Since commit 0998d06310
(device-core: Ensure drvdata = NULL when no driver is bound),
the driver core clears the driver data to NULL after device_release
or on probe failure. Thus, it is not needed to manually clear the
device driver data to NULL.

Reversing the dma_data_direction for pci_map_sg() friends is useful
for other drivers, so move it from tcm_qla2xxx into inline code
within target_core_fabric.h.
Also drop internal usage of equivlient in tcm_qla2xxx fabric code.

On any Adapter when we flash through FC/FCoE without card reset option it still
shows the Running FW version in Flashed FW version. This new interface will be
used by API to instruct the driver to update its cache versions.

Unless there is a need to observe them, the queue depth ramp up/down messages

Earlier IDC-TIME-EXTEND aen was a nop and ignored by driver.
We now have to handle the aen so that other protocol drivers can use time

The patch changes a specifier used to output some variables. Instead of using
stack for each byte the '%*ph[CN]' allows to take a one pointer and prints
entire buffer as a hexadecimal string with the separator ':' or ''.

Changing this attribute to 0 will mean that all initiators that login
to the target while the target has generate_node_acls=1 will see all
TPG LUNs, which may be the desired behavior in some cases.

Avoid that the fcport structure gets leaked if
bsg_job->request->msgcode == FC_BSG_HST_ELS_NOLOGIN, the fcport
allocation succeeds and the !vha->flags.online branch is taken.
This was detected by Coverity. However, Coverity does not recognize
that all qla2x00_process_els() callers specify either
FC_BSG_RPT_ELS or FC_BSG_HST_ELS_NOLOGIN in the field
bsg_job->request->msgcode and that the value of that field is not
modified inside that function. This results in a false positive
report about a possible memory leak in an error path for
bsg_job->request->msgcode values other than the two mentioned
values. Make it easy for Coverity (and for humans) to recognize
that there is no fcport leak in the error path by changing the
bsg_job->request->msgcode == FC_BSG_HST_ELS_NOLOGIN test into
bsg_job->request->msgcode != FC_BSG_RPT_ELS.

Change the 'rval' variable from QLA_FUNCTION_TIMEOUT into QLA_SUCCESS
before starting a loop that is only executed if rval is initialized

Coverity reports "Overrunning struct type ct_sns_req of 1228 bytes
by passing it to a function which accesses it at byte offset 8207"
for each qla2x00_prep_ct_req(), qla2x00_prep_ct_fdmi_req() and
qla24xx_prep_ct_fm_req() call. Help Coverity to recognize that
these calls do not trigger a buffer overflow by making it explicit
that these three functions initializes both the request and reply
structures. This patch does not change any functionality.

When we are intentionally generating a firmware dump by executing the
MBC_GEN_SYSTEM_ERROR command, the command actually times out. The normal
course of action when a mailbox command times out is to take a firmware dump.
However, in this special case we do not want to do this since the
MBA_SYSTEM_ERR AEN already generates a firmware dump.

This patch fixes a critical bug that was introduced in 3.9
related to VLAN tagging FCoE frames.
Comment from original 2012 patch:
In all our testing this function has never returned true. However, the
dropping of hardware_lock necessary to call this function seems to cause
a use-after-free we manage to hit rather frequently. Given this
cost-benefit ratio, I'm willing to remove some 100 lines of code.
And since the same problem exists around shutdown_sess and put_sess,
this patch changes them from taking the hardware_lock to requiring the
hardware_lock to be taken. In most cases the caller already had the
lock and had to drop it for the called method to reacquire it. At best
that hurts performance and in rare instances it causes races with fatal
consequences.
We dropped the original 2012 patch when upgrading our kernel and it took
us nearly half a year to discover we still need it.

Suppose an initiator sends a DATA IN command with an allocation length
shorter than the FC transfer length -- we get a target message like
TARGET_CORE[qla2xxx]: Expected Transfer Length: 256 does not match SCSI CDB Length: 0 for SAM Opcode: 0x12
In that case, the target core adjusts the data_length and sets
se_cmd->residual_count for the underrun. But now suppose that command
fails and we end up in tcm_qla2xxx_queue_status() -- that function
unconditionally overwrites residual_count with the already adjusted
data_length, and the initiator will burp with a message like
qla2xxx [0000:00:06.0]-301d:0: Dropped frame(s) detected (0x100 of 0x100 bytes).
Fix this by adding on to the existing underflow residual count instead.

The driver uses ha->mbx_cmd_flags variable to pass information between
its ISR and mailbox routines, however, it does so without the protection of
any locks. Under certain conditions, this can lead to multiple mailbox
command completions being signaled, which, in turn, leads to a false
mailbox timeout error for the subsequently issued mailbox command.
The issue occurs frequently but intermittenly with the Qlogic 8GFC mezz
card during card initialization, resulting in card initialization failure.

System crashes, in initiator mode operation, with
qla2xxx_copy_atioqueues() in stack trace when firmware dump is
attempted.
Check for atio_q_length alone does not indicate if atio_ring is
allocated, make explicit check of atio_ring to avoid the crash.

Currently,the driver is processes RSCNs for each new NPIV ports that is created.
Processing the RSCN includes a name server query to see what has changed at the
name server side. The name server query is performed by the physical port and
each virtual port on the physical adapter (since the RSCN is passed to each
virtual port for processing). As the number of virtual ports being created
increases, this causes a lot of traffic and busies the firmware.
Processing the RSCN for a virtual port we already have a priori knowledge of is
not necessary so check the 24-bit fabric ID of the RSCN entry and skip
processing it if the RSCN is for a virtual port we already know about.

Code in qla24xx_report_id_acquisition() incorrectly assumed that
upon completion of list iteration (with no match), the 'pos'
(vp) variable passed to list_for_each_entry() would be set to
NULL. In this context, if the firmware were to return an
unrecognized vp_idx, the follow-on assignments to vp-members
could result in corruption of the structure.

It seems that the sequence of a strcpy followed by a strlen confuses GCC
when it is keeping track of array bounds here. (It is not clear to me
which array triggers this warning and by how much GCC thinks the
subscript is above its bounds. Neither is it clear to me why comparable
code in these two functions doesn't trigger this warning.)
An easy way to silence these warnings is to use preprocessor macros and
strncpy, as that apparently gives GCC enough information to keep track
of array bounds.

Add the following error handling for loopback diagnostic mode with ISP83xx:
1. If we do not receive an MBA_DCBX_COMPLETE after our initial set port
configuration command, try to reset the port back into normal operation.
If that fails, take a FCoE dump and then reset the chip.
2. After completing the loopback diagnostic operation, if the reset of the port
back into normal operation fails then reset the port so we take a FCoE dump
and then reset the chip.
3. When we receive an IDC notification and the requested operation is loopback
extend the loop down timer so the link does not appear to down for an
extended period of time.

There's a subtle race in the loop/bus-reset handling whereby a
VHA's loop-state can get incorrectly set to 'down' after the
loop-reset and firmware's completion of link re-negotiation. The
original code incorrectly assumes that firmware AENs would arrive
only after mailbox-command execution to initiate the link-flap.
Here's a good case with the old code (AENs arrive after
mailbox-command completion):

All told, hch was able to shave off another ~1K LOC by killing the
se_subsystem_dev abstraction, along with a number of PR + ALUA
simplifications. Also, a nice patch by Roland is the refactoring of
MODE_SENSE handling, along with the addition of initial MODE_SELECT
emulation support for virtual backends.

config_group instead of pointers for config_group->default_group[]

When the qla2xxx driver loses access to multiple, remote ports, there is a race
condition which can occur which will keep the request stuck on a scsi request
queue indefinitely.
This bad state occurred do to a race condition with how the FCPORT_UPDATE_NEEDED
bit is set in qla2x00_schedule_rport_del(), and how it is cleared in
qla2x00_do_dpc(). The problem port has its drport pointer set, but it has never
been processed by the driver to inform the fc transport that the port has been
lost. qla2x00_schedule_rport_del() sets drport, and then sets the
FCPORT_UPDATE_NEEDED bit. In qla2x00_do_dpc(), the port lists are walked and
any drport pointer is handled and the fc transport informed of the port loss,
then the FCPORT_UPDATE_NEEDED bit is cleared. This leaves a race where the
dpc thread is processing one port removal, another port removal is marked
with a call to qla2x00_schedule_rport_del(), and the dpc thread clears the
bit for both removals, even though only the first removal was actually
handled. Until another event occurs to set FCPORT_UPDATE_NEEDED, the later
port removal is never finished and qla2xxx stays in a bad state which causes
requests to become stuck on request queues.
This patch updates the driver to test and clear FCPORT_UPDATE_NEEDED
atomically. This ensures the port state changes are processed and not lost.

FC transport on receiving bsg_job submission failure, calls bsg_job->job_done()
and sets the bsg_job->reply->result the returned value. In contrast, when the
success code (0) is returned fc transport doesn't call bsg_job->job_done() and
doesn't populate bsg_job->reply->result.

Pass the sense reason as an explicit return value from the I/O submission
path instead of storing it in struct se_cmd and using negative return
values. This cleans up a lot of the code pathes, and with the sparse
annotations for the new sense_reason_t type allows for much better
error checking.

It is possible for the target code to change the loop_id or s_id of a
target session in reaction to an FC fabric change. However, the
session structures are stored in tables that are indexed by these two
keys, and if we just change the session structure but leave the
pointers to it in the old places in the table, havoc can ensue. For
example, a new session might come along that should go in the old slot
in the table and overwrite the old session pointer.
To handle this, add a new tgt_ops->update_sess() method that also
updates the "by loop_id" and "by s_id" lookup tables when a session
changes, so that the keys where a session pointer is stored in these
tables always matches the keys in the session structure itself.

My draft of SPC-4 says the following about the SCSI name string in
inquiry VPD page 83h:
The SCSI NAME STRING field starts with either:
a) the four UTF-8 characters 'eui.' concatenated with 16, 24, or
32 hexadecimal digits (i.e., the UTF-8 characters 0 through 9
and A through F) for an EUI-64 based identifier (see
7.8.6.5). The first hexadecimal digit shall be the most
significant four bits of the first byte (i.e., most significant
byte) of the EUI-64 based identifier;
b) the four UTF-8 characters 'naa.' concatenated with 16 or 32
hexadecimal digits for an NAA identifier (see 7.8.6.6). The
first hexadecimal digit shall be the most significant four bits
of the first byte (i.e., most significant byte) of the NAA
identifier; or
c) the four UTF-8 characters 'iqn.' concatenated with an iSCSI
Name for an iSCSI-name based identifier (see iSCSI).
However, the .tpg_get_wwn method for tcm_qla2xxx formats the WWN so
the SCSI name string looks like "52:4a:93:7d:24:5f:b2:12,t,0x0001".
This patch corrects the code so that VPD 83h gives a SPC-compliant
SCSI name string like "naa.524a937d245fb212,t,0x0001" while leavig
other uses alone (so configfs will still work with ':' separated WWNs).

"This is workqueue updates for v3.7-rc1. A lot of activities this
round including considerable API and behavior cleanups.
* delayed_work combines a timer and a work item. The handling of the
timer part has always been a bit clunky leading to confusing
cancelation API with weird corner-case behaviors. delayed_work is
updated to use new IRQ safe timer and cancelation now works as
expected.
* Another deficiency of delayed_work was lack of the counterpart of
mod_timer() which led to cancel+queue combinations or open-coded
timer+work usages. mod_delayed_work[_on]() are added.
These two delayed_work changes make delayed_work provide interface
and behave like timer which is executed with process context.
* A work item could be executed concurrently on multiple CPUs, which
is rather unintuitive and made flush_work() behavior confusing and
half-broken under certain circumstances. This problem doesn't
exist for non-reentrant workqueues. While non-reentrancy check
isn't free, the overhead is incurred only when a work item bounces
across different CPUs and even in simulated pathological scenario
the overhead isn't too high.
All workqueues are made non-reentrant. This removes the
distinction between flush_[delayed_]work() and
flush_[delayed_]_work_sync(). The former is now as strong as the
latter and the specified work item is guaranteed to have finished
execution of any previous queueing on return.
* In addition to the various bug fixes, Lai redid and simplified CPU
hotplug handling significantly.
* Joonsoo introduced system_highpri_wq and used it during CPU
hotplug.
There are two merge commits - one to pull in IRQ safe timer from
tip/timers/core and the other to pull in CPU hotplug fixes from
wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."
Fixed a number of trivial conflicts, but the more interesting conflicts
were silent ones where the deprecated interfaces had been used by new
code in the merge window, and thus didn't cause any real data conflicts.

Stale information in the temporary fcport created in
qla2x00_configure_local_loop() causes qla2x00_get_port_database() call
to fail. This reschedules scan, which gets stuck continuously in the
rescheduling-of-scan loop due to the failure.

The qla2xxx firmware actually expects the task management response
code in a CTIO IOCB with SCSI status mode 1 to be in little-endian
byte order, ie the response code should be the first byte in the
sense_data[] array. The old code erroneously byte-swapped the
response code, which puts it in the wrong place on the wire and leads
to initiators thinking every task management request succeeds (since
they see 0 in the byte where they look for the response code).

Every fabric driver has to supply a se_tfo->set_fabric_sense_len()
method, just so iSCSI can return an offset of 2. However, every fabric
driver is already allocating a sense buffer and passing it into the
target core, either via transport_init_se_cmd() or target_submit_cmd().
So instead of having iSCSI pass the start of its sense buffer into the
core and then later tell the core to skip the first 2 bytes, it seems
easier for iSCSI just to do the offset of 2 when it passes the sense
buffer into the core. Then we can drop the se_tfo->set_fabric_sense_len()
everywhere, and just add a couple of lines of code to iSCSI to set the
sense data length to the beginning of the buffer right before it sends
it over the network.

use the sync flushes at all and they're going away.
This patch doesn't make any functional difference.

The PCIE capability offset is saved during PCI bus walking. It will
remove an unnecessary search in the PCI configuration space if this
value is referenced instead of reacquiring it. Also, pci_is_pcie is a
better way of determining if the device is PCIE or not (as it uses the
same saved PCIE capability offset).

We want it to be possible for target_submit_cmd() to return errors up
to its fabric module callers. For now just update the prototype to
return an int, and update all callers to handle non-zero return values
as an error.
This is immediately useful for tcm_qla2xxx to fix a long-standing active
I/O session shutdown race, but tcm_fc, usb-gadget, and sbp-target the
fabric maintainers need to check + ACK that handling a target_submit_cmd()
failure due to session shutdown does not introduce regressions

Since we set se_session.sess_tearing_down and stop new commands from
being added to se_session.sess_cmd_list before we wait for commands to
finish when freeing a session, there's no need for a separate
sess_wait_list -- if we let new commands be added to sess_cmd_list
after setting sess_tearing_down, that would be a bug that breaks the
logic of waiting in-flight commands.
Also rename target_splice_sess_cmd_list() to
target_sess_cmd_list_set_waiting(), since we are no longer splicing
onto a separate list.

Now that target_submit_cmd() / target_get_sess_cmd() check
sess_tearing_down before adding commands to the list, we no longer
need the check in qlt_do_work(). In fact this check is racy anyway
(and that race is what inspired the change to add the check of

The only place that sets qla_tgt_sess.tearing_down calls
target_splice_sess_cmd_list() immediately afterwards, without dropping
the lock it holds. That function sets se_session.sess_tearing_down,
so we can get rid of the qla_target-specific flag, and in the one
place that looks at the qla_tgt_sess.tearing_down flag just test
se_session.sess_tearing_down instead.

If we make a variable an unsigned int and then expect it to be < 0 on
a bad character, we're going to have a bad time. Fix the tcm_qla2xxx

If we go to the "out_term:" exit path in qlt_do_work(), we call
qlt_send_term_exchange() with a NULL cmd, which means that it can't
possibly free the cmd for us. Add an explicit call to free the
command memory, so we don't leak the allocation.
This will also fix warnings about "BUG qla_tgt_cmd_cachep: Objects
remaining on kmem_cache_close" from slub when unloading the qla2xxx
target module.

In qlt_do_ctio_completion(), there's no point in calling
qlt_term_ctio_exchange() with a NULL cmd -- all that it does is crash
in a NULL pointer dereference, since it does
qlt_send_term_exchange(vha, cmd, &cmd->atio, 1);
and dereferencing &cmd->atio is a bad idea if cmd itself is NULL.
If we really need to do this, we could take the values from the
failed CTIO we're processing, but it's not clear if it's worth
the replumbing to do that.

When we create an explicit node ACL in tcm_qla2xxx_make_nodeacl(),
there is a call to tcm_qla2xxx_setup_nacl_from_rport(), which puts the
node ACL into the lport_fcport_map even though there is no session yet
for the initiator. Since the only time we remove entries from this
map is when we free a session, this means that if we later delete this
node ACL without the initiator ever creating a session, we'll leave
the nacl pointer in the btree pointing at freed memory.
This is especially bad if that initiator later does send us a command
that would cause us to create a dynamic ACL and session: we'll find
the stale freed nacl pointer in the btree and end up with use-after-free.
We could add more code to clear the btree entry when deleting the
explicit nacl, but the original insertion is pointless: without a
session attached, we'll just have to update the entry when a session
appears anyway. So we can just delete tcm_qla2xxx_setup_nacl_from_rport()
and the code that calls it.

This patch adds a new tcm_qla2xxx_clear_sess_lookup() call to clear session
specific s_id + loop_id entries used for se_node_acl pointer lookup ahead
of releasing se_session within the process context workqueue callback in
tcm_qla2xxx_free_session().
It makes the call in existing tcm_qla2xxx_clear_nacl_from_fcport_map()
code invoked from qlt_unreg_sess() in interrupt context w/ hardware_lock
held, ahead of the process context callback into qlt_free_session_done()
-> tcm_qla2xxx_free_session().
We are doing this to address a race between incoming ATIO or TMR packets
using stale se_node_acl pointer once session shutdown has been invoked via
qlt_unreg_sess() in qla_target.c LLD code, and when the entire tcm_qla2xxx
endpoint has not been forced into shutdown w/ echo 0 > ../$QLA2XXX_PORT/enable

se_session->sess_kref in order to ensure that qla_hw_data->hardware_lock
can be held while calling qlt_unreg_sess() for the final put.

In the original code, if dma_pool_alloc() fails then we call
dma_pool_free(). It causes an error, possibly a NULL dereference.

This patch adds support for tcm_qla2xxx fabric module for target-core
using the new qla_target.c LLD logic. This includes support for explict
NodeACLs via configfs using tcm_qla2xxx_setup_nacl_from_rport() from libfc
struct fc_host->rports, and demo-mode support for virtual LUN=0 access.
This patch also adds support for using tcm_qla2xxx_lport->lport_fcport_map
and ->lport_loopid_map of btree_head32 to track struct se_node_acl pointers
for individual 24-bit Port ID and 16-bit Loop ID values w/ qla_target_template
->find_sess_by_s_id() and ->find_sess_by_loop_id() used in a number of
locations into the primary I/O dispatch logic in qla_target.c LLD code.
The main piece for FC Nexus setup is in tcm_qla2xxx_check_initiator_node_acl(),
which calls tcm_qla2xxx_set_sess_by_[s_id,loop_id]() to setup our
lport->lport_fcport_map and lport_loopid_map pointers respectively, and
register the new nexus with TCM via __transport_register_session().

Add LLD target mode for >= 24xx series HW. This code was originally based on
external qla2x00t module based on 8.02.01-k4, and has been refactored to
push the bulk of code into mainline qla2xxx.ko LLD -> qla_target.c.
The implementation uses internal workqueues for I/O context submission
into tcm_qla2xxx code, and includes the struct qla_tgt_func_tmpl API for
external interaction to allow qla2xxx LDD to function without direct
target-core dependencies:
It also enables qla_target.c usage within existing qla2xxx LLD code.
This includes:
*) Addition of target mode specific members to existing data
structures in qla_def.h and struct qla_hw_data->tgt_ops using
qla_target.h:struct qla_tgt_func_tmpl
*) Addition of struct qla_tgt_func_tmpl and direct calls into
qla_target.c logic w/ qlt_* prefixed functions.
*) Addition of qla_iocb.c:qla2x00_req_pkt() for ring processing, and
qla2x00_issue_marker() for handling request/response queue processing
for target mode operation
*) Addition of various qla_tgt_mode_enabled() logic checks in
qla24xx_nvram_config(), qla2x00_initialize_adapter(), qla2x00_rff_id(),
qla2x00_abort_isp(), qla24xx_modify_vp_config(), and
qla2x00_vp_abort_isp().
By default the new qlini_mode module parameter is setting initiator-mode
to 'enabled' in order for 'modprobe qla2xxx' to continue to function as
expected in initiator only mode. Enabling target-mode operation will
currently require a:
modprobe qla2xxx qlini_mode="disabled"
in order to explictly disabled initiator mode and allow target-mode
to be enabled via tcm_qla2xxx configfs fabric callers.

The field vp_idx in struct fc_port is a redundant/mirror copy of
the same field in struct scsi_qla_host;
struct fc_port has a pointer vha to scsi_qla_host which allows
the original copy of vp_idx to be readily accessed.

If interrupt registration failed we could crash the machine as we were trying
to deference some pointers which weren't allocated yet. Move the allocation
a little earlier and make some checks to the free resource code to make sure
that we don't try to free a resource that was never allocated.

In case of firmmware detected under-run condition and scsi status of
task_set_full or busy_condition, return that to the mid layer for proper error
handling instead of DID_ERROR (which causes error handler activation and a
full retry).
