AMD's reference BIOS code had a bug that could result in the
firmware failing to reenable the iommu on resume. It
transpires that this causes certain less than desirable
behaviour when it comes to PCI accesses, to whit them ending
up somewhere near Bristol when the more desirable outcome
was Edinburgh. Sadness ensues, perhaps along with filesystem
corruption. Let's make sure that it gets turned back on,
and that we restore its configuration so decisions it makes
bear some resemblance to those made by reasonable people
rather than crack-addled lemurs who spent all your DMA on
Thunderbird.

This patch adds a workaround for an IOMMU BIOS problem to
the AMD IOMMU driver. The result of the bug is that the
IOMMU does not execute commands anymore when the system
comes out of the S3 state resulting in system failure. The
bug in the BIOS is that is does not restore certain hardware
specific registers correctly. This workaround reads out the
contents of these registers at boot time and restores them
on resume from S3. The workaround is limited to the specific
IOMMU chipset where this problem occurs.

This patch moves the setting of the configuration and
feature flags out out the acpi table parsing path and moves
it into the iommu-enable path. This is needed to reliably
fix resume-from-s3.

this patch might sleep.

The data that was stored in this table is now available in
dev->archdata.iommu. So this table is not longer necessary.
This patch removes the remaining uses of that variable and
removes it from the code.

This patch cleans up the code to flush device table entries
in the IOMMU. With this chance the driver can get rid of the
iommu_queue_inv_dev_entry() function.

This patch introduces a list to each protection domain which
keeps all devices associated with the domain. This can be
used later to optimize certain functions and to completly
remove the amd_iommu_pd_table.

This patch adds a reference count to each device to count
how often the device was bound to that domain. This is
important for single devices that act as an alias for a
number of others. These devices must stay bound to their
domains until all devices that alias to it are unbound from
the same domain.

This patch changes IOMMU code to use dev->archdata->iommu to
store information about the alias device and the domain the
device is attached to.
This allows the driver to get rid of the amd_iommu_pd_table
in the future.
