ad6561dffa module: trim exception table on init free.
It's theoretically possible that there are exception table entries
which point into the (freed) init text of modules.  These could cause
future problems if other modules get loaded into that memory and cause
an exception as we'd see the wrong fixup.  The only case I know of is
kvm-intel.ko (when CONFIG_CC_OPTIMIZE_FOR_SIZE=n).

Amerigo fixed this long-standing FIXME in the x86 version, but this
patch is more general.

This implements trim_init_extable(); most archs are simple since they
use the standard lib/extable.c sort code.  Alpha and IA64 use relative
addresses in their fixups, so thier trimming is a slight variation.

Sparc32 is unique; it doesn't seem to define ARCH_HAS_SORT_EXTABLE,
yet it defines its own sort_extable() which overrides the one in lib.
It doesn't sort, so we have to mark deleted entries instead of
actually trimming them.

9b02605826 sparc64: Kill bogus TPC/address truncation during 32-bit faults.
This builds upon eeabac7386
("sparc64: Validate kernel generated fault addresses on sparc64.")

Upon further consideration, we actually should never see any
fault addresses for 32-bit tasks with the upper 32-bits set.

If it does every happen, by definition it's a bug.  Whatever
context created that fault would only have that fault satisfied
if we used the full 64-bit address.  If we truncate it, we'll
always fault the wrong address and we'll always loop faulting
forever.

So catch such conditions and mark them as errors always.  Log
the error and fail the fault.

eeabac7386 sparc64: Validate kernel generated fault addresses on sparc64.
In order to handle all of the cases of address calculation overflow
properly, we run sparc 32-bit processes in "address masking" mode
when running on a 64-bit kernel.

Address masking mode zeros out the top 32-bits of the address
calculated for every load and store instruction.

However, when we're in privileged mode we have to run with that
address masking mode disabled even when accessing userspace from
the kernel.

To "simulate" the address masking mode we clear the top-bits by
hand for 32-bit processes in the fault handler.

It is the responsibility of code in the compat layer to properly
zero extend addresses used to access userspace.  If this isn't
followed properly we can get into a fault loop.

Say that the user address is 0xf0000000 but for whatever reason
the kernel code sign extends this to 64-bit, and then the kernel
tries to access the result.

In such a case we'll fault on address 0xfffffffff0000000 but the fault
handler will process that fault as if it were to address 0xf0000000.
We'll loop faulting forever because the fault never gets satisfied.

So add a check specifically for this case, when the kernel is faulting
on a user address access and the addresses don't match up.

This code path is sufficiently slow path, and this bug is sufficiently
painful to diagnose, that this kind of bug check is warranted.

    Quite a few 64-bit architectures are using `long' for their

Update types.h so we use unsigned long long for u64 and
fix all warnings in sparc64 code.
Tested with an allnoconfig, defconfig and allmodconfig builds.

This patch introduces additional warnings in several drivers.
These will be dealt with in separate patches.

The sparc allmodconfig build broke due to enabling of the
branch_tracer that does some very clever things with
all if conditions. This caused my gcc 3.4.5 to be so confused that
it emitted a warning:

arch/sparc/mm/fault_32.c:176: warning: 'fixup' might be used uninitialized in this function

And with -Werror this broke the build.

Refactor code so it:
1) becomes more readable
2) no longer emit a warning with the branch_tracer enabled

86ed40bd6f sparc: unify sections.h
While doing this use standard names for start/end
so we could use definitions straight from asm-generic

b4f4372f96 sparc64: Make %pil level 15 a pseudo-NMI.
So that we can profile code even in a local_irq_disable() section,
only write 14 (instead of 15) into the %pil register to disable IRQs.

This allows PIL level 15 to serve as a pseudo NMI.

0871420fad sparc64: Add tsb-ratio sysctl.
Add a sysctl to tweak the RSS limit used to decide when to grow
the TSB for an address space.

In order to avoid expensive divides and multiplies only simply
positive and negative powers of two are supported.

The function computed takes the number of TSB translations that will
fit at one time in the TSB of a given size, and either adds or
subtracts a percentage of entries.  This final value is the
RSS limit.

See tsb_size_to_rss_limit().

409832f548 sparc32 cpuinit flase positives
All noise since we don't have CPU hotplug there.  However, they
did expose something very odd-looking in there - poke_viking()
does a bunch of identical btfixup each time it's called (i.e.
for each CPU).  That one is left alone for now; just the trivial
misannotation fixes.

5110bd21b8 sparc: remove CONFIG_SUN4
While doing some easy cleanups on the sparc code I noticed that the
CONFIG_SUN4 code seems to be worse than the rest - there were some
"I don't know how it should work, but the current code definitely cannot
work." places.

And while I have seen people running Linux on machines like a
SPARCstation 5 a few years ago I don't recall having seen sun4
machines, even less ones running Linux.

e003934876 sparc32: Make IOMMU and IO-UNIT init work with device nodes.
And stick the iommu archdata pointer into the generic OF device tree
of_device struct as well.

We still have to pass the sbus_bus object down into the routines so
that the SBUS bus objects get the iommu cookies set properly.  After
drivers get converted to being pure OF drivers, that can go away.

9109fb7b35 mm: drop unneeded pgdat argument from free_area_init_node()
free_area_init_node() gets passed in the node id as well as the node
descriptor.  This is redundant as the function can trivially get the node
descriptor itself by means of NODE_DATA() and the node's id.

I checked all the users and NODE_DATA() seems to be usable everywhere
from where this function is called.

9f2b2a5f68 sparc32: More memory probing consolidation.
The PROM library function prom_meminit() builds a table,
prom_phys_avail[], just so that probe_memory() in
arch/sparc/mm/fault.c can copy it into sp_banks[].

Just have prom_meminit() fill in the sp_banks[] array directly, and
remove duplicated sort() function.

ccc34028d4 sparc32: Kill totally unused memory information tables.
The code in arch/sparc/prom/memory.c computes three tables, the list
of total memory, the list of available memory (total minus what
firmware is using), and the list of firmware taken memory.

Only the available memory list is even used.

Therefore, kill those unused tables and make prom_meminfo() return
just the available memory list.

2f569afd9c CONFIG_HIGHPTE vs. sub-page page tables.
Background: I've implemented 1K/2K page tables for s390.  These sub-page
page tables are required to properly support the s390 virtualization
instruction with KVM.  The SIE instruction requires that the page tables
have 256 page table entries (pte) followed by 256 page status table entries
(pgste).  The pgstes are only required if the process is using the SIE
instruction.  The pgstes are updated by the hardware and by the hypervisor
for a number of reasons, one of them is dirty and reference bit tracking.
To avoid wasting memory the standard pte table allocation should return
1K/2K (31/64 bit) and 2K/4K if the process is using SIE.

Problem: Page size on s390 is 4K, page table size is 1K or 2K.  That means
the s390 version for pte_alloc_one cannot return a pointer to a struct
page.  Trouble is that with the CONFIG_HIGHPTE feature on x86 pte_alloc_one
cannot return a pointer to a pte either, since that would require more than
32 bit for the return value of pte_alloc_one (and the pte * would not be
accessible since its not kmapped).

Solution: The only solution I found to this dilemma is a new typedef: a
pgtable_t.  For s390 pgtable_t will be a (pte *) - to be introduced with a
later patch.  For everybody else it will be a (struct page *).  The
additional problem with the initialization of the ptl lock and the
NR_PAGETABLE accounting is solved with a constructor pgtable_page_ctor and
a destructor pgtable_page_dtor.  The page table allocation and free
functions need to call these two whenever a page table page is allocated or
freed.  pmd_populate will get a pgtable_t instead of a struct page pointer.
 To get the pgtable_t back from a pmd entry that has been installed with
pmd_populate a new function pmd_pgtable is added.  It replaces the pmd_page
call in free_pte_range and apply_to_pte_range.

72a7fe3967 Introduce flags for reserve_bootmem()
This patchset adds a flags variable to reserve_bootmem() and uses the
BOOTMEM_EXCLUSIVE flag in crashkernel reservation code to detect collisions
between crashkernel area and already used memory.

This patch:

Change the reserve_bootmem() function to accept a new flag BOOTMEM_EXCLUSIVE.
If that flag is set, the function returns with -EBUSY if the memory already
has been reserved in the past.  This is to avoid conflicts.

Because that code runs before SMP initialisation, there's no race condition
inside reserve_bootmem_core().

dcca2bde4f During VM oom condition, kill all threads in process group
We have had complaints where a threaded application is left in a bad state
after one of it's threads is killed when we hit a VM: out_of_memory
condition.

Killing just one of the process threads can leave the application in a bad
state, whereas killing the entire process group would allow for the
application to restart, or be otherwise handled, and makes it very obvious
that something has gone wrong.

This change allows the entire process group to be taken down, rather
than just the one thread.

70b0e7a919 [SPARC32]: Remove superfluous 'kernel_end' alignment on sun4c.
In sun4c_init_clean_mmu(), aligning 'kernel_end' using
SUN4C_REAL_PGDIR_ALIGN() is unnecessary since the caller
does this already.

In sun4c_paging_init(), 4 page sizes of "fluff" were added
to the address of &end.  This was necessary a long time ago
when sparc32 would allocate some early data structures
by carving out memory chunks after &end but that no longer
occurs.

da4e9fea85 [SPARC]: Fix exec failures on sun4c.
This deals with a sun4c issue caused by commit b6a2fea393:
mm: variable length argument support.

The new way the code works means that sun4c_update_mmu_cache gets
called before a context has been selected, which results in invalid
operation of the underling mm code.

Simply ignoring update requests when there is no valid context solves
the problem.

1eeb66a1bb move die notifier handling to common code
This patch moves the die notifier handling to common code.  Previous
various architectures had exactly the same code for it.  Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)

arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at.  avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

Introduce pagefault_{disable,enable}() and use these where previously we did
manual preempt increments/decrements to make the pagefault handler do the
atomic thing.

Currently they still rely on the increased preempt count, but do not rely on
the disabled preemption, this might go away in the future.

(NOTE: the extra barrier() in pagefault_disable might fix some holes on
       machines which have too many registers for their own good)

46a82b2d55 [PATCH] Standardize pxx_page macros
One of the changes necessary for shared page tables is to standardize the
pxx_page macros.  pte_page and pmd_page have always returned the struct
page associated with their entry, while pte_page_kernel and pmd_page_kernel
have returned the kernel virtual address.  pud_page and pgd_page, on the
other hand, return the kernel virtual address.

Shared page tables needs pud_page and pgd_page to return the actual page
structures.  There are very few actual users of these functions, so it is
simple to standardize their usage.

Since this is basic cleanup, I am submitting these changes as a standalone
patch.  Per Hugh Dickins' comments about it, I am also changing the
pxx_page_kernel macros to pxx_page_vaddr to clarify their meaning.

Last I checked (with 2.6.14), random programs segfault with dual
HyperSPARC.  And with SuperSPARC II's, it seems stable but will
eventually die from a write lock error (wrong lock owner or something).

I haven't tried the HyperSPARC + highmem combination recently, so that
may still be a problem.

Remove set_page_count from outside mm/, and replace those users with
init_page_count() and set_page_refcounted().

This allows more debug checking, and tighter control on how code is allowed
to play around with page->_count.

Although we tend to associate VM_RESERVED with remap_pfn_range, quite a few
drivers set VM_RESERVED on areas which are then populated by nopage.  The
PageReserved removal in 2.6.15-rc1 changed VM_RESERVED not to free pages in
zap_pte_range, without changing those drivers not to set it: so their pages
just leak away.

Let's not change miscellaneous drivers now: introduce VM_UNPAGED at the core,
to flag the special areas where the ptes may have no struct page, or if they
have then it's not to be touched.  Replace most instances of VM_RESERVED in
core mm by VM_UNPAGED.  Force it on in remap_pfn_range, and the sparc and
sparc64 io_remap_pfn_range.

Revert addition of VM_RESERVED to powerpc vdso, it's not needed there.  Is it
needed anywhere?  It still governs the mm->reserved_vm statistic, and special
vmas not to be merged, and areas not to be core dumped; but could probably be
eliminated later (the drivers are probably specifying it because in 2.4 it
kept swapout off the vma, but in 2.6 we work from the LRU, which these pages
don't get on).

Use the VM_SHM slot for VM_UNPAGED, and define VM_SHM to 0: it serves no
purpose whatsoever, and should be removed from drivers when we clean up.

Convert those few architectures which are calling pud_alloc, pmd_alloc,
pte_alloc_map on a user mm, not to take the page_table_lock first, nor drop it
after.  Each of these can continue to use pte_alloc_map, no need to change
over to pte_alloc_map_lock, they're neither racy nor swappable.

In the sparc64 io_remap_pfn_range, flush_tlb_range then falls outside of the
page_table_lock: that's okay, on sparc64 it's like flush_tlb_mm, and that has
always been called from outside of page_table_lock in dup_mmap.

Remove PageReserved() calls from core code by tightening VM_RESERVED
handling in mm/ to cover PageReserved functionality.

PageReserved special casing is removed from get_page and put_page.

All setting and clearing of PageReserved is retained, and it is now flagged
in the page_alloc checks to help ensure we don't introduce any refcount
based freeing of Reserved pages.

MAP_PRIVATE, PROT_WRITE of VM_RESERVED regions is tentatively being
deprecated.  We never completely handled it correctly anyway, and is be
reintroduced in future if required (Hugh has a proof of concept).

Once PageReserved() calls are removed from kernel/power/swsusp.c, and all
arch/ and driver code, the Set and Clear calls, and the PG_reserved bit can
be trivially removed.

Last real user of PageReserved is swsusp, which uses PageReserved to
determine whether a struct page points to valid memory or not.  This still
needs to be addressed (a generic page_is_ram() should work).

A last caveat: the ZERO_PAGE is now refcounted and managed with rmap (and
thus mapcounted and count towards shared rss).  These writes to the struct
page could cause excessive cacheline bouncing on big systems.  There are a
number of ways this could be addressed if it is an issue.

1da177e4c3 Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
