Merge common RAID and LVM logic to an abstract diskfilter.

Add LDM support using the same framework.

	* Makefile.util.def (libgrubkern): Add grub-core/disk/ldm.c,
	grub-core/disk/diskfilter.c and grub-core/partmap/gpt.c.
	(libgrubmods): Remove grub-core/disk/raid.c and
	grub-core/partmap/gpt.c.
