9dbdac024d staging: octeon-usb: move driver out of staging
The Octeon usb driver has been in staging for a long time and used in
Ubiquiti routers for a while now.
It's been built and then tested on real hardware with several usb devices
and it is proven to be stable and ready to be moved to its proper place
in the kernel tree.

Move it to drivers/usb/host and adjust its Makefile, Kconfig and defconfig
dependencies.

Many thanks to the developers who made it happen.

96b06c0a16 Revert "staging: octeon-usb: delete the octeon usb host controller driver"
This reverts commit 95ace52e40. Re-instate
the code so subsequent commits can clean it up and get it building
properly.

95ace52e40 staging: octeon-usb: delete the octeon usb host controller driver
This driver was merged back in 2013 and shows no progress toward every
being merged into the "correct" part of the kernel.  The code doesn't
even build for anyone unless you have the specific hardware platform
selected, so odds are it doesn't even work anymore.

Remove it for now and is someone comes along that has the hardware and
is willing to fix it up, it can be reverted.

af8e4c71f6 staging: octeon-usb: use __BITFIELD_FIELD
Use __BITFIELD_FIELD to define bitfields. OCTEON can at least in theory
run also in little-endian mode, so the bitfield definitions should not
assume big-endian byte order.

f121910396 staging: octeon-usb: use a single .h file
Merge USBC and USBN register definitions into a single header
file. Although all HW definitions are purely internal to the driver,
it's better to keep them separate due to the large size of the file.
