Vineeth Vijayan 89c0c62e94 s390/cio: unregister device when the only path is gone
Currently, if the device is offline and all the channel paths are
either configured or varied offline, the associated subchannel gets
unregistered. Don't unregister the subchannel, instead unregister
offline device.

Vineeth Vijayan b1b0d5aec1 s390/cio: include subchannels without devices also for evaluation
Currently when the new channel-path is enabled, we do evaluation only
on the subchannels with a device connected on it. This is because,
in the past, if the device in the subchannel is not working or not
available, we used to unregister the subchannels. But, from the 'commit
2297791c92 ("s390/cio: dont unregister subchannel from child-drivers")'
we allow subchannels with or without an active device connected
on it. So, when we do the io_subchannel_verify, make sure that,
we are evaluating the subchannels without any device too.

Here is the large set of driver core changes for 6.3-rc1.
 
 There's a lot of changes this development cycle, most of the work falls
 into two different categories:
   - fw_devlink fixes and updates.  This has gone through numerous review
     cycles and lots of review and testing by lots of different devices.
     Hopefully all should be good now, and Saravana will be keeping a
     watch for any potential regression on odd embedded systems.
   - driver core changes to work to make struct bus_type able to be moved
     into read-only memory (i.e. const)  The recent work with Rust has
     pointed out a number of areas in the driver core where we are
     passing around and working with structures that really do not have
     to be dynamic at all, and they should be able to be read-only making
     things safer overall.  This is the contuation of that work (started
     last release with kobject changes) in moving struct bus_type to be
     constant.  We didn't quite make it for this release, but the
     remaining patches will be finished up for the release after this
     one, but the groundwork has been laid for this effort.
 
 Other than that we have in here:
   - debugfs memory leak fixes in some subsystems
   - error path cleanups and fixes for some never-able-to-be-hit
     codepaths.
   - cacheinfo rework and fixes
   - Other tiny fixes, full details are in the shortlog
 
 All of these have been in linux-next for a while with no reported
 problems.

Vineeth Vijayan 0c6924c262 s390/cio: introduce locking for register/unregister functions
Unbinding an I/O subchannel with a child-CCW device in disconnected
state sometimes causes a kernel-panic. The race condition was seen
mostly during testing, when setting all the CHPIDs of a device to
offline and at the same time, the unbinding the I/O subchannel driver.

The kernel-panic occurs because of double delete, the I/O subchannel
driver calls device_del on the CCW device while another device_del
invocation for the same device is in-flight.  For instance, disabling
all the CHPIDs will trigger the ccw_device_remove function, which will
call a ccw_device_unregister(), which ends up calling the device_del()
which is asynchronous via cdev's todo workqueue. And unbinding the I/O
subchannel driver calls io_subchannel_remove() function which calls the
ccw_device_unregister() and device_del().

This double delete can be prevented by serializing all CCW device
registration/unregistration calls into the driver core. This patch
introduces a mutex which will be used for this purpose.

Greg Kroah-Hartman 2a81ada32f driver core: make struct bus_type.uevent() take a const *
The uevent() callback in struct bus_type should not be modifying the
device that is passed into it, so mark it as a const * and propagate the
function signature changes out into all relevant subsystems that use
this callback.

Alexander Gordeev 5c2e5a0cf5 s390/cio: sort out physical vs virtual pointers usage
This does not fix a real bug, since virtual addresses
are currently indentical to physical ones.

Use virt_to_phys() for intparm interrupt parameter to
convert a 64-bit virtual address to the 32-bit physical
address, which is expected to be below 2GB.

Vineeth Vijayan dd9cb842fa s390/cio: verify the driver availability for path_event call
If no driver is attached to a device or the driver does not provide the
path_event function, an FCES path-event on this device could end up in a
kernel-panic. Verify the driver availability before the path_event
function call.

Vineeth Vijayan a84d1c5006 s390/cio: remove uevent suppress from cio driver
commit fa1a8c23eb ("s390: cio: Delay uevents for subchannels")
introduced suppression of uevents for a subchannel until after it is
clear that the subchannel would not be unregistered again
immediately. This was done to avoid uevents being generated for I/O
subchannels with no valid device, which can happen on LPAR.

However, this also has some drawbacks: All subchannel drivers need to
manually remove the uevent suppression and generate an ADD uevent as
soon as they are sure that the subchannel will stay around. This misses
out on all uevents that are not the initial ADD uevent that would be
generated while uevents are suppressed; for example, all subchannels
were missing the BIND uevent.

As uevents being generated even for I/O subchannels without an
operational device turned out to be not as bad as missing uevents and
complicating the code flow, let's remove uevent suppression for
subchannels.

Vineeth Vijayan fa172f043f s390/cio: unregister the subchannel while purging
The cio_ignore list is used to create and maintain the list of devices
which is to be ignored by Linux. During boot-time, this list is adjusted
and accommodate all the devices which are configured on the HMC
interface. Once these devices are accessible, they are then available to
Linux and set online.

cio_ignore purge function should align with this functionality. But
currently, the subchannel associated with the offline-devices are not
unregistered during purge. Add an explicit subchannel-unregister function
in the purge_fn callback.

Vineeth Vijayan 2297791c92 s390/cio: dont unregister subchannel from child-drivers
The devices owned by the parent-driver (css) was getting unregistered
from the io-subchannel driver is clearly a layering violation.
Remove the subchannel unregistration from the child-drivers.

This also means, if the device connected to the subchannel is not
operational, or not accessible, the subchannel will not be unregistered.
Instead the CIO layer will allow valid subchannels without any operational
devices in sysfs. And the userspace tooling might need to be modified to
optimally handle this new situation.

Uwe Kleine-König fc7a6209d5 bus: Make remove callback return void
The driver core ignores the return value of this callback because there
is only little it can do when a device disappears.

This is the final bit of a long lasting cleanup quest where several
buses were converted to also return void from their remove callback.
Additionally some resource leaks were fixed that were caused by drivers
returning an error code in the expectation that the driver won't go
away.

With struct bus_type::remove returning void it's prevented that newly
implemented buses return an ignored error code and so don't anticipate
wrong expectations for driver authors.

Uwe Kleine-König a7bdb9a976 s390/cio: Make struct css_driver::remove return void
The driver core ignores the return value of css_remove()
(because there is only little it can do when a device disappears) and
all callbacks return 0 anyhow.

So make it impossible for future drivers to return an unused error code
by changing the remove prototype to return void.

The real motivation for this change is the quest to make struct
bus_type::remove return void, too.

Vineeth Vijayan 2f7484fd73 s390/cio: remove invalid condition on IO_SCH_UNREG
The condition to check the cdev pointer validity on
css_sch_device_unregister() is a leftover from the 'commit 8cc0dcfdc1
("s390/cio: remove pm support from ccw bus driver")'. This could lead to a
situation, where detaching the device is not happening completely. Remove
this invalid condition in the IO_SCH_UNREG case.

Julian Wiedmann 4520a91a97 s390/cio: use dma helpers for setting masks
Bypassing the DMA API is bad style, even when we don't expect any
actual problems. Let's utilize the right API helpers for setting the
DMA masks and check for returned errors, so that we benefit from
common sanity checks.

io_subchannel_allocate_dev() required some extra massaging, so that we
can return an errno other than -ENOMEM.

Set the bus type when initializing the cdev structure. The device core
won't act on it until we call device_add().

Cornelia Huck 05ce3e53f3 s390/cio: avoid duplicated 'ADD' uevents
The common I/O layer delays the ADD uevent for subchannels and
delegates generating this uevent to the individual subchannel
drivers. The io_subchannel driver will do so when the associated
ccw_device has been registered -- but unconditionally, so more
ADD uevents will be generated if a subchannel has been unbound
from the io_subchannel driver and later rebound.

To fix this, only generate the ADD event if uevents were still
suppressed for the device.

Halil Pasic 05668e1d74 s390/cio: fix virtio-ccw DMA without PV
Commit 37db8985b2 ("s390/cio: add basic protected virtualization
support") breaks virtio-ccw devices with VIRTIO_F_IOMMU_PLATFORM for non
Protected Virtualization (PV) guests. The problem is that the dma_mask
of the ccw device, which is used by virtio core, gets changed from 64 to
31 bit, because some of the DMA allocations do require 31 bit
addressable memory. For PV the only drawback is that some of the virtio
structures must end up in ZONE_DMA because we have the bounce the
buffers mapped via DMA API anyway.

But for non PV guests we have a problem: because of the 31 bit mask
guests bigger than 2G are likely to try bouncing buffers. The swiotlb
however is only initialized for PV guests, because we don't want to
bounce anything for non PV guests. The first such map kills the guest.

Since the DMA API won't allow us to specify for each allocation whether
we need memory from ZONE_DMA (31 bit addressable) or any DMA capable
memory will do, let us use coherent_dma_mask (which is used for
allocations) to force allocating form ZONE_DMA while changing dma_mask
to DMA_BIT_MASK(64) so that at least the streaming API will regard
the whole memory DMA capable.

Suzuki K Poulose 92ce7e83b4 driver_find_device: Unify the match function with class_find_device()
The driver_find_device() accepts a match function pointer to
filter the devices for lookup, similar to bus/class_find_device().
However, there is a minor difference in the prototype for the
match parameter for driver_find_device() with the now unified
version accepted by {bus/class}_find_device(), where it doesn't
accept a "const" qualifier for the data argument. This prevents
us from reusing the generic match functions for driver_find_device().

For this reason, change the prototype of the driver_find_device() to
make the "match" parameter in line with {bus/class}_find_device()
and adjust its callers to use the const qualifier. Also, we could
now promote the "data" parameter to const as we pass it down
as a const parameter to the match functions.

There is an arbitrary difference between the prototypes of
bus_find_device() and class_find_device() preventing their callers
from passing the same pair of data and match() arguments to both of
them, which is the const qualifier used in the prototype of
class_find_device().  If that qualifier is also used in the
bus_find_device() prototype, it will be possible to pass the same
match() callback function to both bus_find_device() and
class_find_device(), which will allow some optimizations to be made in
order to avoid code duplication going forward.  Also with that, constify
the "data" parameter as it is passed as a const to the match function.

For this reason, change the prototype of bus_find_device() to match
the prototype of class_find_device() and adjust its callers to use the
const qualifier in accordance with the new prototype of it.

Halil Pasic 37db8985b2 s390/cio: add basic protected virtualization support
As virtio-ccw devices are channel devices, we need to use the
dma area within the common I/O layer for any communication with
the hypervisor.

Note that we do not need to use that area for control blocks
directly referenced by instructions, e.g. the orb.

It handles neither QDIO in the common code, nor any device type specific
stuff (like channel programs constructed by the DASD driver).

An interesting side effect is that virtio structures are now going to
get allocated in 31 bit addressable storage.

Sebastian Ott 135a8b4ce5 s390/cio: fix unbind of io_subchannel_driver
If the io_subchannel_driver is unbound from a subchannel it bluntly kills
all I/O on the subchannel and sets the ccw_device state to not operable
before deregistering the ccw_device. However, for online devices we should
set the device offline (disband path groups etc.) which does not happen if
the device is in not oper state.

Simply deregister the ccw device - ccw_device_remove is smart enough to set
the device offline properly. If everything fails call io_subchannel_quiesce
afterwards as a safeguard.

Greg Kroah-Hartman 13d1d559f0 s390: drivers: Remove redundant license text
Now that the SPDX tag is in all drivers/s390/ files, that identifies the
license in a specific and legally-defined manner.  So the extra GPL text
wording can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text.  And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

Greg Kroah-Hartman 724117b77b s390: cio: add SPDX identifiers to the remaining files
It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/s390/cio/ files with the correct SPDX license
identifier based on the license text in the file itself.  The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

Sebastian Ott 55fb734757 s390/cio: recover from bad paths
In some situations we don't receive notification from firmware that
a previously unusable channelpath is usable again.

Schedule recovery for devices that return from path verification
without using all potentially usable paths. The recovery thread will
periodically trigger a path verification on the affected devices.

Paul Gortmaker a00f761f09 s390: cio: make it explicitly non-modular
The Makefile currently controlling compilation of this code is:

...meaning that the files here are not being built as modular.

Lets remove the couple traces of modular infrastructure use, so that
when reading the code there is no doubt it is builtin-only.

Since module_init translates to device_initcall in the non-modular
case, the init ordering remains unchanged with this commit.

Also note that MODULE_DEVICE_TABLE is a no-op for non-modular code.

We delete the MODULE_LICENSE tag etc. since all that information
was (or is now) contained at the top of the file in the comments.

We replace module.h with export.h where the file does export some
symbols.

Sebastian Ott a6ef15652d s390/cio: fix use after free in cmb processing
Devices with active channel measurement are included in a list. When a
device is removed without deactivating channel measurement first the
list_head is freed but still used. Fix this by making sure that
channel measurement is deactivated during device deregistration.

For devices that we deregister because they are no longer accessible
deactivating channel measurement will fail. In this case we can report
success because the FW will no longer access the measurement block.

In addition to these steps keep an extra device reference while
channel measurement is active.

Sebastian Ott 2c3e7e15c7 s390/cio: set device name as early as possible
Currently we set the device name at the time we call device_add after
we receive the interrupt for the first I/O. When something is not working
as expected during that first I/O (e.g. we don't receive an interrupt) we
print a message including the device name which has not yet been
initialized.

Set the device name after calling device_initialize (prior to starting
the first I/O) so that we have the name present if some unexpected error
occurs during that first I/O.
