5bd2cc5666 hexagon: mm: convert to GENERIC_IOREMAP
By taking GENERIC_IOREMAP method, the generic ioremap_prot() and iounmap()
are visible and available to arch.  This change will simplify
implementation by removing duplicated code with generic ioremap_prot() and
iounmap(), and has the equivalent functioality.

For hexagon, the current ioremap() and iounmap() are the same as generic
version.  After taking GENERIC_IOREMAP way, the old ioremap() and
iounmap() can be completely removed.

2820cfdc08 hexagon: use generic strncpy/strnlen from_user
Remove the hexagon implementation of strncpy/strnlen and instead use
the generic version.  The hexagon version reads the data twice for
strncpy() by doing an extra strnlen(), and it apparently lacks a check
for user_addr_max().

782de70c42 mm: consolidate pgtable_cache_init() and pgd_cache_init()
Both pgtable_cache_init() and pgd_cache_init() are used to initialize kmem
cache for page table allocations on several architectures that do not use
PAGE_SIZE tables for one or more levels of the page table hierarchy.

Most architectures do not implement these functions and use __weak default
NOP implementation of pgd_cache_init().  Since there is no such default
for pgtable_cache_init(), its empty stub is duplicated among most
architectures.

Rename the definitions of pgd_cache_init() to pgtable_cache_init() and
drop empty stubs of pgtable_cache_init().
