Pull libnvdimm and DAX updates from Dan Williams:
 "New support for clearing memory errors when a file is in DAX mode,
  alongside with some other fixes and cleanups.

  Previously it was only possible to clear these errors using a truncate
  or hole-punch operation to trigger the filesystem to reallocate the
  block, now, any page aligned write can opportunistically clear errors
  as well.

  This change spans x86/mm, nvdimm, and fs/dax, and has received the
  appropriate sign-offs. Thanks to Jane for her work on this.

  Summary:

   - Miscellaneous cleanups and fixes for nfit_test (nvdimm unit tests)"

047218ec90 dax: add .recovery_write dax_operation
Introduce dax_recovery_write() operation. The function is used to
recover a dax range that contains poison. Typical use case is when
a user process receives a SIGBUS with si_code BUS_MCEERR_AR
indicating poison(s) in a dax range, in response, the user process
issues a pwrite() to the page-aligned dax range, thus clears the
poison and puts valid data in the range.

e511c4a3d2 dax: introduce DAX_RECOVERY_WRITE dax access mode
Up till now, dax_direct_access() is used implicitly for normal
access, but for the purpose of recovery write, dax range with
poison is requested. To make the interface clear, introduce
	enum dax_access_mode {
		DAX_ACCESS,
		DAX_RECOVERY_WRITE,
	}
where DAX_ACCESS is used for normal dax access, and
DAX_RECOVERY_WRITE is used for dax recovery write.

06083a0921 dax: fix missing writeprotect the pte entry
Currently dax_mapping_entry_mkclean() fails to clean and write protect the
pte entry within a DAX PMD entry during an *sync operation. This can
result in data loss in the following sequence:

  1) process A mmap write to DAX PMD, dirtying PMD radix tree entry and
     making the pmd entry dirty and writeable.
  2) process B mmap with the @offset (e.g. 4K) and @length (e.g. 4K)
     write to the same file, dirtying PMD radix tree entry (already
     done in 1)) and making the pte entry dirty and writeable.
  3) fsync, flushing out PMD data and cleaning the radix tree entry. We
     currently fail to mark the pte entry as clean and write protected
     since the vma of process B is not covered in dax_entry_mkclean().
  4) process B writes to the pte. These don't cause any page faults since
     the pte entry is dirty and writeable. The radix tree entry remains
     clean.
  5) fsync, which fails to flush the dirty PMD data because the radix tree
     entry was clean.
  6) crash - dirty data that should have been fsync'd as part of 5) could
     still have been in the processor cache, and is lost.

Just to use pfn_mkclean_range() to clean the pfns to fix this issue.

e583b5c472 dax: fix cache flush on PMD-mapped pages
The flush_cache_page() only remove a PAGE_SIZE sized range from the cache.
However, it does not cover the full pages in a THP except a head page.
Replace it with flush_cache_range() to fix this issue. This is just a
documentation issue with the respect to properly documenting the expected
usage of cache flushing before modifying the pmd. However, in practice
this is not a problem due to the fact that DAX is not available on
architectures with virtually indexed caches per:

952da06375 iomap: add a IOMAP_DAX flag
Add a flag so that the file system can easily detect DAX operations
based just on the iomap operation requested instead of looking at

but not things like fiemap that are based on the block device.
In the long run it should also allow turning the bdev, dax_dev

c6f4046865 fsdax: decouple zeroing from the iomap buffered I/O code
Unshare the DAX and iomap buffered I/O page zeroing code. This code

Pull iomap updates from Darrick Wong:
 "The most notable externally visible change for this cycle is the
  addition of support for reads to inline tail fragments of files, which
  was requested by the erofs developers; and a correction for a kernel
  memory corruption bug if the sysadmin tries to activate a swapfile
  with more pages than the swapfile header suggests.

  We also now report writeback completion errors to the file mapping
  correctly, instead of munging all errors into EIO.

  Internally, the bulk of the changes are Christoph's patchset to reduce
  the indirect function call count by a third to a half by converting
  iomap iteration from a loop pattern to a generator/consumer pattern.
  As an added bonus, fsdax no longer open-codes iomap apply loops.

1a14e3779d dax: fix ENOMEM handling in grab_mapping_entry()
grab_mapping_entry() has a bug in handling of ENOMEM condition. Suppose
we have a PMD entry at index i which we are downgrading to a PTE entry.
grab_mapping_entry() will set pmd_downgrade to true, lock the entry, clear
the entry in xarray, and decrement mapping->nrpages. The it will call:

	entry = dax_make_entry(pfn_to_pfn_t(0), flags);
	dax_lock_entry(xas, entry);

which inserts new PTE entry into xarray. However this may fail allocating
the new node. We handle this by:

	if (xas_nomem(xas, mapping_gfp_mask(mapping) & ~__GFP_HIGHMEM))
		goto retry;

however pmd_downgrade stays set to true even though 'entry' returned from
get_unlocked_entry() will be NULL now. And we will go again through the
downgrade branch. This is mostly harmless except that mapping->nrpages is
decremented again and we temporarily have an invalid entry stored in
xarray. Fix the problem by setting pmd_downgrade to false each time we
lookup the entry we work with so that it matches the entry we found.

237388320d dax: Wake up all waiters after invalidating dax entry
I am seeing missed wakeups which ultimately lead to a deadlock when I am
using virtiofs with DAX enabled and running "make -j". I had to mount
virtiofs as rootfs and also reduce to dax window size to 256M to reproduce
the problem consistently.

So here is the problem. put_unlocked_entry() wakes up waiters only
if entry is not null as well as !dax_is_conflict(entry). But if I
call multiple instances of invalidate_inode_pages2() in parallel,
then I can run into a situation where there are waiters on
this index but nobody will wake these waiters.

invalidate_inode_pages2()
  invalidate_inode_pages2_range()
    invalidate_exceptional_entry2()
      dax_invalidate_mapping_entry_sync()
        __dax_invalidate_entry() {
                xas_lock_irq(&xas);
                entry = get_unlocked_entry(&xas, 0);
                ...
                ...
                dax_disassociate_entry(entry, mapping, trunc);
                xas_store(&xas, NULL);
                ...
                ...
                put_unlocked_entry(&xas, entry);
                xas_unlock_irq(&xas);
        }

Say a fault in in progress and it has locked entry at offset say "0x1c".
Now say three instances of invalidate_inode_pages2() are in progress
(A, B, C) and they all try to invalidate entry at offset "0x1c". Given
dax entry is locked, all tree instances A, B, C will wait in wait queue.

When dax fault finishes, say A is woken up. It will store NULL entry
at index "0x1c" and wake up B. When B comes along it will find "entry=0"
at page offset 0x1c and it will call put_unlocked_entry(&xas, 0). And
this means put_unlocked_entry() will not wake up next waiter, given
the current code. And that means C continues to wait and is not woken
up.

This patch fixes the issue by waking up all waiters when a dax entry
has been invalidated. This seems to fix the deadlock I am facing
and I can make forward progress.

7716506ada mm: introduce and use mapping_empty()
Patch series "Remove nrexceptional tracking", v2.

We actually use nrexceptional for very little these days. It's a minor
pain to keep in sync with nrpages, but the pain becomes much bigger with
the THP patches because we don't know how many indices a shadow entry
occupies. It's easier to just remove it than keep it accurate.

Also, we save 8 bytes per inode which is nothing to sneeze at; on my
laptop, it would improve shmem_inode_cache from 22 to 23 objects per
16kB, and inode_cache from 26 to 27 objects. Combined, that saves
a megabyte of memory from a combined usage of 25MB for both caches.
Unfortunately, ext4 doesn't cross a magic boundary, so it doesn't save
any memory for ext4.

This patch (of 4):

Instead of checking the two counters (nrpages and nrexceptional), we can
just check whether i_pages is empty.

6bbdd563ee dax: Create a range version of dax_layout_busy_page()
virtiofs device has a range of memory which is mapped into file inodes
using dax. This memory is mapped in qemu on host and maps different
sections of real file on host. Size of this memory is limited
(determined by administrator) and depending on filesystem size, we will
soon reach a situation where all the memory is in use and we need to
reclaim some.

As part of reclaim process, we will need to make sure that there are
no active references to pages (taken by get_user_pages()) on the memory
range we are trying to reclaim. I am planning to use
dax_layout_busy_page() for this. But in current form this is per inode
and scans through all the pages of the inode.

We want to reclaim only a portion of memory (say 2MB page). So we want
to make sure that only that 2MB range of pages do not have any
references (and don't want to unmap all the pages of inode).

Hence, create a range version of this function named
dax_layout_busy_page_range() which can be used to pass a range which
needs to be unmapped.

3f666c56c6 dax: Pass dax_dev instead of bdev to dax_writeback_mapping_range()
As of now dax_writeback_mapping_range() takes "struct block_device" as a
parameter and dax_dev is searched from bdev name. This also involves taking
a fresh reference on dax_dev and putting that reference at the end of
function.

We are developing a new filesystem virtio-fs and using dax to access host
page cache directly. But there is no block device. IOW, we want to make
use of dax but want to get rid of this assumption that there is always
a block device associated with dax_dev.

So pass in "struct dax_device" as parameter instead of bdev.

ext2/ext4/xfs are current users and they already have a reference on
dax_device. So there is no need to take reference and drop reference to
dax_device on each call of this function.

Merge tag 'iomap-5.5-merge-11' of git://git.kernel.org/pub/scm/fs/xfs/xfs-linux

Pull iomap updates from Darrick Wong:
 "In this release, we hoisted as much of XFS' writeback code into iomap
  as was practicable, refactored the unshare file data function, added
  the ability to perform buffered io copy on write, and tweaked various
  parts of the directio implementation as needed to port ext4's directio
  code (that will be a separate pull).

  Summary:

   - Make iomap_dio_rw callers explicitly tell us if they want us to
     wait

   - Port the xfs writeback code to iomap to complete the buffered io
     library functions

   - Refactor the unshare code to share common pieces

   - Add support for performing copy on write with buffered writes

   - Other minor fixes

   - Fix unchecked return in iomap_bmap

   - Fix a type casting bug in a ternary statement in
     iomap_dio_bio_actor

   - Improve tracepoints for easier diagnostic ability

   - Fix pipe page leakage in directio reads"

6370740e5f fs/dax: Fix pmd vs pte conflict detection
Users reported a v5.3 performance regression and inability to establish
huge page mappings. A revised version of the ndctl "dax.sh" huge page
unit test identifies commit 23c84eb783 "dax: Fix missed wakeup with
PMD faults" as the source.

Update get_unlocked_entry() to check for NULL entries before checking
the entry order, otherwise NULL is misinterpreted as a present pte
conflict. The 'order' check needs to happen before the locked check as
an unlocked entry at the wrong order must fallback to lookup the correct
order.

c039b99792 iomap: use a srcmap for a read-modify-write I/O
The srcmap is used to identify where the read is to be performed from.
It is passed to ->iomap_begin, which can fill it in if we need to read
data for partially written blocks from a different location than the
write target. The srcmap is only supported for buffered writes so far.

d75996dd02 dax: dax_layout_busy_page() should not unmap cow pages
Vivek:

    "As of now dax_layout_busy_page() calls unmap_mapping_range() with last
     argument as 1, which says even unmap cow pages. I am wondering who needs
     to get rid of cow pages as well.

     I noticed one interesting side affect of this. I mount xfs with -o dax and
     mmaped a file with MAP_PRIVATE and wrote some data to a page which created
     cow page. Then I called fallocate() on that file to zero a page of file.
     fallocate() called dax_layout_busy_page() which unmapped cow pages as well
     and then I tried to read back the data I wrote and what I get is old
     data from persistent memory. I lost the data I had written. This
     read basically resulted in new fault and read back the data from
     persistent memory.

     This sounds wrong. Are there any users which need to unmap cow pages
     as well? If not, I am proposing changing it to not unmap cow pages.

     I noticed this while while writing virtio_fs code where when I tried
     to reclaim a memory range and that corrupted the executable and I
     was running from virtio-fs and program got segment violation."

Dan:

    "In fact the unmap_mapping_range() in this path is only to synchronize
     against get_user_pages_fast() and force it to call back into the
     filesystem to re-establish the mapping. COW pages should be left
     untouched by dax_layout_busy_page()."

61c30c98ef dax: Fix missed wakeup in put_unlocked_entry()
The condition checking whether put_unlocked_entry() needs to wake up
following waiter got broken by commit 23c84eb783 ("dax: Fix missed
wakeup with PMD faults"). We need to wake the waiter whenever the passed
entry is valid (i.e., non-NULL and not special conflict entry). This
could lead to processes never being woken up when waiting for entry
lock. Fix the condition.
