Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers. That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

This API is the equivalent of alloc_pages, except that the returned memory
is guaranteed to be DMA addressable by the passed in device. The
implementation will also be used to provide a more sensible replacement
for DMA_ATTR_NON_CONSISTENT flag.
Additionally dma_alloc_noncoherent is switched over to use dma_alloc_pages
as its backend.

For s390 we can have VFs that are passed-through without the associated
PF. Firmware provides an emulation layer to allow these devices to
operate independently, but is missing emulation of the Memory Space
Enable bit. For these as well as linked VFs, set no_command_memory
which specifies these devices do not implement PCI_COMMAND_MEMORY.

clp_rescan_pci_devices_simple() is neither simpler than
clp_scan_pci_devices() nor does it really scan PCI devices, in particular
it will neither add newly discovered devices nor remove those which
disappeared.
Instead it only refreshes PCI function handles and also
has just a single callsite in the same translation unit left which
in fact only refreshes one specific function handle identified by
a FID.
Clarify this by renaming the function and its helper to
clp_refresh_fh() respectvely __clp_refresh_fh() and make it take
a fid directly which saves us dealing with the NULL case which
updated all function handles but is not used anymore.
Furthermore since the only callsite is in the same translation unit
make it static.

all the function does is call zpci_remove_reserved_devices()
followed by a duplicating clp_scan_pci_devices().
So inline the single call as a call to zpci_remove_reserved_devices()
and clp_scan_pci_devices() and remove the function.

currently we have multiple #ifdef CONFIG_PCI_IOV blocks spread over
different compliation units and headers, all dealing with SR-IOV
specific behavior.
This violates the style guide which discourages conditionally compiled
code blocks and hinders maintainability by speading SR-IOV functionality
over many files.
Let's move all of this into a conditionally compiled pci_iov.c file and
local header and prefix SR-IOV specific functions with zpci_iov_*.

With our current support for the new MIO PCI instructions, write
combining/write back MMIO memory can be obtained via the pci_iomap_wc()
and pci_iomap_wc_range() functions.
This is achieved by using the write back address for a specific bar
as provided in clp_store_query_pci_fn()
These functions are however not widely used and instead drivers often
rely on ioremap_wc() and ioremap_prot(), which on other platforms enable
write combining using a PTE flag set through the pgrprot value.
While we do not have a write combining flag in the low order flag bits
of the PTE like x86_64 does, with MIO support, there is a write back bit
in the physical address (bit 1 on z15) and thus also the PTE.
Which bit is used to toggle write back and whether it is available at
all, is however not fixed in the architecture. Instead we get this
information from the CLP Store Logical Processor Characteristics for PCI
command. When the write back bit is not provided we fall back to the
existing behavior.

commit f606b3ef47 ("s390/pci: adapt events for zbus") removed the
zpci_disable_device() call for a zPCI event with PEC 0x0304 because
the device is already deconfigured by the platform.
This however skips the Linux side of the disable in particular it leads
to leaking the DMA tables and bitmaps because zpci_dma_exit_device() is
never called on the device.
If the device transitions to the Reserved state we call zpci_zdev_put()
but zpci_release_device() will not call zpci_disable_device() because
the state of the zPCI function is already ZPCI_FN_STATE_STANDBY.
If the device is put into the Standby state, zpci_disable_device() is
not called and the device is assumed to have been put in Standby through
platform action.
At this point the device may be removed by a subsequent event with PEC
0x0308 or 0x0306 which calls zpci_zdev_put() with the same problem
as above or the device may be configured again in which case
zpci_disable_device() is also not called.
Fix this by calling zpci_disable_device() explicitly for PEC 0x0304 as
before. To make it more clear that zpci_disable_device() may be called,
even if the lower level device has already been disabled by the
platform, add a comment to zpci_disable_device().

We found that callers of dma_get_seg_boundary mostly do an ALIGN
with page mask and then do a page shift to get number of pages:
ALIGN(boundary + 1, 1 << shift) >> shift
However, the boundary might be as large as ULONG_MAX, which means
that a device has no specific boundary limit. So either "+ 1" or
passing it to ALIGN() would potentially overflow.
According to kernel defines:

We can simplify the logic here into a helper function doing:

Currently there are four places in which a PCI function is scanned
and made available to drivers:
1. In pci_scan_root_bus() as part of the initial zbus
creation.
2. In zpci_bus_add_devices() when registering
a device in configured state on a zbus that has already been
scanned.
3. When a function is already known to zPCI (in reserved/standby state)
and configuration is triggered through firmware by PEC 0x301.
4. When a device is already known to zPCI (in standby/reserved state)
and configuration is triggered from within Linux using
enable_slot().
The PF/VF linking step and setting of pdev->is_virtfn introduced with
commit e5794cf1a2 ("s390/pci: create links between PFs and VFs") was
only triggered for the second case, which is where VFs created through
sriov_numvfs usually land. However unlike some other platforms but like
POWER VFs can be individually enabled/disabled through
/sys/bus/pci/slots.
Fix this by doing VF setup as part of pcibios_bus_add_device() which is
called in all of the above cases.
Finally to remove the PF/VF links call the common code
pci_iov_remove_virtfn() function to remove linked VFs.
This takes care of the necessary sysfs cleanup.

For fixing the PF to VF link removal we need to perform some action on
every removal of a zdev from the common PCI subsystem.
So in preparation re-introduce zpci_remove_device() and use that instead
of directly calling the common code functions. This was actually still
declared from earlier code but no longer implemented.

We were missing the pci_dev_put() for candidate PFs. Furhtermore in
discussion with upstream it turns out that somewhat counterintuitively
some common code, in particular the vfio-pci driver, assumes that
pdev->is_virtfn always implies that pdev->physfn is set, i.e. that VFs
are always linked.
While POWER does seem to set pdev->is_virtfn even for unlinked functions
(see comments in arch/powerpc/kernel/eeh.c:eeh_debugfs_break_device())
for now just be safe and only set pdev->is_virtfn on linking.
Also make sure that we only search for parent PFs if the zbus is
multifunction and we thus know the devfn values supplied by firmware
come from the RID.

A configuration request event may be stale, that is the event
may reference a zdev which was already configured.
This can happen when a hotplug happens during boot such that
the device is discovered and configured in the initial clp_list_pci(),
then after initialization we enable events and process
the original configuration request which additionally still contains
the old disabled function handle leading to a failure during device
enablement and subsequent I/O lockout.
Fix this by restoring the check that the device to be configured is in
standby which was removed in commit f606b3ef47 ("s390/pci: adapt events
for zbus").
This check does not need serialization as we only enable the events after
zPCI has fully initialized, which includes the initial clp_list_pci(),
rescan only does updates and events are serialized with respect to each
other.

In usual IPL or hot plug scenarios a zPCI function transitions directly
from reserved (invisible to Linux) to configured state or is configured
by Linux itself using an SCLP, however it can also first go from
reserved to standby and then from standby to configured without
Linux initiative.
In this scenario we first get a PEC event 0x302 and then 0x301. This may
happen for example when the device is deconfigured at another LPAR and
made available for this LPAR. It may also happen under z/VM when
a device is attached while in some inconsistent state.
However when we get the 0x301 the device is already known to zPCI
so calling zpci_create() will add it twice resulting in the below
BUG. Instead we should only enable the existing device and finally
scan it through the PCI subsystem.

Let's use the same signature and parameter names as in the generic
ioremap() definition making the physical address' type explicit.
Add a check against address wrap around as in the generic
lib/ioremap.c:ioremap_prot() code.
Finally use free_vm_area() instead of vunmap() as in the generic code.
Besides being clearer free_vm_area() can also skip a few additional
checks compared with vunmap().

On s390 PCI Virtual Functions (VFs) are scanned by firmware and are made
available to Linux via the hot-plug interface. As such the common code
path of doing the scan directly using the parent Physical Function (PF)
is not used and fenced off with the no_vf_scan attribute.
Even if the partition created the VFs itself e.g. using the sriov_numvfs
attribute of a PF, the PF/VF links thus need to be established after the
fact. To do this when a VF is plugged we scan through all functions on
the same zbus and test whether they are the parent PF in which case we
establish the necessary links.
With these links established there is now no more need to fence off
pci_iov_remove_virtfn() for pdev->no_vf_scan as the common code now
works fine.

The s390_mmio_read/write syscalls are currently broken when running with
MIO.
The new pcistb_mio/pcstg_mio/pcilg_mio instructions are executed
similiarly to normal load/store instructions and do address translation
in the current address space. That means inside the kernel they are
aware of mappings into kernel address space while outside the kernel
they use user space mappings (usually created through mmap'ing a PCI
device file).
Now when existing user space applications use the s390_pci_mmio_write
and s390_pci_mmio_read syscalls, they pass I/O addresses that are mapped
into user space so as to be usable with the new instructions without
needing a syscall. Accessing these addresses with the old instructions
as done currently leads to a kernel panic.
Also, for such a user space mapping there may not exist an equivalent
kernel space mapping which means we can't just use the new instructions
in kernel space.
Instead of replicating user mappings in the kernel which then might
collide with other mappings, we can conceptually execute the new
instructions as if executed by the user space application using the
secondary address space. This even allows us to directly store to the
user pointer without the need for copy_to/from_user().

We allow multiple functions on a single bus.
We suppress the ZPCI_DEVFN definition and replace its
occurences with zpci->devfn.
We verify the number of device during the registration.
There can never be more domains in use than existing
devices, so we do not need to verify the count of domain
after having verified the count of devices.

The zPCI bus is in charge to handle common zPCI resources for
zPCI devices.
Creating the zPCI bus, the PCI bus, the zPCI devices and the
PCI devices and hotplug slots
done in a specific order:
- PCI hotplug slot creation needs a PCI bus
- PCI bus needs a PCI domain
which is reported by the pci_domain_nr() when setting up the
host bridge
- PCI domain is set from the zPCI with devfn 0
this is necessary to have a reproducible enumeration
Therefore we can not create devices or hotplug slots for any PCI
device associated with a zPCI device before having discovered
the function zero of the bus.
The discovery and initialization of devices can be done at several
points in the code:
- On Events, serialized in a thread context
- On initialization, in the kernel init thread context
- When powering on the hotplug slot, in a user thread context
The removal of devices and their parent bus may also be done on
events or for devices when powering down the slot.
To guarantee the existence of the bus and devices until they are
no more needed we use kref in zPCI bus and introduce a reference
count in the zPCI devices.
In this patch the zPCI bus still only accept a device with
a devfn 0.

Firmware provides the bus/devfn part of the PCI addresses of a zPCI
function inside the new field RID of the CLP query PCI function
with a bit to know if this field is available to use.
Let's add these fields to the clp_rsp_query_pci structure,
add corresponding fields to zdev and initialize them.

Using PCI multifunctions in S390 is a new feature we may want
to ignore to continue provide the same topology as in the past
to userland even if the configuration supports exposing the
topology of a multi-Function device.
A new boolean parameters allows to overwrite the kernel
pci configuration:
- pci=norid when on, disallow the use a new firmware field,
RID, which provides the PCI <bus>:<device>.<function> part
of the PCI address.
To be used in the following patches and satisfy the checkpatch.pl
the variable is exposed in pci.h

with the introduction of CPU directed interrupts the kernel
parameter pci=force_floating was introduced to fall back
to the previous behavior using floating irqs.
However we were still setting the affinity in that case,
both in __irq_alloc_descs() and via the irq_set_affinity
callback in struct irq_chip.
For the former only set the affinity in the directed case.
The latter is explicitly set in zpci_directed_irq_init()
so we can just leave it unset for the floating case.

When UID checking is enabled a UID value of 0 is invalid and can not be
set by the user. On z/VM it is however used to indicate an unset UID.
Until now, this lead to the behavior that one PCI function could be
attached with UID 0 after which z/VM would prohibit further attachment.
Now if the user then turns off UID checking in z/VM the user could
seemingly attach additional PCI functions that would however not show up
in Linux as that would not be informed of the change in UID checking
mode. This is unexpected and confusing and lead to bug reports against
Linux.
Instead now, if we encounter an unset UID value of 0 treat it as
indicating that UID checking was turned off, switch to automatic domain
allocation, and warn the user of the possible misconfiguration.

Until now zpci_alloc_domain() only prevented more than
CONFIG_PCI_NR_FUNCTIONS from being added when using automatic domain
allocation. When explicit UIDs were defined UIDs above
CONFIG_PCI_NR_FUNCTIONS were not counted at all.
When more PCI functions are added this could lead to various errors
including under sized IRQ vectors and similar issues.
Fix this by explicitly tracking the number of allocated domains.

Hibernation is known to be broken for many years on s390. Given that
there aren't any real use cases, remove the code instead of spending
time to fix and maintain it.
Without hibernate support it doesn't make too much sense to keep power
management support; therefore remove it completely.

In the initial MIO support introduced in
commit 71ba41c9b1 ("s390/pci: provide support for MIO instructions")
zpci_map_resource() and zpci_setup_resources() default to using the
mio_wb address as the resource's start address. This means users of the
mapping, which includes most drivers, will get write combining on PCI
Stores. This may lead to problems when drivers expect write through
behavior when not using an explicit ioremap_wc().

When we try to recover a PCI function using
echo 1 > /sys/bus/pci/devices/<id>/recover
or manually with
echo 1 > /sys/bus/pci/devices/<id>/remove
echo 0 > /sys/bus/pci/slots/<slot>/power
echo 1 > /sys/bus/pci/slots/<slot>/power
clp_disable_fn() / clp_enable_fn() call clp_set_pci_fn() to first
disable and then reenable the function.
When the function is already in the requested state we may be left with
an invalid function handle.
To get a new valid handle we do a clp_list_pci() call. For this we need
both the function ID and function handle in clp_set_pci_fn() so pass the
zdev and get both.
To simplify things also pull setting the refreshed function handle into
clp_set_pci_fn()

The config option CONFIG_PCI_NR_FUNCTIONS sets a limit on the number of
PCI functions we can support. Previously on reaching this limit there
was no indication why newly attached devices are not recognized by Linux
which could be quite confusing. Thus this patch adds a pr_err() for this
case.

When UID checking was turned off during runtime in the underlying
hypervisor, a PCI device may be attached with the same UID. This is
already detected but happens silently. Add an error message so it can
more easily be understood why a device was not added.

While the default ->mmap and ->get_sgtable implementations work for the
majority of our dma_map_ops impementations they are inherently safe
for others that don't use the page allocator or CMA and/or use their
own way of remapping not covered by the common code. So remove the
defaults if these methods are not wired up, but instead wire up the
default implementations for all safe instances.

Command line option values passed to __setup callbacks are always
null-terminated and "s390_iommu=" may only accept "strict" as value.
So replace strncmp with strcmp.
While at it also make s390_iommu_setup return 1, which means this
command line option is handled by this callback.

Move enablement of mio addressing control from detect_machine_facilities
to pci_base_init. detect_machine_facilities runs so early that the
static branches have not been toggled yet, thus mio addressing control
was always off. In pci_base_init we have to use the SMP aware
ctl_set_bit though.

There's a lot of changes to the locking implementation that makes
it similar to qrwlock, including owner handoff for more fair
locking.

The changes are extensive and the patch-set has been through
several iterations addressing various locking workloads. There
might be more regressions, but unless they are pathological I
believe we want to use this new implementation as the baseline
going forward.
- jump-label optimizations by Daniel Bristot de Oliveira: the primary
motivation was to remove IPI disturbance of isolated RT-workload
CPUs, which resulted in the implementation of batched jump-label
updates. Beyond the improvement of the real-time characteristics
kernel, in one test this patchset improved static key update
overhead from 57 msecs to just 1.4 msecs - which is a nice speedup
as well.
- atomic64_t cross-arch type cleanups by Mark Rutland: over the last
~10 years of atomic64_t existence the various types used by the
APIs only had to be self-consistent within each architecture -
which means they became wildly inconsistent across architectures.
Mark puts and end to this by reworking all the atomic64
implementations to use 's64' as the base type for atomic64_t, and
to ensure that this type is consistently used for parameters and
return values in the API, avoiding further problems in this area.

Recent firmware will store PCI MIO information also when enabling MIO
instructions via set PCI function. We do not use this information but
currently calling enable MIO will fail because of insufficient response
block length. Fix this by putting a struct mio_info at the end of the
affected response block struct.

Up until now all interrupts on s390 have been floating. For MSI interrupts
we've used a global summary bit vector (with a bit for each function) and
a per-function interrupt bit vector (with a bit per MSI).
This patch introduces a new IRQ delivery mode: CPU directed interrupts.
In this new mode a per-CPU interrupt bit vector is used (with a bit per
MSI per function). Further it is now possible to direct an IRQ to a
specific CPU so we can finally support IRQ affinity.
If an interrupt can't be delivered because the appointed CPU is occupied
by a hypervisor the interrupt is delivered floating. For this a global
summary bit vector is used (with a bit per CPU).

With the handle_simple_irq irq_flow_handler it must be ensured to
not call generic_handle_irq with the same IRQ number on 2 CPUs at
the same time (interrupts are floating on s390).
Contrary to my initial investigation the irq_desc's lock usage in
handle_simple_irq does not ensure this. Thus re-introduce the bit-
lock usage in s390's pci handler.

If we want to map memory from the DMA allocator to userspace it must be
zeroed at allocation time to prevent stale data leaks. We already do
this on most common architectures, but some architectures don't do this
yet, fix them up, either by passing GFP_ZERO when we use the normal page
allocator or doing a manual memset otherwise.

The interrupt handler uses bit_spin_lock around a call to retrieve
per irq data (the irq number). However this per irq data is only
set during irq setup time and never changed until the irq is freed.
Thus it's safe to remove the lock usage.

S390 already returns (~(dma_addr_t)0x0) on mapping failures, so we can
switch over to returning DMA_MAPPING_ERROR and let the core dma-mapping
code handle the rest.

During interrupt setup we allocate interrupt vectors, walk the list of msi
descriptors, and fill in the message data. Requesting more interrupts than
supported on s390 can lead to an out of bounds access.
When we restrict the number of interrupts we should also stop walking the
msi list after all supported interrupts are handled.

Most mainstream architectures are using 65536 entries, so lets stick to
that. If someone is really desperate to override it that can still be
done through <asm/dma-mapping.h>, but I'd rather see a really good
rationale for that.
dma_debug_init is now called as a core_initcall, which for many
architectures means much earlier, and provides dma-debug functionality
earlier in the boot process. This should be safe as it only relies
on the memory allocator already being available.

This was used by the ide, scsi and networking code in the past to
determine if they should bounce payloads. Now that the dma mapping
always have to support dma to all physical memory (thanks to swiotlb
for non-iommu systems) there is no need to this crude hack any more.

In a virtualized setup lazy flushing can lead to the hypervisor
running out of resources when lots of guest pages need to be
pinned. In this situation simply trigger a global flush to give
the hypervisor a chance to free some of these resources.

As of today QEMU does not provide the AIS facility to its guest. This
prevents Linux guests from using PCI devices as the ais facility is
checked during init. As this is just a performance optimization, we can
move the ais check into the code where we need it (calling the SIC
instruction). This is used at initialization and on interrupt. Both
places do not require any serialization, so we can simply skip the
instruction.
Since we will now get all interrupts, we can also avoid the 2nd scan.
As we can have multiple interrupts in parallel we might trigger spurious
irqs more often for the non-AIS case but the core code can handle that.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Multiple architectures define this as an empty function, and I'm adding
another one as part of the RISC-V port. Add a __weak version of
pcibios_fixup_bus() and delete the now-obselete ones in a handful of
ports.
The only functional change should be that microblaze used to export
pcibios_fixup_bus(). None of the other architectures exports this, so I
just dropped it.

When uid checking is enabled firmware guarantees uniqueness of the uids
and we use them for device enumeration. Tests have shown that uid checking
can be toggled at runtime. This is unfortunate since it can lead to name
clashes.
Recognize these name clashes by allocating bits in zpci_domain even for
firmware provided ids.

PCI hotplug events basically notify about the new state of a
function. Unfortunately some hypervisors implement hotplug
events in a way where it is not clear what the new state of
the function should be.
Use clp_get_state to find the current state of the function
and handle accordingly.

Cleanup in zpci_fmb_enable_device when fmb registration fails. Also
don't free the fmb when deregistration fails in zpci_fmb_disable_device
but handle error situations when a function was hot-unplugged.
Also remove the mod_pci helper since it is no longer used.

DMA tables are freed in zpci_dma_exit_device regardless of the return
code of zpci_unregister_ioat. This could lead to a use after free. On
the other hand during function hot-unplug, zpci_unregister_ioat will
always fail since the function is already gone.
So let zpci_unregister_ioat report success when the function is gone
but don't cleanup the dma table when a function could still have it
in access.

When we ask a function to stop creating interrupts this may fail
due to the function being already gone (e.g. after hot-unplug).
Consequently we don't free associated resources like summary bits
and bit vectors used for irq processing. This could lead to
situations where we ran out of these resources and fail to setup
new interrupts.
The fix is to just ignore the errors in cases where we can be
sure no new interrupts are generated.

similar enough to the core DMA mapping code that with a few changes
it was possible to remove the IB DMA mapping code entirely and
switch the RDMA stack to use the core DMA mapping code. This resulted

We have 2 strategies to reduce the number of RPCIT instructions:
* A HW feature indicated via the tlb_refresh bit allows us to omit RPCIT for
invalid -> valid translation-table entry updates.
* With "lazy flush" we omit RPCIT for valid -> invalid updates until we run
out of dma addresses. When we have to reuse dma addresses we issue a global
tlb flush using only one RPCIT instruction.
Currently lazy flushing depends on tlb_refresh. Since there is no technical
reason for this remove this dependency.

__s390_dma_map_sg maps a dma-contiguous area. Although we only map
whole pages we have to take into account that the area doesn't start
or stop at a page boundary because we use the dma address to loop
over the individual sg entries. Failing to do that might lead to an
access of the wrong sg entry.

Lazy unmap (defer tlb flush after unmap until dma address reuse) can
greatly reduce the number of RPCIT instructions in the best case. In
reality we are often far away from the best case scenario because our
implementation suffers from the following problem:
To create dma addresses we maintain an iommu bitmap and a pointer into
that bitmap to mark the start of the next search. That pointer moves from
the start to the end of that bitmap and we issue a global tlb flush
once that pointer wraps around. To prevent address reuse before we issue
the tlb flush we even have to move the next pointer during unmaps - when
clearing a bit > next. This could lead to a situation where we only use
the rear part of that bitmap and issue more tlb flushes than expected.
To fix this we no longer clear bits during unmap but maintain a 2nd
bitmap which we use to mark addresses that can't be reused until we issue
the global tlb flush after wrap around.

Split dma_update_trans into __dma_update_trans which handles updating
the dma translation tables and __dma_purge_tlb which takes care of
purging associated entries in the dma translation lookaside buffer.
The map_sg API makes use of this split approach by calling
__dma_update_trans once per physically contiguous address range but
__dma_purge_tlb only once per dma contiguous address range.
This results in less invocations of the expensive RPCIT instruction
when using map_sg.

Our map_sg implementation mapped sg entries independently of each other.
For ease of use and possible performance improvements this patch changes
the implementation to try to map as many (likely physically non-contiguous)
sglist entries as possible into a contiguous DMA segment.

We calculate dma addresses using an iommu bitmap. Since commit
69eea95c ("s390/pci_dma: fix DMA table corruption with > 4 TB main memory")
we've made sure that addresses created using that bitmap are below
the maximum reported by firmware. Thus the additional check for
that address to be within range can be removed.

The 'report_error' interface for PCI devices found on s390 can be
used by a user space program to inject an adapter error notification.
Add a new kernel interface zpci_report_error to allow a PCI device
driver to inject these error notifications without a detour over
user space.

The dma-mapping core and the implementations do not change the DMA
attributes passed by pointer. Thus the pointer can point to const data.
However the attributes do not have to be a bitfield. Instead unsigned
long will do fine:
1. This is just simpler. Both in terms of reading the code and setting
attributes. Instead of initializing local attributes on the stack
and passing pointer to it to dma_set_attr(), just set the bits.
2. It brings safeness and checking for const correctness because the
attributes are passed by value.

When we use the iommu_area_alloc helper to get dma addresses
we specify the boundary_size parameter but not the offset (called
shift in this context).
As long as the offset (start_dma) is a multiple of the boundary
we're ok (on current machines start_dma always seems to be 4GB).
Don't leave this to chance and specify the offset for iommu_area_alloc.

After a failure during registration of the dma_table (because of the
function being in error state) we free its memory but don't reset the
associated pointer to zero.
When we then receive a notification from firmware (about the function
being in error state) we'll try to walk and free the dma_table again.
Fix this by resetting the dma_table pointer. In addition to that make
sure that we free the iommu_bitmap when appropriate.
