() waits until all outstanding packets have been freed.
This wait should be momentary. During a link down event, the cleanup
handling does not ensure that all packets caught by the link down are
flushed properly.
This is caused by the fact that the freeze path and the link down
event is handled the same. This is not correct. The freeze path
waits until the HFI is unfrozen and then restarts PIO. A link down
is not a freeze event. The link down path cannot restart the PIO
until link is restored. If the PIO path is restarted before the link
comes up, the application (QP) using the PIO path will hang (until
link is restored).
Fix by separating the linkdown path from the freeze path and use the
link down path for link down events.
Close a race condition sc_disable() by acquiring both the progress
and release locks.
Close a race condition in sc_stop() by moving the setting of the flag
bits under the alloc lock.

Before idr_remove(), ucma_get_ctx() could still find the ctx
and after rdma_destroy_id(), rdma_resolve_addr() may still
access id_priv pointer. Also, ucma_put_ctx() may use ctx after
ucma_free_ctx() too.
ucma_close() should call ucma_put_ctx() too which tests the
refcnt and waits for the last one releasing it. The similar
pattern is already used by ucma_destroy_id().

The current code grabs the private_data of whatever file descriptor
userspace has supplied and implicitly casts it to a `struct ucma_file *`,
potentially causing a type confusion.
This is probably fine in practice because the pointer is only used for
comparisons, it is never actually dereferenced; and even in the
comparisons, it is unlikely that a file from another filesystem would have
a ->private_data pointer that happens to also be valid in this context.
But ->private_data is not always guaranteed to be a valid pointer to an
object owned by the file's filesystem; for example, some filesystems just
cram numbers in there.
Check the type of the supplied file descriptor to be safe, analogous to how
other places in the kernel do it.

Currently a uverbs completion event queue is flushed of events in
ib_uverbs_comp_event_close() with the queue spinlock held and then
released. Yet setting ev_queue->is_closed is not set until later in
uverbs_hot_unplug_completion_event_file().
In between the time ib_uverbs_comp_event_close() releases the lock and
uverbs_hot_unplug_completion_event_file() acquires the lock, a completion
event can arrive and be inserted into the event queue by
ib_uverbs_comp_handler().
This can cause a "double add" list_add warning or crash depending on the
kernel configuration, or a memory leak because the event is never dequeued
since the queue is already closed down.
So add setting ev_queue->is_closed = 1 to ib_uverbs_comp_event_close().

If a packet stream uses an UnsupportedVL (virtual lane), the send
engine will not send the packet, and it will not indicate that an
error has occurred. This will cause the packet stream to block.
HFI has 8 virtual lanes available for packet streams. Each lane can
be enabled or disabled using the UnsupportedVL mask. If a lane is
disabled, adding a packet to the send context must be disallowed.
The current mask for determining unsupported VLs defaults to 0 (allow
all). This is incorrect. Only the VLs that are defined should be
allowed.
Determine which VLs are disabled (mtu == 0), and set the appropriate
unsupported bit in the mask. The correct mask will allow the send
engine to error on the invalid VL, and error recovery will work
correctly.

The sgid_tbl->tbl[] array is allocated in bnxt_qplib_alloc_sgid_tbl().
It has sgid_tbl->max elements. So the > should be >= to prevent

Once the qp has been flushed, it cannot be flushed again. The user qp
flush logic wasn't enforcing it however. The bug can cause

Inside of start_xmit() the call to check if the connection is up and the
queueing of the packets for later transmission is not atomic which leaves
a window where cm_rep_handler can run, set the connection up, dequeue
pending packets and leave the subsequently queued packets by start_xmit()
sitting on neigh->queue until they're dropped when the connection is torn
down. This only applies to connected mode. These dropped packets can
really upset TCP, for example, and cause multi-minute delays in
transmission for open connections.
Here's the code in start_xmit where we check to see if the connection is
up:

When AF_IB addresses are used during rdma_resolve_addr() a lock is not
held. A cma device can get removed while list traversal is in progress
which may lead to crash. ie

According to "Annex A16: RDMA over Converged Ethernet (RoCE)":
A16.4.3 MANAGEMENT INTERFACES
As defined in the base specification, a special Queue Pair, QP0 is defined
solely for communication between subnet manager(s) and subnet management
agents. Since such an IB-defined subnet management architecture is outside
the scope of this annex, it follows that there is also no requirement that
a port which conforms to this annex be associated with a QP0. Thus, for
end nodes designed to conform to this annex, the concept of QP0 is
undefined and unused for any port connected to an Ethernet network.
CA16-8: A packet arriving at a RoCE port containing a BTH with the
destination QP field set to QP0 shall be silently dropped.

Currently if the cm_id is not bound to any netdevice, than for such cm_id,
net namespace is ignored; which is incorrect.
Regardless of cm_id bound to a netdevice or not, net namespace must
match. When a cm_id is bound to a netdevice, in such case net namespace
and netdevice both must match.

hns bitmap allocation functions return 0 on success and -1 on failure.

The commit cited below checked that the port numbers provided in the
primary and alt AVs are legal.
That is sufficient to prevent a kernel panic. However, it is not
sufficient for correct operation.
In Linux, AVs (both primary and alt) must be completely self-described.
We do not accept an AV from userspace without an embedded port number.

Flows can be created on UD and RAW_PACKET QP types. Attempts to provide
other QP types as an input causes to various unpredictable failures.
The reason is that in order to support all various types (e.g. XRC), we
are supposed to use real_qp handle and not qp handle and expect to
driver/FW to fail such (XRC) flows. The simpler and safer variant is to
ban all QP types except UD and RAW_PACKET, instead of relying on
driver/FW.

ucma_process_join() will free the new allocated "mc" struct,
if there is any error after that, especially the copy_to_user().
But in parallel, ucma_leave_multicast() could find this "mc"
through idr_find() before ucma_process_join() frees it, since it
is already published.
So "mc" could be used in ucma_leave_multicast() after it is been
allocated and freed in ucma_process_join(), since we don't refcnt
it.
Fix this by separating "publish" from ID allocation, so that we
can get an ID first and publish it later after copy_to_user().

In commit 357d23c811a7 ("Remove the obsolete libibcm library")
in rdma-core [1], we removed obsolete library which used the
/dev/infiniband/ucmX interface.
Following multiple syzkaller reports about non-sanitized
user input in the UCMA module, the short audit reveals the same
issues in UCM module too.
It is better to disable this interface in the kernel,
before syzkaller team invests time and energy to harden
this unused interface.

The __get_txreq() function can return a pointer, ERR_PTR(-EBUSY), or NULL.
All of the relevant call sites look for IS_ERR, so the NULL return would
lead to a NULL pointer exception.
Do not use the ERR_PTR mechanism for this function.
Update all call sites to handle the return value correctly.
Clean up error paths to reflect return value.

The following code fails to allocate a buffer for the
tail address that the hardware DMAs into when the user
context DMA_RTAIL is set.

The fix is to test for both user and kernel DMA_TAIL options

rcvhdrtail_kvaddr during the mmap processing.
Additionally, all downstream testing of the capmask for DMA_RTAIL
have been eliminated in favor of testing rcvhdrtail_kvaddr.

All threads queuing CQ entries on different CQs are unnecessarily
synchronized by a spin lock to check if the CQ kthread worker hasn't
been destroyed before queuing an CQ entry.
The lock used in 6efaf10f16 ("IB/rdmavt: Avoid queuing work into a
destroyed cq kthread worker") is a device global lock and will have
poor performance at scale as completions are entered from a large
number of CPUs.
Convert to use RCU where the read side of RCU is rvt_cq_enter() to
determine that the worker is alive prior to triggering the
completion event.
Apply write side RCU semantics in rvt_driver_cq_init() and
rvt_cq_exit().

User send context integrity bits are cleared before the context is
disabled. If the send context is still processing data, any packets
that need those integrity bits will cause an error and halt the send
context.
During the disable handling, the driver waits for the context to drain.
If the context is halted, the driver will eventually timeout because
the context won't drain and then incorrectly bounce the link.
Reorder the bit clearing and the context disable.
Examine the software state and send context status as well as the
egress status to determine if a send context is in the halted state.
Promote the check macros to static functions for consistency with the
new check and to follow kernel style.
Remove an unused define that refers to the egress timeout.

On fatal error the driver simulates CQE's for ULPs that rely on
completion of all their posted work-request.
For the GSI traffic, the mlx5 has its own mechanism that sends the
completions via software CQE's directly to the relevant CQ.
This should be kept in fatal error too, so the driver should simulate
such CQE's with the specified error state in order to complete GSI QP
work requests.

To allow rereg_user_mr to modify the MR from read-only to writable without
using get_user_pages again, we needed to define the initial MR as writable.
However, this was originally done unconditionally, without taking into
account the writability of the underlying virtual memory.
As a result, any attempt to register a read-only MR over read-only
virtual memory failed.
To fix this, do not add the writable flag bit when the user virtual memory
is not writable (e.g. const memory).
However, when the underlying memory is NOT writable (and we therefore
do not define the initial MR as writable), the IB core adds a
"force writable" flag to its user-pages request. If this succeeds,
the reg_user_mr caller gets a writable copy of the original pages.
If the user-space caller then does a rereg_user_mr operation to enable
writability, this will succeed. This should not be allowed, since
the original virtual memory was not writable.

A warm restart will fail to unload the driver, leaving link state
potentially flapping up to the point the BIOS resets the adapter.
Correct the issue by hooking the shutdown pci method,
which will bring port down.

When commit [1] was added, SGID was queried to derive the SMAC address.
Then, later on during a refactor [2], SMAC was no longer needed. However,
the now useless GID query remained. Then during additional code changes
later on, the GID query was being done in such a way that it caused iWARP
queries to start breaking. Remove the useless GID query and resolve the
iWARP breakage at the same time.
This is discussed in [3].

When IRQ affinity is set and the interrupt type is unknown, a cpu

Currently, the kernel protects access to the agent ID allocator on a per
port basis using a spinlock, so it is impossible for two apps/threads on
the same port to get the same TID, but it is entirely possible for two
threads on different ports to end up with the same TID.
As this can be confusing (regardless of it being legal according to the
IB Spec 1.3, C13-18.1.1, in section 13.4.6.4 - TransactionID usage),
and as the rdma-core user space API for /dev/umad devices implies unique
TIDs even across ports, make the TID an atomic type so that no two
allocations, regardless of port number, will be the same.

When skb is sent, it will pass the following functions in soft roce.
rxe_send [rdma_rxe]
ip_local_out
__ip_local_out
ip_output
ip_finish_output
ip_finish_output2

dev_hard_start_xmit
In the above functions, if error occurs in the above functions or
iptables rules drop skb after ip_local_out, kfree_skb will be called.
So it is not necessary to call kfree_skb in soft roce module again.
Or else crash will occur.

w/o RXE_START_MASK, the last_psn of IB_OPCODE_RC_SEND_ONLY_INV
will not be updated in update_wqe_psn, and the corresponding
wqe will not be acked in rxe_completer due to its last_psn is
zero. Finally, the other wqe will also not be able to be acked,
because the wqe of IB_OPCODE_RC_SEND_ONLY_INV with last_psn 0
is still there. This causes large amount of io timeout when
nvmeof is over rxe.
Add RXE_START_MASK for IB_OPCODE_RC_SEND_ONLY_INV to fix this.

In the cases where iwpm_hash_bucket is NULL and where function
get_mapinfo_hash_bucket returns NULL then the map_info is never added
to hash_bucket_head and hence there is a leak of map_info. Fix this
by nullifying hash_bucket_head and if that is null we know that
that map_info was not added to hash_bucket_head and hence map_info
should be free'd.

There are few issues with validation of netdevice and listen id lookup
for IB (IPoIB) while processing incoming CM request as below.
1. While performing lookup of bind_list in cma_ps_find(), net namespace
of the netdevice can get deleted in cma_exit_net(), resulting in use
after free access of idr and/or net namespace structures.
This lookup occurs from the workqueue context (and not userspace
context where net namespace is always valid).

2. While netdevice is validated for IP address in given net namespace,
netdevice's net namespace and/or ifindex can change in
cma_get_net_dev() and cma_match_net_dev().
Above issues are overcome by using rcu lock along with netdevice
UP/DOWN state as described below.
When a net namespace is getting deleted, netdevice is closed and
shutdown before moving it back to init_net namespace.
change_net_namespace() synchronizes with any existing use of netdevice
before changing the netdev properties such as net or ifindex.
Once netdevice IFF_UP flags is cleared, such fields are not guaranteed
to be valid.
Therefore, rcu lock along with netdevice state check ensures that,
while route lookup and cm_id lookup is in progress, netdevice of
interest won't migrate to any other net namespace.
This ensures that associated net namespace of netdevice won't get
deleted while rcu lock is held for netdevice which is in IFF_UP state.

Previously, if a method contained mandatory attributes in a namespace
that wasn't given by the user, these attributes weren't validated.

When a GID entry is invalid EAGAIN is returned. This is an incorrect error
code, there is nothing that will make this GID entry valid again in
bounded time.
Some user space tools fail incorrectly if EAGAIN is returned here, and
this represents a small ABI change from earlier kernels.
The first patch in the Fixes list makes entries that were valid before
to become invalid, allowing this code to trigger, while the second patch
in the Fixes list introduced the wrong EAGAIN.
Therefore revert the return result to EINVAL which matches the historical
expectations of the ibv_query_gid_type() API of the libibverbs user space
library.

ah_attr contains the port number to which cm_id is bound. However, while
searching for GID table for matching GID entry, the port number is
ignored.
This could cause the wrong GID to be used when the ah_attr is converted to
an AH.

Before commit f1b65df5a2 ("IB/mlx5: Add support for active_width and
active_speed in RoCE"), the mlx5_ib driver set the default active_width
and active_speed to IB_WIDTH_4X and IB_SPEED_QDR.
When the RoCE port is down, the RoCE port does not negotiate the active
width with the remote side, causing the active width to be zero. When
running userspace ibstat to view the port status, ibstat will panic as it
reads an invalid width from sys file.
This patch restores the original behavior.

Once the FW is transitioned to error, FLUSH cqes can be received.
We want the driver to be aware of the fact that QP is already in error.
Without this fix, a user may see false error messages in the dmesg log,
mentioning that a FLUSH cqe was received while QP is not in error state.

Return code wasn't set properly when CNQ allocation failed.
This only affect error message logging, currently user will
receive an error message that says the qedr driver load failed
with rc '0', instead of ENOMEM

QPs that were configured with ack timeout value lower than 1
msec will not implement re-transmission timeout.
This means that if a packet / ACK were dropped, the QP
will not retransmit this packet.
This can lead to an application hang.

resolved_dev returned might be NULL as ifindex is transient number.
Ignoring NULL check of resolved_dev might crash the kernel.
Therefore perform NULL check before accessing resolved_dev.
Additionally rdma_resolve_ip_route() invokes addr_resolve() which
performs check and address translation for loopback ifindex.
Therefore, checking it again in rdma_resolve_ip_route() is not helpful.
Therefore, the code is simplified to avoid IFF_LOOPBACK check.

The commit cited below added a gid_type field (RoCEv1 or RoCEv2)
to GID properties.
When adding GIDs, this gid_type field was copied over to the
hardware gid table. However, when deleting GIDs, the gid_type field
was not copied over to the hardware gid table.
As a result, when running RoCEv2, all RoCEv2 gids in the
hardware gid table were set to type RoCEv1 when any gid was deleted.
This problem would persist until the next gid was added (which would again
restore the gid_type field for all the gids in the hardware gid table).
Fix this by copying over the gid_type field to the hardware gid table
when deleting gids, so that the gid_type of all remaining gids is
preserved when a gid is deleted.

When using IPv4 addresses in RoCEv2, the GID format for the mapped
IPv4 address should be: ::ffff:<4-byte IPv4 address>.
In the cited commit, IPv4 mapped IPV6 addresses had the 3 upper dwords
zeroed out by memset, which resulted in deleting the ffff field.
However, since procedure ipv6_addr_v4mapped() already verifies that the
gid has format ::ffff:<ipv4 address>, no change is needed for the gid,
and the memset can simply be removed.

IB spec says that a lid should be ignored when link layer is Ethernet,
for example when building or parsing a CM request message (CA17-34).
However, since ib_lid_be16() and ib_lid_cpu16() validates the slid,
not only when link layer is IB, we set the slid to zero to prevent
false warnings in the kernel log.

During driver unload, the driver proceeds with cleanup
without waiting for the scheduled events. So the device
pointers get freed up and driver crashes when the events
are scheduled later.
Flush the bnxt_re_task work queue before starting
device removal.

If remove_commit fails then the lock is left locked while the uobj still
exists. Eventually the kernel will deadlock.
lockdep detects this and says:

If the same attribute is listed twice by the user in the ioctl attribute
list then error unwind can cause the kernel to deref garbage.
This happens when an object with WRITE access is sent twice. The second
parse properly fails but corrupts the state required for the error unwind
it triggers.
Fixing this by making duplicates in the attribute list invalid. This is
not something we need to support.
The ioctl interface is currently recommended to be disabled in kConfig.

User-space may invoke ibv_reg_mr and ibv_dereg_mr in different threads.
If ibv_dereg_mr is called after the thread which invoked ibv_reg_mr has
exited, get_pid_task will return NULL and ib_umem_release will not
decrease mm->pinned_vm.
Instead of using threads to locate the mm, use the overall tgid from the
ib_ucontext struct instead. This matches the behavior of ODP and
disassociate in handling the mm of the process that called ibv_reg_mr.

A pio send egress error can occur when the PSM library attempts to
to send a bad packet. That issue is still being investigated.
The pio error interrupt handler then attempts to progress the recovery
of the errored pio send context.
Code inspection reveals that the handling lacks the necessary locking
if that recovery interleaves with a PSM close of the "context" object
contains the pio send context.
The lack of the locking can cause the recovery to access the already
freed pio send context object and incorrectly deduce that the pio
send context is actually a kernel pio send context as shown by the
NULL deref stack below:

When a CQ is shared by multiple QPs, c4iw_flush_hw_cq() needs to acquire
corresponding QP lock before moving the CQEs into its corresponding SW
queue and accessing the SQ contents for completing a WR.
Ignore CQEs if corresponding QP is already flushed.

When an invalid num_vls is used as a module parameter, the code
execution follows an exception path where the macro dd_dev_err()
expects dd->pcidev->dev not to be NULL in hfi1_init_dd(). This
causes a NULL pointer dereference.
Fix hfi1_init_dd() by initializing dd->pcidev and dd->pcidev->dev
earlier in the code. If a dd exists, then dd->pcidev and
dd->pcidev->dev always exists.

AHG may be armed to use the stored header, which by design is limited
to edits in the PSN/A 32 bit word (bth2).
When the code is trying to send a BECN, the use of the stored header
will lose the BECN bit.
Fix by avoiding AHG when getting ready to send a BECN. This is
accomplished by always claiming the packet is not a middle packet which
is an AHG precursor. BECNs are not a normal case and this should not
hurt AHG optimizations.

The code for handling a marked UD packet unconditionally returns the
dlid in the header of the FECN marked packet. This is not correct
for multicast packets where the DLID is in the multicast range.
The subsequent attempt to send the CNP with the multicast lid will
cause the chip to halt the ack send context because the source
lid doesn't match the chip programming. The send context will
be halted and flush any other pending packets in the pio ring causing
the CNP to not be sent.
A part of investigating the fix, it was determined that the 16B work
broke the FECN routine badly with inconsistent use of 16 bit and 32 bits
types for lids and pkeys. Since the port's source lid was correctly 32
bits the type mixmatches need to be dealt with at the same time as
fixing the CNP header issue.
Fix these issues by:
- Using the ports lid for as the SLID for responding to FECN marked UD
packets
- Insure pkey is always 16 bit in this and subordinate routines

Before the change, if the user passed a static rate value different
than zero and the FW doesn't support static rate,
it would end up configuring rate of 2.5 GBps.
Fix this by using rate 0; unlimited, in cases where FW
doesn't support static rate configuration.

Failure in rereg MR releases UMEM but leaves the MR to be destroyed
by the user. As a result the following scenario may happen:
"create MR -> rereg MR with failure -> call to rereg MR again" and
hit "NULL-ptr deref or user memory access" errors.
Ensure that rereg MR is only performed on a non-dead MR.

The RDMA CM will select a source device and address by consulting
the routing table if no source address is passed into
rdma_resolve_address(). Userspace will ask for this by passing an
all-zero source address in the RESOLVE_IP command. Unfortunately
the new check for non-zero address size rejects this with EINVAL,
which breaks valid userspace applications.
Fix this by explicitly allowing a zero address family for the source.

The c4iw_rdev_close() logic was not releasing all the hw
resources (PBL and RQT memory) during the device removal
event (driver unload / system reboot). This can cause panic
in gen_pool_destroy().
The module remove function will wait for all the hw
resources to be released during the device removal event.

On reboot SM can program port pkey table before ipoib registered its
event handler, which could result in missing pkey event and leave root
interface with initial pkey value from index 0.
Since OPA port starts with invalid pkey in index 0, root interface will
fail to initialize and stay down with no-carrier flag.
For IB ipoib interface may end up with pkey different from value
opensm put in pkey table idx 0, resulting in connectivity issues
(different mcast groups, for example).
Close the window by calling event handler after registration
to make sure ipoib pkey is in sync with port pkey table.

The dd refcount is speculatively incremented prior to allocating
the fd memory with kzalloc(). If that kzalloc() failed the dd
refcount leaks.
Increment refcount on kzalloc success.

The pci_request_irq() interfaces always adds the IRQF_SHARED bit to
all IRQ requests.
When the kernel is built with CONFIG_DEBUG_SHIRQ config flag, if the
IRQF_SHARED bit is set, a call to the IRQ handler is made from the
__free_irq() function. This is testing a race condition between the
IRQ cleanup and an IRQ racing the cleanup. The HFI driver should be
able to handle this race, but does not.

Export IRQ cleanup function so it can be called from other modules.
Using the exported cleanup function:
Re-order the driver cleanup code to clean up IRQ resources before

Failure in XRCD FW deallocation command leaves memory leaked and
returns error to the user which he can't do anything about it.
This patch changes behavior to always free memory and always return
success to the user.

Returning EOPNOTSUPP is problematic because it can also be
returned by the method function, and we use it in quite a few
places in drivers these days.
Instead, dedicate EPROTONOSUPPORT to indicate that the ioctl framework
is enabled but the requested object and method are not supported by
the kernel. No other case will return this code, and it lets userspace
know to fall back to write().
grep says we do not use it today in drivers/infiniband subsystem.

If valid netdevice is not found for RoCE, GID table should not be
searched with NULL netdevice.
Doing so causes the search routines to ignore the netdev argument and may
match the wrong GID table entry if the netdev is deleted.

polling the completion queue directly does not interfere
with the existing polling logic, hence drop the requirement.
Be aware that running ib_process_cq_direct with non IB_POLL_DIRECT
CQ may trigger concurrent CQ processing.
This can be used for polling mode ULPs.

If the application invalidates the MR before the FMR WR, HW parses the
consumer key portion of the stag and returns an invalid stag key
Asynchronous Event (AE) that tears down the QP.
Fix this by zeroing-out the consumer key portion of the allocated stag
returned to application for FMR.

Ensure that cv_end is equal to ibdev->num_comp_vectors for the
NUMA node with the highest index. This patch improves spreading
of RDMA channels over completion vectors and thereby improves
performance, especially on systems with only a single NUMA node.
This patch drops support for the comp_vector login parameter by
ignoring the value of that parameter since I have not found a
good way to combine support for that parameter and automatic
spreading of RDMA channels over completion vectors.

did not really matter whether or not abort handlers like srp_abort()
called .scsi_done() when returning another value than SUCCESS. Since
that commit however this matters. Hence only call .scsi_done() when

The 'if' logic in ucma_query_path was broken with OPA was introduced
and started to treat RoCE paths as as OPA paths. Invert the logic
of the 'if' so only OPA paths are treated as OPA paths.
Otherwise the path records returned to rdma_cma users are mangled
when in RoCE mode.

CQ allocation does not ensure that completion queue entries
and the completion queue structure are allocated on the correct
numa node.
Fix by allocating the rvt_cq and kernel CQ entries on the device node,
leaving the user CQ entries on the default local node. Also ensure
CQ resizes use the correct allocator when extending a CQ.

There are several places in the ucma ABI where userspace can pass in a
sockaddr but set the address family to AF_IB. When that happens,
rdma_addr_size() will return a size bigger than sizeof struct sockaddr_in6,
and the ucma kernel code might end up copying past the end of a buffer
not sized for a struct sockaddr_ib.
Fix this by introducing new variants
int rdma_addr_size_in6(struct sockaddr_in6 *addr);
int rdma_addr_size_kss(struct __kernel_sockaddr_storage *addr);
that are type-safe for the types used in the ucma ABI and return 0 if the
size computed is bigger than the size of the type passed in. We can use
these new variants to check what size userspace has passed in before
copying any addresses.

cma_port_is_unique() allows local port reuse if the quad (source
address and port, destination address and port) for this connection
is unique. However, if the destination info is zero or unspecified, it
can't make a correct decision but still allows port reuse. For example,
sometimes rdma_bind_addr() is called with unspecified destination and
reusing the port can lead to creating a connection with a duplicate quad,
after the destination is resolved. The issue manifests when MPI scale-up
tests hang after the duplicate quad is used.
Set the destination address family and add checks for zero destination
address and port to prevent source port reuse based on invalid destination.

Debugfs file reset_stats is created with S_IRUSR permissions,
but ocrdma_dbgfs_ops_read() doesn't support OCRDMA_RESET_STATS,
whereas ocrdma_dbgfs_ops_write() supports only OCRDMA_RESET_STATS.
The patch fixes misstype with permissions.
Found by Linux Driver Verification project (linuxtesting.org).

The ipoib path database is organized around DGIDs from the LLADDR, but the
SA is free to return a different GID when asked for path. This causes a
bug because the SA's modified DGID is copied into the database key, even
though it is no longer the correct lookup key, causing a memory leak and
other malfunctions.
Ensure the database key does not change after the SA query completes.
Demonstration of the bug is as follows
ipoib wants to send to GID fe80:0000:0000:0000:0002:c903:00ef:5ee2, it
creates new record in the DB with that gid as a key, and issues a new
request to the SM.
Now, the SM from some reason returns path-record with other SGID (for
example, 2001:0000:0000:0000:0002:c903:00ef:5ee2 that contains the local
subnet prefix) now ipoib will overwrite the current entry with the new
one, and if new request to the original GID arrives ipoib will not find
it in the DB (was overwritten) and will create new record that in its
turn will also be overwritten by the response from the SM, and so on
till the driver eats all the device memory.

rvt_mregion uses percpu_ref for reference counting and RCU to protect
accesses from lkey_table. When a rvt_mregion needs to be freed, it
first gets unregistered from lkey_table and then rvt_check_refs() is
called to wait for in-flight usages before the rvt_mregion is freed.
rvt_check_refs() seems to have a couple issues.
* It has a fast exit path which tests percpu_ref_is_zero(). However,
a percpu_ref reading zero doesn't mean that the object can be
released. In fact, the ->release() callback might not even have
started executing yet. Proceeding with freeing can lead to
use-after-free.
* lkey_table is RCU protected but there is no RCU grace period in the
free path. percpu_ref uses RCU internally but it's sched-RCU whose
grace periods are different from regular RCU. Also, it generally
isn't a good idea to depend on internal behaviors like this.
To address the above issues, this patch removes the fast exit and adds
an explicit synchronize_rcu().

I've looked at all those findings again and noticed that they are all
with CONFIG_INFINIBAND_USER_MEM=n, which means ib_umem_get() returns
an error unconditionally and we never initialize or use those variables.
This triggers a condition in gcc iff mr_umem_get() is partially but not
entirely inlined, which in turn depends on the exact combination of
optimization settings. This is a known problem with gcc, with no
easy solution in the compiler, so this adds another workaround that
should be more reliable than my previous attempt.
Returning an error from mlx5_ib_reg_user_mr() earlier means that we
can completely bypass the logic that caused the warning, the compiler
can now see that the variable is never accessed.

This is really being used as an assert that the expected usecnt
is being held and implicitly that the usecnt is valid. Rename it to
assert_uverbs_usecnt and tighten the checks to only accept valid
values of usecnt (eg 0 and < -1 are invalid).
The tigher checkes make the assertion cover more cases and is more
likely to find bugs via syzkaller/etc.

Users of ucma are supposed to provide size of option level,
in most paths it is supposed to be equal to u8 or u16, but
it is not the case for the IB path record, where it can be
multiple of struct ib_path_rec_data.
This patch takes simplest possible approach and prevents providing
values more than possible to allocate.

Holding locks is mandatory when calling __ib_device_get_by_index,
otherwise there are races during the list iteration with device removal.
Since the locks are static to device.c, __ib_device_get_by_index can
never be called correctly by any user out side the file.
Make the function static and provide a safe function that gets the
correct locks and returns a kref'd pointer. Fix all callers.

When using enhanced mode for IPoIB, two threads may execute xmit in
parallel to two different TX queues while the target is the same.
In this case, both of them will add the same neighbor to the path's

Analysis:
Two SKB are scheduled to be transmitted from two cores.
In ipoib_start_xmit, both gets NULL when calling ipoib_neigh_get.
Two calls to neigh_add_path are made. One thread takes the spin-lock
and calls ipoib_neigh_alloc which creates the neigh structure,
then (after the __path_find) the neigh is added to the path's neigh
link list. When the second thread enters the critical section it also
calls ipoib_neigh_alloc but in this case it gets the already allocated
ipoib_neigh structure, which is already linked to the path's neigh
link list and adds it again to the list. Which beside of triggering
the list, it creates a loop in the linked list. This loop leads to
endless loop inside path_rec_completion.
Solution:
Check list_empty(&neigh->list) before adding to the list.
