c5ab571f81 usb: phy: msm: reset controller is mandatory now
Commit a273454341 "usb: phy: msm: Use reset framework for LINK
and PHY resets" introduced a mandatory call to reset_control_get
into the msm usb phy driver, which means we have to add a Kconfig
dependency on the API to avoid this build error:

d183c81929 fsl-usb: do not test for PHY_CLK_VALID bit on controller version 1.6
Per reference manuals of Freescale P1020 and P2020 SoCs, USB controller
present in these SoCs has bit 17 of USBx_CONTROL register marked as
Reserved - there is no PHY_CLK_VALID bit there.

Testing for this bit in ehci_fsl_setup_phy() behaves differently on two
P1020RDB boards available here - on one board test passes and fsl-usb
init succeeds, but on other board test fails, causing fsl-usb init to
fail.

This patch changes ehci_fsl_setup_phy() not to test PHY_CLK_VALID on
controller version 1.6 that (per manual) does not have this bit.

c1db30a2a7 USB: OHCI: fix problem with global suspend on ATI controllers
Some OHCI controllers from ATI/AMD seem to have difficulty with
"global" USB suspend, that is, suspending an entire USB bus without
setting the suspend feature for each port connected to a device.  When
we try to resume the child devices, the controller gives timeout
errors on the unsuspended ports, requiring resets, and can even cause
ohci-hcd to hang; see

This patch fixes the problem by adding a new quirk flag to ohci-hcd.
The flag causes the ohci_rh_suspend() routine to suspend each
unsuspended, enabled port before suspending the root hub.  This
effectively converts the "global" suspend to an ordinary root-hub
suspend.  There is no need to unsuspend these ports when the root hub
is resumed, because the child devices will be resumed anyway in the
course of a normal system resume ("global" suspend is never used for
runtime PM).

This patch should be applied to all stable kernels which include
commit 0aa2832dd0 (USB: use "global suspend" for system sleep on
USB-2 buses) or a backported version thereof.

01bb59ebff usb/xhci: fix compilation warning when !CONFIG_PCI && !CONFIG_PM
When CONFIG_PCI and CONFIG_PM are not selected, xhci.c gets this
warning:
drivers/usb/host/xhci.c:409:13: warning: ‘xhci_msix_sync_irqs’ defined
but not used [-Wunused-function]

Instead of creating nested #ifdefs, this patch fixes it by defining the
xHCI PCI stubs as inline.

This warning has been in since 3.2 kernel and was
caused by commit 421aa841a1
"usb/xhci: hide MSI code behind PCI bars", but wasn't noticed
until 3.13 when a configuration with these options was tried

c09ec25d36 xhci: Switch Intel Lynx Point ports to EHCI on shutdown.
The same issue like with Panther Point chipsets. If the USB ports are
switched to xHCI on shutdown, the xHCI host will send a spurious interrupt,
which will wake the system. Some BIOS have work around for this, but not all.
One example is Compulab's mini-desktop, the Intense-PC2.

The bug can be avoided if the USB ports are switched back to EHCI on
shutdown.

This patch should be backported to stable kernels as old as 3.12,
that contain the commit 638298dc66
"xhci: Fix spurious wakeups after S5 on Haswell"

1f81b6d22a usb: xhci: Prefer endpoint context dequeue pointer over stopped_trb
We have observed a rare cycle state desync bug after Set TR Dequeue
Pointer commands on Intel LynxPoint xHCs (resulting in an endpoint that
doesn't fetch new TRBs and thus an unresponsive USB device). It always
triggers when a previous Set TR Dequeue Pointer command has set the
pointer to the final Link TRB of a segment, and then another URB gets
enqueued and cancelled again before it can be completed. Further
investigation showed that the xHC had returned the Link TRB in the TRB
Pointer field of the Transfer Event (CC == Stopped -- Length Invalid),
but when xhci_find_new_dequeue_state() later accesses the Endpoint
Context's TR Dequeue Pointer field it is set to the first TRB of the
next segment.

The driver expects those two values to be the same in this situation,
and uses the cycle state of the latter together with the address of the
former. This should be fine according to the XHCI specification, since
the endpoint ring should be stopped when returning the Transfer Event
and thus should not advance over the Link TRB before it gets restarted.
However, real-world XHCI implementations apparently don't really care
that much about these details, so the driver should follow a more
defensive approach to try to work around HC spec violations.

This patch removes the stopped_trb variable that had been used to store
the TRB Pointer from the last Transfer Event of a stopped TRB. Instead,
xhci_find_new_dequeue_state() now relies only on the Endpoint Context,
requiring a small amount of additional processing to find the virtual
address corresponding to the TR Dequeue Pointer. Some other parts of the
function were slightly rearranged to better fit into this model.

This patch should be backported to kernels as old as 2.6.31 that contain
the commit ae63674714 "USB: xhci: URB
cancellation support."

4f2fe2d274 USB: EHCI: tegra: set txfill_tuning
To avoid memory fetch underflows with larger USB transfers, Tegra SoCs
need txfill_tuning's txfifothresh register field set to a non-default
value. Add a custom reset override in order to set this up.

These values are recommended practice for all Tegra chips. However,
I've only noticed practical problems when not setting them this way on
systems using Tegra124. Hence, CC: stable only for recent kernels which
actually support Tegra124.

86e2864d7e usb: wusbcore: disable transfer notifications for Alereon HWAs
The HWA driver does not do anything with transfer notifications after
receiving the first one and the Alereon HWA allows them to be disabled
as a performance optimization.  This patch sends a vendor specific
command to the Alereon HWA on startup to disable transfer notifications.
If the command is successful, the DTI system is started immediately
since that would normally be started upon the first reception of a
transfer notification which will no longer be sent.

This reverts commit 247bf55727.

This commit, together with commit 3804fad454
"USBNET: ax88179_178a: enable tso if usb host supports sg dma" were
origially added to get xHCI 1.0 hosts and usb ethernet ax88179_178a devices
working together with scatter gather. xHCI 1.0 hosts pose some requirement on how transfer
buffers are aligned, setting this requirement for 1.0 hosts caused USB 3.0 mass
storage devices to fail more frequently.

USB 3.0 mass storage devices used to work before 3.14-rc1.  Theoretically,
the TD fragment rules could have caused an occasional disk glitch.
Now the devices *will* fail, instead of theoretically failing.
>From a user perspective, this looks like a regression; the USB device obviously
fails on 3.14-rc1, and may sometimes silently fail on prior kernels.

The proper soluition is to implement the TD fragment rules required, but for now
this patch needs to be reverted to get USB 3.0 mass storage devices working at the
level they used to.

When some xHCI host controllers fall back to use the legacy IRQ,

leads to the empty string of the xHCI host controller in

bcffae7708 xhci: Prevent runtime pm from autosuspending during initialization
xHCI driver has its own pci probe function that will call usb_hcd_pci_probe
to register its usb-2 bus, and then continue to manually register the
usb-3 bus. usb_hcd_pci_probe does a pm_runtime_put_noidle at the end and
might thus trigger a runtime suspend before the usb-3 bus is ready.

Prevent the runtime suspend by increasing the usage count in the
beginning of xhci_pci_probe, and decrease it once the usb-3 bus is
ready.

xhci-platform driver is not using usb_hcd_pci_probe to set up
busses and should not need to have it's usage count increased during probe.

14aec58932 storage: accept some UAS devices if streams are unavailable
On some older XHCIs streams are not supported and the UAS driver
will fail at probe time. For those devices storage should try
to bind to UAS devices.
This patch adds a flag for stream support to HCDs and evaluates
it.

[Note: Sarah fixed a bug where the USB 2.0 root hub, not USB 3.0 root
hub would get marked as being able to support streams.]

21d0e51bfb xhci: Kill streams URBs when the host dies.
If the host controller stops responding to commands, we need to kill all
the URBs that were queued to all endpoints.  The current code would only
kill URBs that had been queued to the endpoint rings.  ep->ring is set
to NULL if streams has been enabled for the endpoint, which means URBs
submitted with a non-zero stream_id would never get killed.  Fix this.

In preparation for fixing this function for streams endpoints, refactor
code in the command watchdog timeout function into two new functions.
One kills all URBs on a ring (either stream or endpoint), the other

84c1e40fd7 xhci: The trb_address_map radix tree expects 1KB segment memory aligment
If we align segment dma pool memory to 64 bytes, then a segment can be located
at 0x10000040 - 0x1000043f, and a segment from another ring at 0x10000440 -
0x1000083f. The last trb in the first segment at 0x10000430 will then translate
to the same radix tree key as the first trb of the second segment, while they
are in different rings!

This patches fixes this by changing the alignment of the dma pool to be 1KB
rather then 64 bytes. An alternative fix would be to reduce the shift used
to calculate the radix tree keys, but that would (slighlty) grow the radix
trees so I believe this is the better fix.

Note this patch is mostly theoretical since in practice I've not seen
the dma_pool actually return not 1KB aligned memory.

127329d76b xhci: xhci_mem_cleanup: make sure cmd_ring_reserved_trbs really is 0
cmd_ring_reserved_trbs gets decremented by xhci_free_stream_info(), so set it
to 0 after freeing all rings, otherwise it wraps around to a very large value
when rings with streams are free-ed.

Before this patch the wrap-around could be triggered when xhci_resume
calls xhci_mem_cleanup if the controller resume fails.

d57342232d xhci: Remove segments from radix tree on failed insert.
If we're expanding a stream ring, we want to make sure we can add those
ring segments to the radix tree that maps segments to ring pointers.
Try the radix tree insert after the new ring segments have been allocated
(the last segment in the new ring chunk will point to the first newly
allocated segment), but before the new ring segments are linked into the
old ring.

If insert fails on any one segment, remove each segment from the radix
tree, deallocate the new segments, and return.  Otherwise, link the new
segments into the tree.

HdG: Add a check to only update stream mappings in xhci_ring_expansion when
the ring is a stream ring.

9aad95e292 xhci: For streams the dequeue ptr must be read from the stream ctx
This fixes TR dequeue validation failing on Intel XHCI controllers with the
following warning:

Mismatch between completed Set TR Deq Ptr command & xHCI internal state.

Interestingly enough reading the deq ptr from the ep ctx after a
TR Deq Ptr command does work on a Nec XHCI controller, it seems the Nec
writes the ptr to both the ep and stream contexts when streams are used.

controllers reject Set TR dequeue commands with a COMP_TRB_ERR, leading

ee4aa54bce xhci: Check size rather then number of streams when allocating stream ctxs
Before this a device needing ie 32 stream ctxs would end up with an entry from
the small_streams_pool which has 256 bytes entries, where as 32 stream ctxs
need 512 bytes. Things actually keep running for a surprisingly long time
before crashing because of this.

And warn about this, as that would be a driver bug.

Like wise drivers should ensure that streams are properly free-ed before a
device is reset. So lets warn about that too. This already causes warnings
in the form of:

153413032c xhci: fix usb3 streams
xhci maintains a radix tree for each stream endpoint because it must
be able to map a trb address to the stream ring.  Each ring segment
must be added to the ring for this to work.  Currently xhci sticks
only the first segment of each stream ring into the radix tree.

Result is that things work initially, but as soon as the first segment
is full xhci can't map the trb address from the completion event to the
stream ring any more -> BOOM.  You'll find this message in the logs:

  ERROR Transfer event for disabled endpoint or incorrect stream ring

This patch adds a helper function to update the radix tree, and a
function to remove ring segments from the tree.  Both functions loop
over the segment list and handles all segments instead of just the
first.

[Note: Sarah changed this patch to add radix_tree_maybe_preload() and
radix_tree_preload_end() calls around the radix tree insert, since we
can now insert entries in interrupt context.  There are now two helper
functions to make the code cleaner, and those functions are moved to
make them static.]

25cd2882e2 usb/xhci: Change how we indicate a host supports Link PM.
The xHCI driver currently uses a USB core internal field,
udev->lpm_capable, to indicate the xHCI driver knows how to calculate
the LPM timeout values.  If this value is set for the host controller
udev, it means Link PM can be enabled for child devices under that host.

Change the code so the xHCI driver isn't mucking with USB core internal
fields.  Instead, indicate the xHCI driver doesn't support Link PM on
this host by clearing the U1 and U2 exit latencies in the roothub
SuperSpeed Extended Capabilities BOS descriptor.

The code to check for the roothub setting U1 and U2 exit latencies to
zero will also disable LPM for external devices that do that same.  This
was already effectively done with commit
ae8963adb4 "usb: Don't enable LPM if the
exit latency is zero."  Leave that code in place, so that if a device
sets one exit latency value to zero, but the other is set to a valid
value, LPM is only enabled for the U1 or U2 state that had the valid
value.  This is the same behavior the code had before.

Also, change messages about missing Link PM information from warning
level to info level.  Only print a warning about the first device that
doesn't support LPM, to avoid log spam.  Further, cleanup some
unnecessary line breaks to help people to grep for the error messages.

3e8d6d85ad USB: EHCI: add delay during suspend to prevent erroneous wakeups
High-speed USB connections revert back to full-speed signalling when
the device goes into suspend.  This takes several milliseconds, and
during that time it's not possible to tell reliably whether the device
has been disconnected.

On some platforms, the Wake-On-Disconnect circuitry gets confused
during this intermediate state.  It generates a false wakeup signal,
which can prevent the controller from going to sleep.

To avoid this problem, this patch adds a 5-ms delay to the
ehci_bus_suspend() routine if any ports have to switch over to
full-speed signalling.  (Actually, the delay was already present for
devices using a particular kind of PHY power management; the patch
merely causes the delay to be used more widely.)

supposed to be OS agnostic. After much discussion I gave up, added a:
"depends on !PPC_OF" to Kconfig to avoid a known conflict with PPC-OF platforms
and went with the generic usb-ehci as requested.

In retro-spect I should have chosen something different, the dts files for many
existing boards already claim to be compatible with "usb-ehci", ie they have:

	compatible = "ti,ehci-omap", "usb-ehci";

In theory this should not be a problem since the "ti,ehci-omap" entry takes
presedence, but in practice using a conflicting compatible string is an issue,
because it makes which driver gets used depend on driver registration order.

This patch changes the compatible string claimed by ehci-platform to
"generic-ehci", avoiding the driver registration / module loading ordering
problems, and removes the "depends on !PPC_OF" workaround.

xHCI 1.0 hosts have a set of requirements on how to align transfer

ax88179_178a driver added support for scatter gather in 3.12, with
commit 804fad45411b48233b48003e33a78f290d227c8 "USBNET: ax88179_178a:
enable tso if usb host supports sg dma", it broke the device under xHCI
1.0 hosts.  Under certain network loads, the device would see an
unexpected short packet from the host, which would cause the device to
stop sending ethernet packets, even through USB packets would still be
sent.

Commit 35773dac5f "usb: xhci: Link TRB must not occur within a USB
payload burst" attempted to fix this.  It was a quick hack to partially
implement the TD fragment rules.  However, it caused regressions in the
usb-storage layer and userspace USB drivers using libusb.  The patches
to attempt to fix this are too far reaching into the USB core, and we
really need to implement the TD fragment rules correctly in the xHCI
driver, instead of continuing to wallpaper over the issues.

Disable arbitrarily-aligned scatter-gather in the xHCI driver for 1.0
hosts.  Only the ax88179_178a driver checks the no_sg_constraint flag,
so don't set it for 1.0 hosts.  This should not impact usb-storage or
usbfs behavior, since they pass down max packet sized aligned sg-list
entries (512 for USB 2.0 and 1024 for USB 3.0).

a4aeb21175 ehci-platform: Add support for clks and phy passed through devicetree
Currently ehci-platform is only used in combination with devicetree when used
with some Via socs. By extending it to (optionally) get clks and a phy from
devicetree, and enabling / disabling those on power_on / off, it can be used
more generically. Specifically after this commit it can be used for the
ehci controller on Allwinner sunxi SoCs.

Since ehci-platform is intended to handle any generic enough non pci ehci
device, add a "usb-ehci" compatibility string.

There already is a usb-ehci device-tree bindings document, update this
with clks and phy bindings info.

Although actually quite generic so far the via,vt8500 compatibilty string
had its own bindings document. Somehow we even ended up with 2 of them. Since
these provide no extra information over the generic usb-ehci documentation,
this patch removes them.

The ehci-ppc-of.c driver also claims the usb-ehci compatibility string,
even though it mostly is ibm,usb-ehci-440epx specific. ehci-platform.c is
not needed on ppc platforms, so add a !PPC_OF dependency to it to avoid
2 drivers claiming the same compatibility string getting build on ppc.

Many xHCI host controllers can only handle 32-bit addresses, and writing
64-bits at a time causes them to fail.  Rafał reports that USB devices

Some co-workers of mine bought Samsung laptops that had mostly usb3 ports.
Those ports did not resume correctly (the driver would timeout communicating
and fail).  This led to frustration as suspend/resume is a common use for
laptops.

Poking around, I applied the reset on resume quirk to this chipset and the
resume started working.  Reloading the xhci_hcd module had been the temporary
workaround.

If the bus loading is not high, the imx28 usb can work well at the most
of time. There is a IC errata for this problem, usually, we consider

for that, so we need to add them to stable tree 3.11+.

Commit 35773dac5f "usb: xhci: Link TRB
must not occur within a USB payload burst" attempted to fix an issue
found with USB ethernet adapters, and inadvertently broke USB storage
devices.  The patch attempts to ensure that transfers never span a
segment, and rejects transfers that have more than 63 entries (or
possibly less, if some entries cross 64KB boundaries).

usb-storage limits the maximum transfer size to 120K, and we had assumed
the block layer would pass a scatter-gather list of 4K entries,
resulting in no more than 31 sglist entries:

d6c9ea9069 xhci: Avoid infinite loop when sg urb requires too many trbs
Currently prepare_ring() returns -ENOMEM if the urb won't fit into a
single ring segment.  usb_sg_wait() treats this error as a temporary
condition and will keep retrying until something else goes wrong.

The number of retries should be limited in usb_sg_wait(), but also
prepare_ring() should not return an error code that suggests it might
be worth retrying.  Change it to -EINVAL.

If CONFIG_PCI is enabled, make sure xhci_cleanup_msix()
doesn't try to free a bogus PCI IRQ or dereference an invalid
pci_dev when the xHCI device is actually a platform_device.

This patch should be backported to kernels as old as 3.9, that
